Abstract

Purpose
This study aimed to evaluate the use of the respiratory rate oxygenation (ROX) index, ROX-heart rate (ROX-HR) index, and saturation of percutaneous oxygen/fraction of inspired oxygen ratio (SF ratio) to predict weaning from high-flow nasal cannula (HFNC) in patients with respiratory distress in a pediatric intensive care unit.
Methods
A total of 107 children admitted to the pediatric intensive care unit were enrolled in the study between January 1, 2017, and December 31, 2021. Data on clinical and personal information, ROX index, ROX-HR index, and SF ratio were collected from nursing records. The data were analyzed using an independent t-test, χ² test, Mann–Whitney U test, and area under the curve (AUC).
Results
Seventy-five (70.1%) patients were successfully weaned from HFNC, while 32 (29.9%) failed. Considering specificity and sensitivity, the optimal cut off points for predicting treatment success and failure of HFNC oxygen therapy were 6.88 and 10.16 (ROX index), 5.23 and 8.61 (ROX-HR index), and 198.75 and 353.15 (SF ratio), respectively. The measurement of time showed that the most significant AUC was 1 hour before HFNC interruption.
Conclusion
The ROX index, ROX-HR index, and SF ratio appear to be promising tools for the early prediction of treatment success or failure in patients initiated on HFNC for acute hypoxemic respiratory failure. Nurses caring for critically ill pediatric patients should closely observe and periodically check their breathing patterns. It is important to continuously monitor three indexes to ensure that ventilation assistance therapy is started at the right time.

서론

1. 연구의 필요성

호흡곤란은 가스교환 기능에 장애가 생겨 충분한 산소화가 유지되지 않거나 환기가 되지 않는 상태로 저산소증과 때로는 고이산화탄소증을 유발하게 되는데[1], 호흡부전이 발생하면 소아환아의 생명을 위협할 수 있다[2]. 급성 저산소성 호흡부전환자에게 시행하는 고유량 비강 캐뉼라(High-flow nasal cannula, HFNC)산소요법은 가온 및 가습된 산소를 최대 60 L/min의 유속으로 전달하는 비침습적 호흡보조요법이다[3]. 최근 HFNC 산소요법은 환자의 안위도모, 산소포화도 향상, 호흡노력의 감소, 기도삽관을 늦추는 등 치료의 효율성 때문에 사용이 증가하고 있으며, 계획된 발관 이후 호흡부전의 발생을 줄이기 위해서도 널리 사용되고 있고[4], 신생아부터 청소년기 소아환아에게도 많이 사용하고 있다[5].

그러나 HFNC 산소요법을 하면서 기도삽관이 지연되면 오히려 사망률이 증가할 수 있으므로 HFNC 산소요법의 실패를 조기에 예측할 필요가 있으며, 환자의 호흡양상과 산소요구도 등을 면밀히 모니터링하여 적절한 시기에 기도삽관하는 것을 고려해야 한다[6]. HFNC 산소요법이 효과가 있는지를 확인하는 지표로 Respiratory rate oxygenation index (ROX index)를 사용하면 환자의 호흡상태를 평가할 수 있는데, 이 지표는 폐렴이나 급성호흡부전이 있는 환자에게 적용한 HFNC 산소요법의 효과를 예측하기 위해 사용되어 왔다[7]. ROX index에 심박동수를 보완한 ROX-heart rate index (ROX-HR index) 또한 환자의 호흡상태에 대한 평가지표로 사용되고 있다[8].

산소포화도(saturation of percutaneous oxygen, SpO₂)/흡입산소농도(fraction of inspired oxygen, FIO₂)비 즉 SF ratio는 호흡상태를 나타내는 지표로 사용되며[9], 동맥혈산소분압(pressure of arterial oxygen, PaO₂)/FIO₂비 즉, PF ratio는 동맥혈 가스분석결과에서 도출하여 저산소혈증의 정도를 평가하는 데 사용된다. 비침습적으로 측정할 수 있는 SF ratio는 산소화 기준을 평가할 때 호흡상태 지표로 PF ratio 대신 사용되어 왔으며[10], 호흡곤란이 있는 성인환자의 SF ratio는 민감도와 특이성을 고려할 때 235 미만이거나 315 이상이면 HFNC 산소요법의 실패와 성공을 예측할 수 있는데, SF ratio 235와 315는 PF ratio의 200과 300에 해당한다[11]. 급성호흡곤란 증후군이 있는 소아중환자실 환아 120명의 SF ratio와 PF ratio를 비교한 연구에서도 두 지표의 수치는 거의 유사하여 SF ratio는 통증에 민감한 소아환아에게 유용한 지표라고 하였다[9]. Khemani 등[12]도 소아환아의 SF ratio와 PF ratio를 비교하였고, SF ratio가 201 미만이거나 263 이상일 때 HFNC 산소요법의 실패와 성공을 예측할 수 있으며, SF ratio 201과 263은 PF ratio 200과 300에 해당한다고 하였다.

