Adjustment Patterns of Illness Process of People with Hemophilia in Korea

Purpose

The purpose of this study was to explore adjustment pattern of illness process of people with hemophilia in Korea.

Method

23 people with hemophilia had participated for this study. The data were collected through in-depth interviews and analyzed using Strauss & Corbin's grounded theory method.

Result

“would be free from” was emerged as a core category and it reflects that all participants wanted to be free from the constraints of the disease. The adjustment process was categorized into two stage, the ‘ unstable stage’ and the ‘ stable stage’. In the process of “would be free from” four different patterns were identified: hopelessness type; appreciation type; challenge type; and transcendence type. These types were identified based on the degree of pursuing normal life and managing the disease, and social support. The most frequently occurring type was hopelessness type but the participants of this type suffered the most. The transcendence type was the most ideal type, but it occurred the least.

Conclusion

The results of this study indicate that people with hemophilia in Korea still suffer from the disease and they need supports. The results would be useful for health care professionals in establishing education and counseling program for the people with hemophilia.

Immersion Experience of the Cyber World of Adolescents

Purpose

The purpose of this research was to explore the cyber world immersion experience of adolescents.

Method

Multiple strategies for data collecting were used: an in depth face-to-face interview; analysis of adolescent` writings; and analysis of examples of phenomenon in the realistic world. The sample group consisted of 10 adolescents.

Result

Although the experience was different for all adolescent interviewed, the essential themes of experience emerged: “fill up”, “homoeologous feeling”, “the older generation has a conflicting negative opinion”, “change in social character”, “become habitually skeptic”, “have bad health”, “mean of superiority and getting everything solved”, “ease of access”, “monetary benefit”, “addiction to the computer”, “forget real life solved stress”, “do harm to society”, “take comfort”, or “new job”.

Conclusion

Accordingly this paper suggests that contact with various software is necessary in adolescents, and good quality contents function to prepare and activate adolescents to apply the internet for good use.

The Study for Development of Day Hospital Program of Eating Disorders

Purpose

The purpose of this study was to develop the day hospital program for Korean eating disorders patients and to examine the effect of the day hospital program on improving frequency of binging and purging, eating disorders symptoms, self-esteem, and depression.

Method

The subjects were 24 binge eaters visited at eating disorders clinic “M”. They participated in a modified day hospital program based on the Toronto Day Hospital Program. All subjects completed the Eating Disorders Inventory(EDI)-2, Rosenberg Self-Eesteem Scale, Beck Depression Inventory(BDI) at pre and post intervention, and recorded daily food records.

Result

In paired t-test analysis, frequency of binging and purging, self-esteem, depression, and 11 sub-scales of EDI-2(drive for thinness, bulimia, body dissatisfaction, ineffectiveness, interoceptive awareness, perfectionism, interpersonal distrust, maturity fear, asceticism, impulse regulation, social insecurity) were significantly improved after treatment.

Conclusion

The results of this study suggest that the day hospital program for Korean eating disorders patients may be an effective initial approach to overcoming various eating disorders symptoms. Further studies should investigate longer term outcome data using a larger sample.

Secondary Analysis: Focusing on Qualitative Research

Purpose

This article explored issues arising from secondary analysis of qualitative data and analyzed the results of qualitative secondary analysis studies published in English, focusing on the methodological aspects.

Method

A total of 29 studies were identified as qualitative secondary analysis studies, retrieving from the CINAHL database from 1982 to 3rd week of April 2003. These studies were analyzed by publication year, research method, and type of approach to secondary analysis.

Result

The year that the qualitative secondary analysis study first published was 1992 and the number of the studies using secondary analysis has increased after the middle of 1990s. Grounded theory was the one the most frequently used(n=11, 37.9%) and phenomenological study the second most(n=6, 20.7%). In terms of types of approach, fifteen studies(51.7%) focused on the specific concepts that were not explored in the primary studies. Nine(31.0%) focused on the specific types of participants. Six were aimed to integrate contexts or perspectives to generate more general and abstract analysis of qualitative data.

Conclusion

The results of this article will stimulate methodological discussion of qualitative secondary analysis and activate qualitative studies using secondary analysis.

The Influencing Factors on Alienation in High School Students

Purpose

This study was performed to identify the influencing factors on alienation among high school students.

Method

Data was collected by questionnaires from 550 students of academic and vocational high schools in G city. The data was analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression.

