PURPOSE: The purpose of this study was to identify the effects of a nurse presence program on suicide prevention for elders with chronic disease. METHOD: The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Twenty subjects in the control group received no intervention and nineteen subjects in the experimental group received a nurse presence program. RESULT: There were more significant decreases in suicide ideation, and the cortisol level and increases in life satisfaction in the experimental group compared to the control group. CONCLUSION: According to the above results, a nurse presence program for elders with a chronic disease decreased stressful events like suicide ideation and increased self esteem through therapeutic interaction. These findings suggest that this program can be used as an efficient independent nursing intervention for elders in a critical situation.
OBJECTIVES: The purpose of this study was to identify predictors of coronary heart disease risk factors in healthy men and women. METHOD: The subjects of this study were 346 people (173 men and women aged 20 years and over) who received health screenings. Data was collected from December 1, 2005 to February 28, 2006. The FANTASTIC Lifestyle Assessment Inventory except smoking and the Framingham risk score of subjects were investigated. Data was analyzed by descriptive analysis, t-test, ANOVA, pearson correlation coefficients and stepwise multiple regression using the SPSS 10.0 program. RESULTS: The mean score of the lifestyle of the women (64.24) was higher than that of the men (59.12). The mean score of the risk of coronary heart disease of the men (5.28%) was higher than that of the women (0.28%). The framingham risk for men was significantly related to lifestyle such as dietary habit, use of caffeine and drugs, anxiety and depression, job satisfaction, and closeness with family. The main predictors of framingham risk for men and women were 'use of caffeine and drugs', and 'menopause' which explained 16.5%, and 30.7% respectively. CONCLUSION: Since lifestyles can be changed with effort, coronary heart disease can be prevented while people are healthy.
PURPOSE: This study was conducted to investigate the effects of a cognitive behavior program on cognition, depression, and activities of daily living in elderly with Cognitive Impairment. METHOD: The research design was a nonequivalent control group pretest-posttest design. There were 21 subjects in the experimental group and 19 in the control group among 40 senior residents in a Hall for the elderly in the city of S. The subjects scores ranged from 15 to 23 on the MMSE-K(Korean Version of Mini-Mental State Examination) over age 65. The length of time for data collection and intervention was from Jun 26 to September 1, 2006. The cognitive behavior program consisted of 'Facing problem behavior', 'Searching for a coping skill', and 'Training in the coping skill'. It was applied to the experimental group twice a week, fifty minutes per session for six weeks. RESULT: Cognition(t=-4.232, p< .001) and IADL(t=-2.939, p< .01) in the experimental group were significantly higher than those of the control group. Depression in the experimental group was significantly less than the control group(t=3.870, p< .01). However, ADL in the experimental group was not significantly higher than the control group. CONCLUSION: These findings confirmed that a cognitive behavior program contributed to improving cognition and IADL, and to reducing depression in the elderly with Cognitive Impairment.
PURPOSE: The purpose of this study was to compare the effects of intravenous fluid warming and skin surface warming on peri-operative body temperature and acid base balance of abdominal surgical patients under general anesthesia. METHOD: Data collection was performed from January 4th, to May 31, 2004. The intravenous fluid warming(IFW) group(30 elderly patients) was warmed through an IV line by an Animec set to 37 degrees C. The skin surface warming(SSW) group(30 elderly patients) was warmed by a circulating-water blanket set to 38 degrees C under the back and a 60W heating lamp 40 cm above the chest. The warming continued from induction of general anesthesia to two hours after completion of surgery. Collected data was analyzed using Repeated Measures ANOVA, and Bonferroni methods. RESULTS: SSW was more effective than IFW in preventing hypothermia(p= .043), preventing a decrease of HCO3-(p= .000) and preventing base excess(p= .000) respectively. However, there was no difference in pH between the SSW and IFW(p= .401) groups. CONCLUSION: We conclude that skin surface warming is more effective in preventing hypothermia, and HCO3- and base excess during general anesthesia, and returning to normal body temperature after surgery than intravenous fluid warming; however, skin surface warming wasn't able to sustain a normal body temperature in elderly patients undergoing abdominal surgery under general anesthesia.