현재 임상에서는 의료진이 호흡부전으로 중환자실에 입실하는 소아환아의 호흡양상, 혈중 이산화탄소 농도나 산소포화도 등으로 기도삽관이 필요한지 판단하고 있는데, ROX index와 ROX-HR index 및 SF ratio는 동맥혈을 채혈하지 않고도 산출할 수 있기 때문에 소아환아에서 사용하기 용이한 장점이 있음에도 불구하고[9] 아직까지 간호현장에서 활용되지 않는 실정이다. HFNC 산소요법은 환자의 호흡부전을 호전시키는 장점이 있지만 기도삽관 시기를 지연시키는 단점도 있으므로 환자에게 HFNC 산소요법을 지속할 것인지, 중단하고 인공호흡 보조요법을 시도할 것인지를 지속적으로 평가하는 것은 매우 중요하다[13]. 24개월 미만의 소아환아에게 HFNC 산소요법 시 여러 시점에서 성공 및 실패 예측 정확도를 측정한 연구[14]에서 ROX-HR 지수가 HFNC 산소요법 시작 후 1시간과 6시간에서 HFNC 산소요법의 실패를 예측하는 데 유용한 도구라고 하였다. 하지만, 소아환아는 연령에 따라 맥박과 호흡수에 높은 변동성을 보이므로[15], 본 연구에서는 ROX index와 ROX-HR index 및 SF ratio를 함께 제시하여 소아환아에서 유용한 지표가 무엇인지 확인하고자 하였다. 한편, 코로나19 감염증 성인환자를 대상으로 ROX index를 다룬 연구는 많았지만[16, 17, 18], HFNC 산소요법을 시행하는 1세에서 18세 소아환아의 호흡능력을 측정하는 도구로 ROX index를 사용한 연구[19]와 24개월 미만 446명을 대상으로 ROX index와 ROX-HR index를 모두 사용한 연구[14] 외에 아직까지 소아환아를 대상으로 한 국내외 연구는 미흡한 실정이다.

이에 본 연구는 소아중환자실에 입실한 후 HFNC 산소요법을 받다가 호흡상태가 호전되어 중단한 환아(성공군)와 호흡상태가 악화되어 기도삽관 후 인공호흡 보조요법을 받은 환아(실패군)를 대상으로 ROX index와 ROX-HR index 및 SF ratio를 모두 적용하여 유용한 지표를 확인하고자 하였다.

2. 연구 목적

본 연구의 목적은 HFNC 산소요법을 받은 소아중환자실 환아의 ROX index와 ROX-HR index 및 SF ratio를 분석하기 위함이다.

1) HFNC 산소요법 성공군과 실패군의 특성을 비교한다.
2) HFNC 산소요법 성공군과 실패군의 시점별 ROX index와 ROX-HR index 및 SF ratio를 확인한다.
3) ROX index와 ROX-HR index 및 SF ratio의 민감도와 특이도를 확인하고, 시점별 성공 예측률을 확인한다.

연구 방법

1. 연구 설계

본 연구는 HFNC 산소요법을 받은 소아중환자실 환아의 의무 기록을 이용하여 ROX index와 ROX-HR index 및 SF ratio를 분석한 후향적 조사 연구이다.

2. 연구 대상

대상자는 2017년 1월부터 2021년 12월까지 가톨릭대학교 서울성모병원 소아중환자실에 입원하여 HFNC 산소요법을 받은 환아 150명 중 24시간 안에 HFNC 산소요법이 중단된 34명과 24세 이상인 5명, 심폐소생술 직후 입실한 2명, 입실 후 연명의료중단으로 기도삽관이 지연된 2명을 제외한 107명이었다. HFNC 산소요법의 성공은 상태가 호전되어 HFNC 산소요법을 중단하는 것으로 정의하였고, 실패는 기도삽관을 통해 인공호흡 보조요법이 필요한 상태로 분류하였다. HFNC 산소요법의 성공군은 75명이었고, 실패군은 32명이었다(Figure 1).