Result

The scores of alienation among students in financially lower middle class and lower class were higher than those of the upper middle class students, resulting in significant differences(F=6.87, p=.00). A sense of alienation showed a significantly negative correlation with the scores of responding parenting style(r=-.32), family cohesion(r=-.33), school attachment(r=-.51), academic performance(r=-.34), peer relationships(r=-.38), self-control (r=-.43), and social skills(r=-.33). The most powerful predictor of alienation among high school students was school attachment and the variance explained was 26%. A combination of school attachment, self control, peer relationships, family cohesion, demanding parenting style, and academic performance account for 40% of the variance in alienation among high school students.

Conclusion

This study suggests that school attachment, self control, peer relationships, family cohesion, demanding parenting style, and academic performance are significant influencing factors on alienation in high school students. Therefore, nursing strategy is needed to manage these revealed factors.

Clinical trial of Aromatherapy on Postpartum Mother's Perineal Healing

Purpose

This study was designed to verify the effect of aromatherapy on a postpartum mother's perineal healing.

Method

The research design was a clinical trial. The methods of aromatherapy were applied sitz bath or soap application using essential oils with Lavender, Myrrh, Neroli, Rose, Grapefruit, Mandarin, Orange, and Roman Chamomile. The subjects of this experiment were postpartum mothers who delivered vaginally with an episiotomy. They were allocated to one of three groups; the aroma-sitz bath group, aroma-soap application group or control group. To evaluate the effect of aromatherapy, the perineal healing status was measured using the REEDA scale and smears of episiotomy wound were obtained. The data were analyzed by repeated measures of ANOVA, ANCOVA, χ2-test, and multiple response analysis via SPSS program.

Result

The REEDA scale was significantly low in the experimental group at postpartum 5th and 7th days (P=.009, P=.003), respectively. Most were observed ‘ few’(5-10 bacteria per field) bacteria in the smears of episiotomy wound. The one bacteria was identified in the 50.8% of subjects in pretest and two bacteria in the 60.3% in posttest. Most frequently identified bacteria were Escherichia coli and Enterococcus faecalis.

Conclusion

In conclusion, these findings indicate that postpartum aromatherapy for perineal care could be effective in healing the perineum perineal care could be effective in healing the perineum.

The Influencing Factors of the Compliance Level with Therapeutic Regimen after the Bone Mineral Densitometry

Purpose

This study was to identify the influencing factors of the compliance level to a therapeutic regimen after a bone mineral densitometry test.

Method

The sample for the study was 95 people who took the bone mineral densitometry test from March, 2002 to July, 2002. Data was collected by mail using aself reporting questionnaire on the selected variables such as the compliance level, self efficacy, health locus of control, susceptibility, severity, usefulness, barrier, and self esteem.

Result

The average compliance level was 63.93. Through multiple regression, three independent variables including chance health locus of control on personality, the result of bone mineral density and self-efficacy were entered in the model as the significant determinants of the compliance level after a bone mineral densitometry test. The coefficients of determination of each variable were 10.9%, 8.3% and 8.1% respectively.

Conclusion

The identification of the determinants of the compliance level to the therapeutic regimen after bone mineral densitometry is expected to contribute to the development of an intervention program to improve the compliance level to the therapeutic regimen in osteoporosis patients.

Effects of a Constipation Intervention Program on Inpatients' Defecation

Purpose

The purpose of this study was to evaluate the effect of a constipation reduction program for inpatients.

Method

Subjects were selected in one medical ward of C University Hospital from May, 2001 to November, 2001. Twenty-nine subjects were assigned to an experimental group and 32 subjects to a control group. Data related to the frequency of defecation and to the length and amount of laxative drugs used was collected by a medical record review and data on the degree of constipation was obtained by a self-report using a constipation assessment scale.

Result

More than 90% of the subjects admitted in the department of neurology and one third of total subjects presented with activity limitation and about one fourth of the subjects were fed with a nasogastric tube. There was a significant difference in the degree of constipation, frequency of defecation, and the length and amount of laxative drug use between the two groups.

Conclusion

This program is effective in inpatient's constipation reduction. Further studies need to apply this program in various clinical environments and properly use this program in different clinical settings.

Development of a Sublimation Program for Korean Adolescents' Aggression

Purpose

The purpose of this study was to identify a path diagram for the influence of family, personality, sexual abuse, drug abuse, coping strategies, and aggressive impulsiveness on aggression, and to develop a sublimation program for Korean adolescent's aggression.

