Stimulation-Oriented Interventions for Behavioral Problems among People with Dementia: A Systematic Review and Meta-Analysis

Purpose
This study was a systematic review and meta-analysis designed to investigate the effects of stimulation-oriented interventions for behavioral problems among people with dementia.
Methods
Based on the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), a literature search was conducted using seven electronic databases, gray literature, and other sources. Methodological quality was assessed using the Scottish Intercollegiate Guidelines Network (SIGN) for randomized controlled trials (RCTs). Data were analyzed using R with the ‘meta’ package and the Comprehensive Meta-Analysis (CMA 2.0) program.
Results
Sixteen studies were included for meta-analysis to investigate the effect of stimulation-oriented interventions. The quality of individual studies was rated as ‘++’ for eight studies and ‘+’ for the rest. The effect sizes were analyzed according to three subgroups of interventions (light, music, and others); Hedges’ g=0.04 (95% CI: -0.38~0.46), -0.23 (95% CI: -0.56~0.10), -0.34 (95% CI: -0.34~0.00), respectively. To explore the possible causes of heterogeneity (I2=62.8%), meta-regression was conducted with covariates of sample size, number of sessions, and length of session (time). No moderating effects were found for sample size or number of sessions, but session time showed a significant effect (Z=1.96, 95% CI: 0.00~0.01). Finally, a funnel plot along with Egger's regression test was performed to check for publication bias, but no significant bias was detected.
Conclusion
Based on these findings, stimulation-oriented interventions seem to have a small effect for behavioral problems among people with dementia. Further research is needed to identify optimum time of the interventions for behavioral problems among dementia patients.

Purpose
The purpose of this study was to evaluate the psychometric properties of the Korean version of the Alzheimer's Disease Knowledge Scale (ADKS-K) to determine its applicability to Korean adults.
Methods
Cross-cultural validity was performed according to Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN). The Kuder-Richardson Formula 20 for internal consistency and Intraclass Correlation Coefficient (ICC) for test-retest reliability were conducted. Content validity, criterion related validity and construct validity were evaluated. The Classical Test Theory (CTT) model and the Item Response Theory (IRT) model were applied in performing the item analysis.
Results
The KR 20 was .71, and the ICC was .90, indicating that the ADKS-K has internal consistency and stability reliability. Thirty items of the ADKS-K had significant Content Validity Ratio (CVR) values, i.e., mean of 0.82 and range of 0.60~1.00. Mean item difficulty and discrimination indices calculated by TestAn program were 0.63 and 0.23, respectively. Mean item difficulty and discrimination indices calculated by BayesiAn program were -0.60 and 0.77, respectively. These tests indicate that ADKS-K has an acceptable level of difficulty and discriminating efficiency.
Conclusion
Results suggest that ADKS-K has the potential to be a proper instrument for assessing AD knowledge in Korean adults.

Purpose
The purpose of this study was to evaluate the reliability and validity of the Korean version of the Successful Aging Inventory (SAI-K) to determine its suitability for use with older Korean adults.
Methods
Exploratory and confirmatory factor analyses were utilized to assess the factor structure and the construct validity of the SAI-K. First- and second-order Confirmatory Factor Analysis (CFA) were conducted to identify the most adequate model. Cronbach's alpha was used to test the reliability.
Results
Using a second-order CFA, a four-factor structure was validated (χ2=122.82, p<.001, GFI=.92, AGFI=.88, SRMR=.06 RMSEA=.07, 90% CI=.05-.09, CFI=.93). The four-factor SAI-K showed reliable internal consistency with a Cronbach's alpha for the total scale of .86.
Conclusion
The four-factor, 13-item SAI-K showed satisfactory reliability and validity and, thus, has the potential to be an appropriate instrument for measuring successful aging in older Korean adults.

Purpose
The main purpose of this study was to identify the correlation between the child rearing attitudes and the maternal role strain of mother with infants.
Method
This study is designed as a descriptive research study and the data was collected from 82 mothers and infants by means of an interview and questionnaire in a period from July 2002 to December 2002, when they came to screen their infant's growth and developmental state at a public health center.
Result
The results of this study were as follows : There was a significant positive correlation between the child rearing attitudes and maternal role strain(r.= .53, p= .000). There was a significant difference between the father's employment state and child rearing attitude of mothers(t.=5.22, p<.000). There was a significant difference between male infant and female infants in maternal role strain(t.=3.8, p=0.04).
Conclusion
When the child rearing attitude was positive, the subject's maternal role strain was high. Also further research is needed on social support or other factors in the subjects in child-rearing attitudes and maternal role strain.

Purpose
The purpose of this study was to explore factors that influence care dependency of institutionalized patients with dementia.
Method
This study utilized descriptive correlational design. The convenience sample was composed of 110 residents with dementia of two long-term care facilities in Korea. Stepwise multiple regression was used to identify significant factors influencing care dependency in patients with dementia. Care dependency was measured using the Care Dependency Scale, Korean version(CDS-K). Cognition was measured by the MMSE-K. Functional disability was measured by the PULSES Profile. Behavioral dysfunction was measured by the modified E-BEHAVE AD.
Result
Care dependency was significantly influenced by cognition, functional disability, behavioral dysfunction, and duration of dementia. This regression model explained 61% of the variances in care dependency. Cognition explained 37% of the variances, and functional disability explained 21% of the variances.
Conclusion
Results of this study suggest that professional caregivers intervene more effectively in caring for their patients with dementia by recognizing the patients cognitive, functional, behavioral disability, and its periodic change. Individually, remaining abilities-focused intervention should be applied to enhance patient to be dependent and to prevent unnecessary independency.

PURPOSE: The primary purpose of this study is to provide a useful insight for developing effective teaching and student counseling methods by understanding the relationship between four preference patterns of MBTI personality types and EWHA defense mechanisms of nursing college students. METHOD The data used in this research were collected administrating EWHA defense mechanism test and MBTI personality type instrument to 195 nursing students in the period of November 3 through September 16 in 2000. Collected data were analysed by pc-SPSS 10.0.
RESULT: Three general conclusions were drawn from the study results: (1) in terms of personality type, 'Extraversion', 'Sensing', 'Thinking', and 'Judgement' types showed high frequency respectively; (2) in terms of defense mechanism, 'Identification' showed the highest score, and then 'suppression' scored next followed by 'Humor'. However, 'Acting out' showed the lowest average score, and then 'Projection' followed by 'Denial'; (3) Based on the relationship between defense mechanism and four MBTI preference patterns, it can be drawn that students with the preference of 'Extraversion' usually use defense mechanism of 'Altruism', 'Sublimation', 'Distortion', 'Controlling', and 'Humor'. In comparison, students with the preference of 'Introversion' often use the defence mechanism of 'Evasion' and 'Projection'.
CONCLUSION: The study results can be utilized in both fields of education and counseling: (1) in the field of education, this study can provide a basis for developing a suitable teaching method for each personality types of nursing students; (2) for counseling practitioners, the results of this study provide the useful insights to understand nursing college students' communication skills, behavior patterns, and ability of coping with problems in the process of counseling.