3. 연구도구

1) 대상자의 특성

대상자의 일반적 특성은 성별, 연령, 체중, 키, 체질량지수(Body mass index, BMI)로 파악하였고, 질환관련 특성 중 진단명은 혈액질환, 감염질환, 심장질환, 폐질환, 신경계질환 및 기형으로 분류하였다. 만성 호흡기 질환은 기관지폐이형성증과 천식유무로, 호흡부전의 원인은 배양검사를 통해 박테리아와 바이러스 혹은 질환과 관련된 것(수술이나 출혈, 이식 후 발생한 폐질환 등)으로 파악하였다. 소아사망예측(Pediatric Risk Of Mortality, PRISM)점수는 수축기 혈압, 이완기 혈압, 맥박 수, 호흡 수, 글래스고 혼수척도(Glasgow coma scale), 동공반사, 동맥혈산소분압/흡입산소량(PaO₂/FIO₂), 이산화탄소분압, 중탄산염 농도, 혈액응고검사, 총 빌리루빈, 칼슘, 칼륨, 혈당 등 6가지 생리학적 지표 및 8가지 혈액학적인 결과로 측정된다. 각 지표에서 측정된 값의 합으로서 모든 지표가 정상이라면 0점이고, 점수가 높을수록 사망률이 높은 것으로 해석하며[20] 본 연구에서는 중환자실 입실 시 측정한 점수를 활용하였다. 중환자실 입원경로는 응급실에서 직접 입원하였는지, 병실에서 이실되었는지로 구분하였다. HFNC 산소요법 사용기간은 HFNC 산소요법을 시작한 날부터 제거한 날까지 기간으로 파악하였다. HFNC 산소요법을 적용한 시점과 적용 후 2시간, 6시간, 12시간, 24시간 및 HFNC 산소요법 중단 1시간 전 최대 흡입산소농도와 HFNC 산소요법의 최대 유량, 동맥혈 산소포화도, 호흡수 및 맥박수는 의무기록을 통해 조사하였고, 수소이온 농도지수(power of hydrogen [PH])와 동맥혈이산화탄소분압(PaCO₂), 동맥혈산소분압(PaO₂) 및 염기과잉(base excess)은 HFNC 산소요법 적용시점에 시행한 동맥혈 가스분석 결과로 조사하였다. 또한 HFNC 산소요법을 받았던 총 입원기간과 입원 중 사망여부를 조사하였다.

2) ROX index

ROX index는 Roca 등[21]이 호흡상태를 평가하기 위하여 사용한 생리적인 지표로, SpO₂를 FIO₂로 나눈 후 호흡수로 나누는 수치이다. 선행연구에서는 HFNC 산소요법을 적용하고 12~24시간 경과 시점까지 ROX index를 모니터링해야 하며, 3.85 미만이면 HFNC 산소요법의 실패를, 4.88 이상이면 HFNC 산소요법의 성공을 예측할 수 있다고 하였다[3, 21, 22]. 본 연구에서는 HFNC 산소요법 적용시점과 적용 후 2시간, 6시간, 12시간, 24시간 및 HFNC 산소요법 중단 1시간 전의 ROX index를 조사하였다.

3) ROX–HR index

ROX–HR index는 심박동수를 보완하여 호흡상태를 평가하는 생리적 지표로[8] 심박동수를 ROX index로 나누고 100을 곱하여 산출한다. ROX–HR index는 HFNC 산소요법 적용 후 10시간 정도에서 민감도와 특이도가 가장 높았는데, 6.80 미만이거나 8.00 이상일 때 HFNC 산소요법의 실패와 성공을 예측할 수 있다[8]. 본 연구에서는 HFNC 산소요법 적용시점과 적용 후 2시간, 6시간, 12시간, 24시간 및 HFNC 산소요법 중단 1시간 전의 ROX–HR index를 조사하였다.

4) SpO₂/FIO₂ ratio

SF ratio는 SpO₂를 FIO₂로 나누어 호흡상태를 평가하기 위한 생리적 지표이다. 본 연구에서는 HFNC 산소요법 적용시점과 적용 후 2시간, 6시간, 12시간, 24시간 및 HFNC 산소요법 중단 1시간 전의 SF ratio를 조사하였다.