The purpose of this study was to identify the effects of a smoking cessation program on the amount of smoking and nicotine dependence and the self-efficacy of smoking cessation for smoking workers.
The total number of subjects was 38 smokers and was divided into two: 16 smokers were placed in the experimental group and 22 in the control group. A nonequivalent control group pretestposttest design was used. The smoking cessation program consisted of two sessions and was implemented for two hours daily for five days. Data was analyzed by using the SPSS/WIN 11.0 program with χ2-test, t-test or Mann-Whitney U and ANCOVA.
After the treatment, those belonging to the experimental group showed a significantly decreased amount of smoking, a decreased nicotine dependency and increased self-efficacy compared to the control group.
The smoking cessation program was effective for diminishing the amount of smoking, decreasing nicotine dependence and improving self-efficacy. Therefore, this program is recommended as a smoking cessation strategy for adult smokers.
Although concerns of female sexual dysfunction (FSD) are increasing in Korea, sexual dysfunction related factors are limited in research studies. The aim of this study was to develop an explanatory model that will further explain the continuously increasing female sexual dysfunction cases in Korea.
Survey visits were conducted to four hundred and eighty five women, over 25 years of age and presently residing in either urban or rural areas. All of them were analyzed using a structured questionnaire. A total of 8 instruments were used in this model. The analysis of data was done with both SPSS WIN for descriptive statistics and AMOS 5.0 for covariance structure analysis.
As a result, variables that showed notably direct effects on FSD were: sexual concept (sexual attitude), sexual distress, and psychosocial health (depression, crisis, traumatic life events). On the other hand, variables such as age, educational level, economic status, and marital status showed indirect influences on health-promoting behaviors.
By comprehensively addressing the factors related to sexual dysfunction, and comparing each influence, this study can contribute to designing an appropriate sexual dysfunction prevention strategy in tune with the particular characteristics and problems of a client.
This study was done to survey health behaviors in people at risk for a Cerebrovascular Accident(CVA).
From November 21 to December 29, 2005, a questionnaire survey was conducted with 171 people at risk for a Cerebrovascular Accident(LDL of above 130 mg/dl & homocysteine of above 15.0 micromol/L). Their physical composition was measured and blood was collected.
1. Of the subjects, 34.5% were smokers, 61.4% were drinkers, 56.7% did not exercise regularly, 57.3% did not control their weight, 26.9% preferred eating meat, and 32.2% preferred salty food. 2. The gender was different between smoking status(χ2=10.734, p=.001), and drinking status(χ2=7.185, p=.007), and the age was different between smoking statusχ2=6.656, p=.010), and drinking status(χ2=10.722, p=.001). The HbA1C level was different for regular exercise(χ2= 4.824, p=.028) and the HDL-cholesterol was different for meat-eating preference(χ2=7.928, p=.005). The observance of troubling signs was different for a salty food preference(χ2=4.313, p=.038).
It is necessary to develop programs for taking care of people at risk for a Cerebrovascular Accident and test the effects of the programs in order to reduce the risk factors of CVA and enhance health behavior promotion.
The purpose of this study was to analyze the types of life adjustment of widowed elderly women in Korea.
The research design was a Q-Methodological Approach. Thirty selected Q-statements from 32 subjects were classified into a shape of normal distribution using a 7 point scale. The collected data was analyzed using a QUANL PC program.
Three types of life adjustment were identified. Type I is the independent type: overcome the reality. Type II is the abandonment type: deny the reality. Type III is the accommodation type: accept the reality.
The results of the study indicate that different approaches to the quality of life for widowed elderly women in Korea are recommended based on the three types of life adjustment.