Method

Data was collected by self-report questionnaires. Subjects consisted of 2,111 adolescents. A proportional stratified random sampling method was used. The major instrument was the Mental Health Questionnaire for Korean Adolescents, and the Cronbach's Alpha ranged from .54 to .95 for each subscale. Statistical methods were Chi-square, correlation analysis, and path analysis.

Result

The strongest contributing variables on aggression were person-related aggressive impulsiveness, antisocial personality, self-injured aggressive impulsiveness, gender, sexual abuse, psychosomatic symptoms II, drug abuse, age, parent-child relationship, alcohol abuse and cognitive avoidance coping strategies in the order named. Also the author developed a multi-systemic sublimation program for Korean adolescents's aggression. The multi-systemic sublimation program involves four domains including adolescents, parents, peers and community, and has several therapeutic sub-programs for each domain.

Conclusion

The ecology of human development is composed of multiple, integrated levels of organization, including biological, individual-psychological, social-interpersonal, cultural, and historical levels. Therefore, this multi-systemic sublimation program will prevent and decrease the rate of aggressive behavior among Korean adolescents.

Situational Meaning and Maternal Self-esteem in Mothers with High Risk Newborn

Purpose

This study was designed to explore the relationship of situational meaning with maternal self-esteem in mothers with high risk newborn.

Method

The subjects of this study were 82 mothers with high risk newborn. Data were collected using a translated Family Meaning Attribution Scale and Maternal Self-Report Inventory. Data were analyzed using descriptive statistics, t-test, Pearson Correlation Coefficients and Stepwise Multiple Regression.

Result

The average score of the situational meaning in high risk newborn mothers was 64.01(possible score is between 0-96) and the average score of each item was 1.98. The average score of the maternal self-esteem in high risk newborn mothers was 81.96(possible score is between 26-104) and the average score of each item was 3.15. No significant differences were found in situational meaning according to general characteristics except whether it was a planned pregnancy or not. No significant differences were found in maternal self-esteem according to general characteristics except disease or admission experience during pregnancy. There was significant positive correlation between situational meaning and maternal self-esteem.

Conclusion

It is necessary for nurses to provide high risk newborn mothers with care for improving situational meaning that is attributed to the mothers. It can be helpful to improve maternal self-esteem and in the end it will facilitate the maternal transition in mothers with high risk newborn.

The Relationships of Internet Addiction, Depression, and Suicidal ideation in Adolescents

Purpose

This study was done to identify the state of internet addiction and to investigate the relationships among internet addiction, depression and suicidal ideation in adolescents.

Method

Participants were 1,670 high school students in an urban city. The Internet Addiction Scale was used to measure internet addiction, the DISC-MDD-SQ was used to measure depression, and the Suicidal Ideation Questionnaire-JR was used to measure suicidal ideation.

Result

38.1% of participants were perceived to be in the early stages of internet addiction and 1.5% reported heavy addiction. Overall the internet addiction scale score was 37.57(±12.52). The differences of internet addiction in sample characteristics were found to be significant in rank in class. The differences in DISC-MDD-SQ and IAS scores among the states of internet addiction were statistically significant. The level of internet addiction correlated positively to the level of depression and suicidal ideation.

Conclusion

Based upon these findings, there were significant positive correlations among Internet addiction, depression, and suicidal ideation in adolescents. As schools are often the first line for the identification of potential life-threatening behaviors, nurses, especially school-based health professionals, need to be cognizant of measures and procedures for the assessment of Internet addiction, depression, and suicidal ideation.

Health Problems and Support from the Health & Welfare Service in the Elderly at Home

Purpose

This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home.

Method

Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003.

Result

Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ ADL 1(Independence)’ 9.87, ‘ ADL 2(Partial independence)’ 12.78, ‘ ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘ meals on wheels’ 40.7%, ‘ home helper’ 38.2%, ‘ visiting of social welfare worker’ 21.0%, ‘ physical therapy’ 19.6%, ‘ day care center’ 12.3%, ‘ volunteer's service’ 9.9%, ‘ home visiting care’ 3.7%, ‘ occupational therapy’ 3.7%, and ‘ speech therapy’ 2.5%.

Conclusion

The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.

The Effects of a Stretching Exercise Program in Elderly Women

Purpose

This study was aimed to identify the effects of a ten-week stretching exercise program on physiological, psychological functions, and activities of daily living(ADL) among elderly women.