4. 자료수집 방법

본 연구는 후향적 조사연구로 가톨릭대학교 서울성모병원 간호부와 소아청소년과 책임자에게 자료수집에 대한 허락을 받고 의무기록을 통하여 연구자 1인(Choi SH)이 직접 조사하였다.

5. 자료분석 방법

자료는 IBM SPSS Statistics version 24.0 (IBM Co., Armonk, NY, USA) 프로그램을 이용하여 분석하였다. HFNC 산소요법 성공군과 실패군의 특성은 Chi-square test, Independent t-test, Fisher’s exact test로 분석하였고, HFNC 산소요법 성공군과 실패군의 시점별 ROX index와 ROX–HR index 및 SF ratio의 차이는 t-test나 Mann–Whitney U test로 알아보았다. HFNC 산소요법 성공군과 실패군에서 유의한 차이가 있었던 변수는 민감도(sensitivity)와 특이도(1-specificity)로 그려지는 곡선인 Receiver operating characteristic (ROC) curve와 곡선아래 면적인 Area under the curve (AUC)를 확인하여 HFNC 산소요법 성공여부를 예측하였고, ROX index와 ROX-HR index 및 SF ratio가 HFNC 산소요법 성공을 정확히 예측할 수 있는지 절단점(cut off point)을 확인하였다.

6. 윤리적 고려

본 연구의 내용과 방법에 대하여 가톨릭대학교 서울성모병원 생명윤리심의위원회(Institutional Review Board)의 승인(No. KC22RISI0235)을 받았다. 본 연구는 의무기록을 이용한 후향적 조사연구로 자료수집에 대한 대상자의 동의는 면제되었으며, 대상자를 식별할 수 있는 모든 개인정보는 숫자화하였다. 수집된 자료는 안전한 장소에 보안을 유지하여 보관하였으며, 연구목적 이외에는 사용하지 않았고, 파일은 암호화하여 연구자 이외는 접근할 수 없도록 하였다. 본 자료는 3년 후 파기될 예정이다.

연구 결과

1. HFNC 산소요법 성공군과 실패군의 특성 비교

HFNC 산소요법을 받은 소아중환자실 환아의 나이는 평균 36.4개월이었고, 5개월 미만이 57명(53.3%)으로 가장 많았고 61개월 이상 22명(20.6%), 6~11개월 16명(14.9%)순이었다. 성별은 여아가 65명(60.7%)이었고, BMI는 평균 17.14 kg/m²였으며, 진단명은 심장질환이 33명(30.8%), 혈액질환이 27명(25.2%)이었다. 만성폐질환이 있는 환아는 11명(10.3%)이었고, 호흡부전의 원인은 질병과 관련된 경우가 91명(85.0%)이었으며, 병동에서 중환자실로 이실한 경우가 62명(57.9%)이었다. HFNC 산소요법 치료기간은 평균 6.82일이었고, PRISM 점수는 평균 9.41점이었으며, 사망한 환아는 23명(21.5%)이었다.

HFNC 산소요법의 성공군의 BMI는 16.10 kg/m²로 실패군의 19.55 kg/m²보다 낮았으며(t = -3.14, p = .002), 동맥혈 가스분석 결과 base excess는 성공군이 1.19로 실패군의 4.65보다 낮았고(t = -2.37, p = .022), 사망률은 성공군이 6명(8.0%)으로 실패군의 17명(53.1%)보다 낮았다(χ² = 27.07, p < .001) (Table 1).

2. HFNC 산소요법 성공군과 실패군의 시점별 ROX index와 ROX-HR index 및 SF ratio 비교

심박동수는 HFNC 산소요법 적용 6시간 후에 성공군이 134.11회/min로 실패군의 121.09회/min보다 많았고(t = 2.58, p = .011), HFNC 산소요법 중단 1시간 전에는 성공군이 124.79회/min로 실패군의 136.00회/min보다 적었다(t =-2.58, p = .010). 호흡수는 HFNC 산소요법 적용 6시간 후에 성공군이 41.43회/min로 실패군의 36.34회/min보다 많았다(Z = -2.08, p = .038).