PURPOSE: The purpose of this study was to identify the effects of hand acupuncture therapy on sleep quality by means of a sleep questionnaire, polysomnography and a cerebral blood flow test in 30~59 year old adults. METHODS: The study was a sham controlled design. Twenty-two adults were assigned to the pellet stimulating group (11) or sham group (11). The pellet stimulating group received hand acupuncture therapy using New Seoam Press Pellets number 1 for 4 weeks. On the other hand, the sham group used the same Adhesive tape in terms of shape, size and quality as New Seoam Press Pellets number 1 for 4 weeks. A Transcranial Doppler Ultrasonography and Carotid Duplex Ultrasonography examination were used for evaluating cerebral blood flow. Data was analyzed using the SPSS 12.0 version program with Chi2-test, Fisher's exact test and Mann Whitney U-test. RESULTS: In the pellet stimulating group, subjective sleep quality significantly improved more than that of the sham group. Among the sleep indices of the polysomnography, total sleep time and sleep latency of the sham group significantly improved. The cerebral blood flow test didn't show any differences. CONCLUSION: These results suggest that hand acupuncture therapy is effective for subjective sleep improvement only, not polysomnographical sleep indices and cerebral blood flow.
PURPOSE: This study was done to develop the concept of grief focusing on the process of spousal bereavement in Korea. METHODS: The Hybrid model was used for analysis according to the 3 phases. An extensive literature review was done for the Theoretical phase. In-depth interviews were conducted with 15 participants whose spouses died within the past 3 years in the Field phase. In the Final analytic phase, the results in the Theoretical and the Field phases were compared, analyzed, and integrated according to the process of grief. RESULTS: The antecedent of the concept of spousal grief was spousal death. The dimensions of grief were classified to inner dimensions related to oneself, relational dimensions related to family and others, and existential dimensions related to the meaning of being. The attributes of grief were physical suffering, decline of cognitive ability, heartbreaking sorrow, expectations and conflicts of a new life, social stigma, dependence on or resentment towards God, etc. The empirical referent of grief was physical, psychological, social, and spiritual health status. The grieving progressed through 3 phasesshock-emancipation, suffering, and integration. CONCLUSION: Nurses should recognize the importance of their unique position as supporters for grievers, and try to assess individual characteristics and to provide tailored nursing interventions.
PURPOSE: The purpose of this study was to investigate the relationships among depression, somatic symptoms, and activities of daily living of elderly women in urban areas. METHOD: After obtaining participant's consent forms, a one-time, face-to-face, and private interview was conducted with each participant from Sep, 2006 to Jan, 2007 by trained graduate-level students. The questionnaire consisted of K-GDS, PHQ-15, Barthel Index, and Instrumental Activity of Daily Living. The collected data was analyzed with the SPSS/PC 12.0 program, which was used for frequency, percentage, mean, standard deviation, Pearson correlation coefficients, and multiple regression. RESULTS: The major findings of this study were as follows 1) 34.1% of participants belonged to the depression group. 2) There were significant relationships between depression and monthly income, somatic symptoms, ADL, IADL, and number of chronic disease. 3) Significant factors influencing depression were somatic symptoms, ADL, and monthly income. CONCLUSION: The results of this study give useful information for designing interventions and program development for appropriate depression management and care for elderly women.
The purpose of this study was to investigate the effects of a community-based group smoking cessation program among adult smokers in a rural community.
The study design was quasi-experimental with a pre and posttest. A total of 55 adult smokers participated in the study. They were evaluated 1, 3, and 6 months after the program to identify the long-term effectiveness of the program. The program consisted of a total of 5 sessions provided twice a week. To test the effectiveness of the program, urine creatinine, expired air carbon monoxide, nicotine dependence, and smoking-related knowledge were used as dependent variables. Data was analyzed with the SPSS 10.0 program with a t-test, paired t-test, and Scheffe test.