Method

Using a quasi-experimental design, the experimental group received a ten-week stretching exercise program 3 times a week from March to May in 2002. They were divided into 22 persons in the experimental groups in 2 halls where the program was incorporated, and 22 persons in the control group in 2 halls, where the program wasnot incorporated. Data was analyzed with descriptive statistics, the chi-square test, Fisher's Exact test, and ANCOVA was considered significant as a 2-tailed test.

Result

There was a significant improvement in diastolic blood pressure(P=0.023), total cholesterol (P=0.019), triglycerides (P=0.002), spine ROM(P=0.000), trunk and hip-joint ROM(P=0.000), percent of body fat(P=0.039) as physiological functions, depression(P=0.041) as a psychological function, and activities of daily living(P=0.001) in the experimental group compared to the control group.

Conclusion

A stretching exercise program showed good effects on improving physiological functions, psychological functions and activities of daily living among the elderly women in a city. Therefore, we recommend this program be utilized as a health promoting program for the elderly in the community.

The Effect of Oral Health Education on Oral Health in Kindergarten Children

Purpose

This study was performed to investigate the effects of oral health education on kindergarten children.

Method

The study was designed as a quasi-experimental, nonequivalent control group pre -post test design. Data was collected from April 1st to November 30th, 2001. The total samples were sixty seven healthy kindergarten children; the experiment group consisted of thirty three and the control group was thirty four children. The experimental group received an oral health education program daily for 20 minutes for four weeks.

Result

There was significant improvements on oral health behavior in the experimental group. Streptococcus mutans and lactobacilli of the salivary variables in the experimental group were significantly higher than the control group. The dmft was lower in the experimental group than the control group, but there was no significant difference between the two groups. However it was significantly lower in the experimental group than control group over time.

Conclusion

Oral health education for kindergarten children showed an increase in the use of tooth paste and practicing correct tooth brush usage. Also, it decreased the rate of eating cariogenic food and had a positive effect on oral health through suppressing dental cavities.

A Study on Regular Cervical Cancer Screening Behavior among Middle-aged Women

Purpose

To identify the status of regular cervical cancer screening practices among middle-aged women, the associations between regular practice and research factors, and the predictive model and factors effecting such behavior was studied.

Method

Two hundred women, aged 40 to 60, were selected by convenience in one urban area of Seoul. They were asked about their regular attendance for screening, knowledge of cancer and screening, health belief, health self-determination index and certain personal factors.

Result

Approximately 54.5% of the women had periodic screening tests every 6 months to 2 years. Their knowledge of cervical cancer and health belief were at the medium level of each scale, but their health self-determination scores (HSDI) were low. Some influencing factors, and their cancer odds ratio were identified through univariate regression analysis. These variables were included in a predictive model, and this model proved to have enough fit and classification power (83.5%). In this model, the financial state, self-belief and self-determination scores were found to be significant.

Conclusion

Middle-age women's intrinsic motivation for healthy behavior was found to be low in those who felt to be in a poor financial state, had higher perceived barriers, lower perceived benefits and a lower prevalence of undergoing regular screening test.

Effect of DHEA on Hindlimb Muscles in a Focal Cerebral Ischemia Model Rat

Purpose

The purpose of this study was to determine the effect of DHEA on hindlimb muscles(soleus, plantaris and gastrocnemius) in a focal brain ischemia model rat.

Method

Twenty-seven male Sprague-Dawley rats were randomly divided into three groups: CINS(cerebral ischemia + normal saline), CIDH(cerebral ischemia + DHEA), or SHNS(sham + normal saline). Both the CINS and CIDH groups underwent a transient right middle cerebral artery occlusion operation. In the SHNS group, a sham operation was done. 0.34mmol/kg DHEA was administered daily by an intraperitoneal injection for 7days.

Result

The muscle weight, muscle fiber cross-sectional area of the Type I muscle fiber of soleus and Type II muscle fiber of plantaris and gastrocnemius, myofibrillar protein content of gastrocnemius, and muscle strength in the CINS group decreased compared with the SHNS group. The muscle weight, muscle fiber cross-sectional area of the Type II muscle fiber of plantaris and gastrocnemius, myofibrillar protein content of soleus, and muscle strength in the CIDH group increased compared with the CINS group.

Conclusion

It was identified that muscle atrophy could be induced during 7 days after a cerebral infarction, and DHEA administration during the early stages of a cerebral infarction might attenuate muscle atrophy.