HFNC 산소요법 적용 24시간에 ROX index는 성공군이 8.28로 실패군의 6.82보다 높았고(Z = -2.50, p = .012), HFNC 산소요법 중단 1시간 전에도 성공군이 10.16으로 실패군의 6.88보다 높았다(t = -3.91, p < .001). ROX-HR index는 HFNC 산소요법 중단 1시간 전에 성공군이 8.61로 실패군의 5.23보다 높았다(t = -3.90, p < .001).

SF ratio는 성공군이 HFNC 산소요법 적용시점(Z = -2.99, p = .003), 적용 후 2시간(Z = -3.28, p < .001), 적용 후 6시간(Z = -2.97, p = .003), 적용 후 12시간(Z = -3.59, p < .001), 적용 후 24시간(Z = -3.64, p < .001) 및 중단 1시간 전에 모두 실패군보다 높았다(t = -5.57, p < .001) (Table 2).

3. ROX index와 ROX-HR index 및 SF ratio의 HFNC 산소요법 성공 예측률

HFNC 산소요법 중단 1시간 전에 ROX index는 AUC = 0.74 (95% confidence interval [CI]: 0.62~0.86, p < .001)의 성공 예측률을 보였고, ROX-HR index는 AUC = 0.74 (95% CI: 0.62~0.85, p < .001)의 성공 예측률을 보였다. SF ratio는 HFNC 산소요법 적용 후 12시간과 적용 후 24시간의 AUC가 각각 0.72 (95% CI: 0.60~0.84, p < .001)이었으며, 중단 1시간 전의 AUC는 0.84 (95% CI: 0.75~0.94, p < .001)로 성공 예측률이 높았다(Table 3).

논의

본 연구의 결과 ROX index와 ROX-HR index 및 SF ratio는 HFNC 산소요법을 받는 소아중환자실 환아의 치료 성공을 예측하는 변수였고, 이러한 지표 중에서 민감도와 특이도가 가장 높은 것은 SF ratio였으며, ROX-HR index, ROX index 순이었다. 성인환자를 대상으로 HFNC 산소요법 시작 후 1, 2, 4, 6, 8, 10, 12, 18, 24, 48시간과 산소요법 중단 1시간 전에 ROX index와 ROX-HR index를 측정한 결과, HFNC 산소요법 시작 후 10시간에서 ROX index 7.00와 ROX-HR index 8.00의 성공 절단점이 결정되었고, 각각 6.00의 실패 절단점이 결정되었다[8]. 본 연구에서는 HFNC 산소요법 적용 직후와 2, 6, 12, 24시간 및 중단 1시간 전에 ROX index와 ROX-HR index 및 SF ratio를 측정하였는데, 소아환아에게 적용한 HFNC 산소요법의 성공과 실패를 예측하는 최적 절단점은 HFNC 산소요법을 중단하기 1시간 전이었으며, 성공 절단점과 실패 절단점은 ROX i ndex가 6.88에서 10.16, ROX-HR index는 5.23에서 8.61, SF ratio는 198.75에서 353.15였다. 이는 HFNC 산소요법을 받는 소아환아의 ROX index가 10.16, ROX-HR index가 8.61, SF ratio가 353.15 이상일 경우에는 HFNC 산소요법을 중단하는 것이 가능하며, ROX index가 6.88, ROX-HR index 5.23, SF ratio가 198.75 미만인 경우에는 HFNC 산소요법을 중단하고 인공호흡 보조요법이 필요하고, 각 지표별로 상하절단점 사이를 유지하는 경우에는 HFNC 산소요법을 지속하면서 환아의 상태와 각 지표를 계속 모니터링 하는 것이 필요함을 의미한다.

본 연구에서 HFNC 산소요법을 받은 소아중환자실 환아 중 실패군 32명(29.9%)은 모두 인공호흡 보조요법을 받았는데, 이는 18세 이하의 급성호흡곤란 환아를 대상으로 한 연구[23]의 실패율 15.7%이나 신생아부터 청소년환아 539명에게 HFNC 산소요법을 적용한 연구[24]의 실패율 9.8%, 튀르키예 대학병원 소아병동이나 소아중환자실 혹은 소아응급실에서 HFNC 산소요법을 받는 환아의 실패율 15.7% [19]보다 높았으나 348명의 소아환아를 대상으로 HFNC 산소요법의 효과를 확인한 연구[25]의 실패율 33.5%보다는 낮았다. 본 연구에서 성공군과 실패군의 PRISM 점수는 차이가 없었지만, 선행연구[19]에서는 HFNC 산소요법이 24시간 이내 실패한 환아와 성공한 환아의 PRISM 점수에 차이가 있었다. 이는 본 연구 대상자의 27%가 질병의 특성상 사망률이 높은 혈액종양환아로 급성호흡부전증 소아환아가 많았던 선행연구[19]보다 PRISM 점수가 높았기에 실패군과 성공군의 PRISM 점수에 차이가 없었던 것으로 추정된다. 본 연구에서 HFNC 산소요법 성공군은 실패군보다 BMI가 낮았는데, 이는 소아에서 폐기능은 BMI와 음의 상관관계를 보였다고 한 연구[26]와 맥락이 유사하였으나 중환자실 성인을 대상으로 한 연구결과[27]와 달랐으므로 성인에 비해 소아에서는 과체중이나 비만이 천식 등과 연관되어 폐기능 저하를 가져오기 때문으로 사료된다[28].