Urine creatinine, expired air carbon monoxide, and nicotine dependence were significantly decreased after the program. Also, the rates of continuous abstinence were 81.8% at posttest, 65.5% at 1 month, 54.5% at 3 months, and 54.5% at 6 months follow-up.
This community-based group smoking cessation program implemented by a nurse and smoking cessation counselors was effective for quitting smoking and decreasing urine creatinine and nicotine dependence. Therefore, this smoking cessation program could be recommended to induce smoking cessation as health promotion management in the rural community.
The purpose of this study was to identify the risk factors for a nosocomial urinary tract infection in intensive care units with a foley catheterization which showed a positive urine culture.
Three-hundred eighty-seven patients were included in the study. A retrospective review of the electrical medical record system's databases and medical record sheets in hospitalized patients from January 2003 to December 2003 was used. The collected data was analyzed by descriptive statistics, t-test, chi-square test and logistic regression analysis.
The frequency of the participants' nosocomial urinary tract infection was 72.9%. Significant risk factors for a nosocomial urinary tract infection were ‘age’, ‘place of catheter insertion’, ‘frequency of catheter change’, and ‘duration of catheterization’. These variables explained 18.4% of variance in the experience of nosocomial urinary tract infection in intensive care units with foley catheterization.
Medical personnel can decrease the incidence of a nosocomial urinary tract infection by recognizing and paying attention to the duration of catheterization, frequency of catheter change, and place of catheter insertion. As a result, specific and scrupulous strategies should be developed to reflect these factors for decreasing nosocomial urinary tract infections.
The purpose of this study was to investigate the stages of change in smoking cessation after a Coronary Artery Bypass Graft(CABG) and to identify the related factors.
The subjects (n=157) were patients who underwent a CABG in a university hospital from March 1998 to October 2005 and were smokers before the CABG. Data was collected viachart review and a telephone interview, and analyzed with descriptive statistics, χ2 test, one-way ANOVA, and Kruskal-Wallis procedure by the SPSS/PC win 12.0 program.
The subjects smoked for an average of 34 years (21 cigarettes per day) before surgery. Eleven percent of the subjects were in pre-contemplation, 6.4% in contemplation, 13.5% in preparation, 4.5% in action, and 64.5% in the maintenance stage. Nicotine dependence and selfefficacy were different among the groups with different stages of change in smoking cessation. Nicotine dependence was the lowest (p=0.00) and self-efficacy was the highest (p=0.00) in the maintenance stage. The number of subjects in pre-contemplation and contemplation significantly increased 6 years after surgery(p=0.05).
To implement effective smoking cessation interventions for CABG patients, the intervention should be developed to accommodate individual readiness for smoking cessation, especially so for those who had a CABG more than 6 years previously.
The purpose of this study was to categorize adult's subjectivity of their attitudes towards life sustaining treatment, and thereby understand the differences among these life sustaining treatment types using Q methodology.
Q-methodology, which provides a method of analyzing the subjectivity of each item, was used. Thirty selected Q-statements received from 52 adults were classified into a shape of normal distribution using a 7 point scale. The collected data was analyzed using a QUANL pc program.

The purpose of this study was to identify the effects of a weight control program and compliancy in overweight women.
This program was composed of strategies to modify diet and exercise and to change compliance and self determination over an 8 week period. The subjects were 19 overweight women who participated in our project voluntarily. Data was collected from May 4 to Jun 30 of 2007. The program consisted of regular rapid walking exercise, diet, mobile phone messages and e-mail. The data was analyzed by Repeated Measures ANOVA using the SPSS WIN program.
According to 3 assessment periods, there were significant differences in body weight, body mass index, and compliance. There were no significant differences in self determination.
These findings suggest that more intensive interventions may be needed to demonstrate a change in self determination.
The purpose of this study was to examine the relationship among fear of falling, pain, anxiety and depression, and to identify influencing factors in elderly women patients with degenerative arthritis living in the community.