The Concept Development of Smoking Temptation

Purpose

Smoking temptation is communicated frequently among people related to smoking cessation and has specific meanings within the context of smoking. However, the concept of smoking temptation has not been well articulated in the literatures. This study was conducted to clarify and to conceptualize the phenomena of smoking temptation.

Method

The Hybrid Model of concept development was applied to develop a concept of smoking temptation, which included a field study carried out in Seoul, South Korea using in-depth interviews with 5 adult smokers.

Result

The concept of smoking temptation emerged as a complex phenomenon having meanings in several different dimensions which encompassed several attributes. In addition, the conceptual structure of smoking temptation centered around five dimensions: Response to conditioned stimuli, Distancing from harsh reality, Nicotine dependency, Habitual routine, and Lack of control.

Conclusion

Smoking temptation is a concept that needs to be treated in a specified individual way and it is possible to enrich the meanings and methods to manage smoking temptation in nursing smoking cessation interventions that its application may have positive impacts on patients' well being.

Prediction on the Negative Outcomes of Anger in Female Adolescents

Purpose

This study was designed to construct a structural model for explaining negative outcomes of anger in female adolescents.

Method

Data was collected by questionnaires from 199 female adolescents ina female high school in Seoul. Data analysis was done with SAS for descriptive statistics and a PC-LISREL Program for Covariance structural analysis.

Result

The fit of the hypothetical model to the data was moderate, thus it was modified by excluding 7 paths and adding free parameters to it. The modified model withthe paths showed a good fit to the empirical data(χ2 =5.62, p=.69, GFI=.99, AGFI=.97, NFI=.99, NNFI=1.01, RMSR=.02, RMSEA=.00). Trait anger, state anger, and psychosocial problems were found to have a significant direct effect on psychosomatic symptoms. State anger, psychosocial problems, and learning behaviorswere found to have direct effects on depression of female adolescents.

Conclusion

The derived modelis considered appropriate for explaining and predicting negative outcomes of anger in female adolescents. Therefore, it can effectively be used as a reference model for further studies and is a suggested direction in nursing practice.

Health Problems and Health Behaviors of Preschoolers

Purpose

This study was conducted to understand the health status and health behaviors of preschoolers to provide baseline data for developing health promotion programs.

Method

Parents of children attending day-care centers were recruited for the study. Participating day-care centers were selected using a stratified sampling method. Data was collected from June to August 2002 using a questionnaire.

Result

Among 754 preschoolers, 17.3% were overweight, while 18.2% were underweight. The most frequent infectious diseases that children have had previously were hand-foot-mouth disease (20.2%) and chicken pox (18.7%). Current health conditions that children have frequently are respiratory disease (28%) and atopic dermatitis (23.8%). Only 61% brush their teeth everyday at bedtime, 54.3% wash their hands every time after returning home, 8.8% wear bicycle helmets, 9.3% use a child car seat, and 8.1% eat fruits and vegetables five times a day. Children residing in the metropolitan area were more likely to have positive health behaviors, and children of parents with an advanced college level education were more likely to have positive health behaviors than those with only a high school level education.

Conclusion

Based on the study results, health professionals could plan and develop health promotion programs to change unhealthy behaviors of preschoolers targeting high-risk groups.

Economic Evaluation of Visitng Nurse Services for the Low-income Elderly with Long-term Care Needs

Purpose

This study aimed to evaluate economic viability of public health center visiting nurse services for the low-income elderly with long-term care needs.

Method

The sample consisted of 252 community dwelling elderly who enrolled in public health center visiting nurse services for three months or more. Data was collected on physical (ADL and IADL) and cognitive impairments of the elderly, contents and frequency of visiting nurse services, cost per visit, and costs of alternative services for long-term care.

Result

The mean score of ADL and IADL levels of the elderly was 2.80.4904, which indicated these patients were mostly independent. Eighty four percent of the elderly subjects were cognitively intact. Among visiting nurse services supplied, providing assessment was 34%, followed by education and counseling 26%, medication 22%, and referral. The mean cost per visit was 17,824.1 won, which transformed into a total cost per person per year of 161,130.2 won. Comparing the cost of a visiting nurse service with those of other long-term care alternatives, the visiting nurse service was the least costly alternative, followed by an outpatient clinic, hospital based home care, and nursing home.

Conclusion

Overall, the results of the study provide evidence of the economic viability of visiting nurse services for the low-income elderly among long-term care alternatives.