소아환아에게 HFNC 산소요법을 적용한 연구[24]에서 ROX index는 소아의 호흡수가 많아서 치료성공을 예측하는 데 적합하지 않다고 하였는데, 본 연구에서는 ROX index가 HFNC 산소요법의 치료성공을 예측하는 민감도와 특이도가 세 지표 중 가장 낮았지만 AUC를 확인한 결과 성공군과 실패군이 구별되어 치료성공을 예측하는 지표로 사용 가능함을 알 수 있었다.

ROX-HR은 호흡수가 많은 소아에게 민감도와 특이도가 증가하여 소아의 연령에 따른 호흡수 변화를 고려하여 사용하기 용이하다. 한편, ROX-HR index의 호흡수 대신 호흡수 z-점수를 사용하여 소아호흡수-산소지수(pediatric respiratory rate-oxygenation index [p-ROXI])를 사용한 연구[19]에서 p-ROXI는 AUC 0.88의 예측률을 보였고 ROC curve에 대한 최적의 절단점을 평가했을 때 HFNC 산소요법 적용 후 24시간에 66.7이었다.

본 연구에서 ROX index의 치료성공과 실패 예측 절단점은 HFNC 산소요법 중단 1시간 전 10.16과 6.88이었는데, 이는 성인의 경우에 HFNC 산소요법 적용 12시간 후 ROX index 4.88과 3.85를 절단점으로 제시한 연구[21]와 달랐다. 또한 ROX-HR index의 치료성공과 실패 예측 최적 절단점은 HFNC 산소요법 중단 1시간 전으로 8.61과 5.23이었는데, 이는 급성 저산소증 호흡부전이 있는 성인환자의 경우 HFNC 산소요법 적용 후 10시간 정도에서 민감도와 특이도가 가장 높았고, 최적의 절단점은 8.00과 6.80인 것과 다른 것으로[8] 성인과 소아환아의 ROX-HR index도 기준점이 다른 것으로 판단된다.

본 연구에서 SF ratio는 HFNC 산소요법을 적용한 시점부터 성공군과 실패군 간의 차이가 있었으며, 시간이 지날수록 실패군에서는 수치가 감소하였고, 최적의 절단점은 HFNC 산소요법 중단 1시간 전으로 353.15과 198.75였다. 급성 폐손상이나 성인호흡부전증 환자를 대상으로 한 연구[11]에서는 민감도와 특이도를 고려할 때 SF ratio가 315 이상이거나 235 미만일 때 HFNC 산소요법의 성공과 실패를 예측할 수 있다고 하여 본 연구와 달랐다. 1세에서 18세의 소아중환자실 환아 204명을 대상으로 한 연구[29]에서는 HFNC 산소요법 적용 1시간 후 SF ratio가 200을 초과하면 HFNC 산소요법의 성공을 예측하는 것을 확인하였다. 소아응급실 환아 154명을 대상으로 조사한 후향적 연구[30]에서는 HFNC 산소요법 적용 후 1시간 동안 SF ratio가 195 미만일 때 치료 실패와 관련이 있다고 하여 본 연구와 유사하였으나 결과 예측을 위한 SF ratio의 정확성과 절단점 값을 식별할 수 없었다.