The subjects of this study were 297 elderly women patients with degenerative arthritis. Data was collected by personal interviews using questionnaires. Data was analyzed by the SPSS(version 12.0) computer program, and it included descriptive statistics, one-way ANOVA, Pearson correlation coefficient, and Stepwise multiple regression.
There was a significant difference (p= .000) in fear of falling according to the level of pain, anxiety, and depression. There was a significant positive correlation among fear of falling, pain, anxiety and depression. Depression, pain, number of medication, age, and anxiety showed significant predictors (43.5%) for fear of falling.
This study suggested that thoroughly assessing predictors making an impact on fear of falling in the initial nursing assessment is the most important for falls prevention of elderly women patients with degenerative arthritis in the community.
This study was aimed to identify the factors that influence eating behaviors in coronary artery disease patients and to create data for nursing which is thought to improve the eating behavior.
The study population was coronary artery disease patients who were treated on an outpatient basis. The measurements were eating behavior, diet self-efficacy, perceived-benefits, perceived-seriousness, family support and medical team support. All of the measurement tools above were thoroughly modified to verify validity and reliability. Statistical analysis was done by the SPSS PC 12.0 program.
The influencing factors for the eating behavior was diet self-efficacy (β=0.476), social support (β=0.253), chest pain (β=0.177), smoking (β=-0.173) and regular exercise (β=.169), which explained 46.2%.
Eating behaviors of coronary artery disease patients were influenced by diet self-efficacy, family support and the presence of chest pain. Therefore, the development of a program for efficient dietary education that prevents the progression of coronary artery disease is needed.
This study was done to investigate cybersex addiction, gender egalitarianism, sexual attitude and the allowance of sexual violence in adolescents, and to identify the relationships among these variables.
The participants were 690 students from two middle schools and three high schools in Seoul. Data was collected through selfreport questionnaires which included a cybersex addiction index, a Korean gender egalitarianism scale for adolescents, a sexual attitude scale and a scale for the allowance of sexual violence. The data was analyzed using the SPSS program.
Of the adolescents, 93.3% reported not being addicted to cybersex, 5.7% reported being mildly addicted, 0.4% moderately addicted, and 0.6% severely addicted. Cybersex addiction, gender egalitarianism, sexual attitude and the allowance of sexual violence in adolescents were different according to general characteristics. Cybersex addiction of adolescents correlated with gender egalitarianism, sexual attitude and the allowance of sexual violence.
Gender egalitarianism, sexual attitude and the allowance of sexual violence in adolescents were influenced by cybersex addiction. Therefore, nursing interventions for prevention and management of cybersex addiction need to be developed and provided to adolescents. In addition, varied programs for teaching sexuality to adolescents should be developed and provided.
The present study was done to discover the structure of universal actual experiences ‘Considering tomorrow’ of health and quality of life among North Korean refugees in terms of the socio-cultural context of South Korea.
The research question was ‘what is the structure of the actual experience of ‘Considering tomorrow?’, which was examined based on the Parse's human becoming research method. Five North Korean adult refugees were recruited from a National Reconciliation Committee in Seoul/Incheon. The data was gathered from dialogues and collected from February, 2006 to November, 2006.
The structures found in this study were: ‘hope for future life by taking responsibility and having harmony with South Koreans, by forming an integrated identification; having a chance for positive engagement, by attaining human freedom and hope; feeling respected, by assimilating self to the new world; getting freedom back, by facing a new challenge and preparing self for a new social role; overcoming cultural differences with fortified hardiness for survival, by making a decision for a life course with individual growth. In addition, conceptual integration was that ‘Considering tomorrow is transforming the enabling-limiting values’.
Health professionals need to know North Korean refugees' psychological difficulties, expectations of treatment, help seeking behavior, and expectations from mainstream culture. Additionally, understanding North Korean refugees' needs for reality, health education and a multi-disciplinary team approach are necessary to improve their health.