급성호흡곤란 증후군이 있는 소아환아 82명에게 시행한 HFNC 산소요법의 효과를 전향적으로 평가한 연구[15]에서 HFNC 산소요법의 실패율은 19.5%였고, SF ratio와 소아 호흡수를 보정한 pediatric ROX index (pROX index)와 임상호흡평가(clinical respiratory score)를 측정한 연구[31]에서 최적의 절단점은 HFNC 산소요법 적용 후 12시간이었으며, pROX 지수는 132 미만이었다. 현재까지 소 아에게 적절한 ROX index와 ROX-HR index 및 SF ratio로 제시한 연구는 없었으며, 소아환아를 대상으로 본 연구와 동일한 도구를 사용한 선행연구가 많지 않아 ROX index와 ROX-HR index에 대한 최적의 절단점과 시기를 확인하기에는 어려움이 있었다.

이에 본 연구에서 확인된 ROX index와 ROX-HR index 및 SF ratio를 통해 HFNC 산소요법을 시작한 소아환아의 호흡상태와 치료효과를 평가하여 특히 기도삽관이 지연됨으로써 적절한 치료시기를 놓치지 않도록 하는 것이 중요하다. 호흡곤란이 있는 소아는 신음소리, 흉부견인, 기침, 폐수포음 등의 증상을 보이는데[32], 튀르키예 소아중환자실에서 HFNC 산소요법을 받는 환아를 대상으로 한 연구[33]에서는 늑간 및 흉부견인, 흉부와 복부의 시소모양의 호흡양상, 비익확장, 호기 시 신음소리, 청색증, 의식수준에 대한 임상점수판(Clinical score chart)을 이용하여 호흡곤란을 조사하였다. 그러나 앞으로는 소아중환자실 환아의 전반적인 호흡양상을 객관적으로 모니터링 할 수 있는 ROX index와 ROX-HR index 및 SF ratio를 추가 활용하도록 제안한다. 본 연구는 호흡부전으로 HFNC 산소요법을 받은 소아중환자실 환아에게 호흡기 증상 및 객관적인 지표와 더불어 ROX index와 ROX-HR index 및 SF ratio가 환아의 호흡상태를 평가할 때 간호사들이 손쉽게 활용할 수 있는 실증적인 3가지 지표로 제안하였다는 것에 간호학적 의의가 있다.

HFNC 산소요법을 받는 소아환아의 ROX index와 ROX-HR index 및 SF ratio 지표를 측정하는 시간과 간격을 프로토콜화하고 의료진이 공유한다면 추가의 침습적 검사를 피하면서 HFNC 산소요법을 중단하거나 지속하는 임상적 의사결정을 할 때 근거로 활용할 수 있을 것이다. 즉 HFNC 산소요법을 받는 환아의 지표들이 본 연구에서 제시한 성공예측의 절단점보다 높다면 HFNC 산소요법의 중단을, 절단점보다 낮다면 HFNC 산소요법을 중단하고 인공호흡 보조요법을 적용하고, 그 중간값이라면 지속으로 모니터링하도록 제안한다. 그러나 본 연구는 일개병원 소아중환자실에서 HFNC 산소요법을 받은 환아를 대상으로 하였다는 제한점이 있다.

결론

본 연구에서는 HFNC 산소요법을 받은 소아중환자실 환아 중상태가 호전되어 중단한 성공군과 인공호흡 보조요법으로 전환한 실패군의 ROX index와 ROX-HR index 및 SF ratio를 비교하고, 이 지표들이 치료성공을 예측할 수 있는지 확인하였다. 본 연구의 결과 ROX index와 ROX-HR index 및 SF ratio는 모두 성공군과 실패군 간에 유의한 차이가 있었으며, HFNC 산소요법 적용 후 시점별로 조사한 ROX index와 ROX-HR index 및 SF ratio는 HFNC 산소요법 중단 1시간 전에 가장 유의한 AUC를 보였다. HFNC 산소요법의 성공과 실패를 예측하는 최적의 절단점은 ROX index가 6.88에서 10.16, ROX-HR index는 5.23에서 8.61, SF ratio는 198.75에서 353.15였다. 이에 근거하여 HFNC 산소요법을 받는 동안 환아의 ROX index와 ROX-HR index 및 SF ratio를 지속적으로 모니터링하여 인공호흡 보조요법을 시작해야 할 시기를 놓치지 않고 필요한 치료가 이루어지도록 하는 것이 중요하다. 본 연구의 결과는 소아중환자실 간호사가 환아의 호흡양상을 반영하는 생리적인 지표를 토대로 근거에 기반한 간호를 수행하는 데 도움이 될 것이라고 기대한다.
