DOI: https://doi.org/10.4040/jnas.1997.27.2.275
AbstractAbstract PDF

The purpose of the study is to explore and describe the lived experience of family members with gastric cancer patients using the grounded theory methodology. The participants were ten spouses of gastric cancer patients who had some kind of treatment at the hospital. They were asked open-ended and descriptive questions in order for them to talk about their experiences in their owl terms. As the interview progressed the questions became more specific to discuss themes and working hypotheses that emerged from the analysis of previous interviews. All interviews were tape-recorded and transcribed for the analysis. Constant the core category that was emerged from the comparative analysis is "magmaggam" which can be described as a psychological distress due to a high level of uncertainty regarding the health of the patient and the future of the caregivers. Psychological distress includes several emotional feelings such as frustration, anxiety, fear, guilty, and self depreciation. Subcategories or strategies related to the core category are 1) managing illness, 2) using folk medicine, 3) giving the patient a reason to live, 4) being patient, 5) losing reality, 6) anticipatory experience on the patient's death and parting, and 7) changing interpersonal relationships. The results of this study would help clinical nurses to develop nursing intervention to help spouses of gastric cancer patients establish efficient coping strategies in dealing with the problems they face.

Journal of Nurses Academic Society 1997;27(2):289-302.   Published online March 30, 2017
DOI: https://doi.org/10.4040/jnas.1997.27.2.289
AbstractAbstract PDF

The main purpose of this study was to identify the effects of integrated stress management program on the stress symptoms of psychophysiological patients, especially patients with peptic ulcer. The study employed a quasi -experimental design using two different experiential groups. The samples in the integrated stress management program participated in autogenic training with biofeedback, discussions on effective coping method, cognitive, behavioral, and emotional management. They were also provided with an educational booklet on stress management and an tape on progressive muscle relaxation. Exch session lasted one hour and the program consisted of seven sessions over four weeks. The other group was only given an tape on progressive muscle relaxation. The data were collected from May 20 to september 25, 1996 A total 47 patients from ore university hospital located in Seoul participated, experiment group 1 (integrated stress management training) had 23 subjects and experiment group 2(progressive muscle relaxation training) had 24 subjects. The effects of these programs were measured by the stress symptom scale developed by Kogan(1991) which was translated by Lee(1892) and the healing status of the ulcer evaluated by a physician The data were analyzed using Chi-square test, t-test, ANOVA, repeated measure ANOVA. The result are as follows : 1. The integrated stress management group reported a significantly lower stress symptom score than the group given the progressive muscle relaxation only. 2. The integrated stress management group showed a significantly improved ulcer status as compared to the group given a progressive muscle relaxation only. In conclusion, it was found that the integrated stress management program was more effective in decreasing self-reported stress and physiological symptoms among patients with peptic ulcer as compared to the progressive muscle relaxation group. Based on this finding, the following suggestions can be made. 1. It is necessary to broaden the scope of nursing Practice for psychophysiological patients so nurses can include stress management as part of Patient care. 2. It is necessary to develop stress management Program for other patients whose symptoms are know to be related to stress. 3. It is necessary to replicated this study with a larger sample in different settings.

Soon Rim Suh, Young Im Park, Sung Jae Kim, In Ja Kim, Mee Ock Gu, Young Hee Yang, Eun Nam Lee, Song Ja Park, Eun Ok Choi, Dong Suk Lee, In Sook Lee, Eun Ok Lee
Journal of Nurses Academic Society 1997;27(2):329-340.   Published online March 30, 2017
DOI: https://doi.org/10.4040/jnas.1997.27.2.329
AbstractAbstract PDF

Resourcefulness was analyzed by Walker and Avant's method to make a theoretical framework for nursing. Also, the appropriate Korean terminology was identified. "Resourcefulness" means the ability to use internal and external resources to eliminate or to control stress. 'Resourcefulness' is chosen to be the most appropriate term to reflect the concept of resourcefulness. Upon the concept analysis availability, controllability, confidence, and self-instruction were identified as the defining characteristics of resourcefulness. Contrary to other work, "availability" means the ability to use the social resources as well as the internal cognitive-behavioral resources. "Controllability" means the ability to delay or control immediate gratification of one's needs or to divert one's own mood in order to solve problem. "Confidence" is the self-efficacy belief in one's control ability. "Self-instruction" is the cognitive ability to instruct oneself positively. Resourcefulness is learned by active experience, vicarious experience, and formal or informal instruction or a need for change. As a result of the use of resourcefulness, the target behavior performed immediately and ultimately improve the quality of life or adaptation. Therefore, it is necessary to develop the instrument including the defining attributes identified in this study. Also, it is necessary to simultaneously analyze the related concepts of self-efficacy, self-control, and self-regulation for appropriate use.

The Three Dimensions of Eating Behavior: Development of scale for assessing cognitive restraint of eating disinhibition, and hunger
Moon Sil Kim, Sook Young Kim
Journal of Nurses Academic Society 1997;27(2):377-388.   Published online March 30, 2017
DOI: https://doi.org/10.4040/jnas.1997.27.2.377
AbstractAbstract PDF

This study was done to develop a eating behavior scale for high school girls using Stunkard and Messick(1985)'s three factors(dietary restraint, disinhibition, hunger) as a conceptual framework and to test the validity and reliability of the scale. The subjects were 202 high school girls. The convenience sample is consist of 96 normal weight and 106 obese girls. Data were collected between October 28 and November 25 in 1996 and analyzed using the SPSS package. The new 32-item scale measuring these factors is presented. 1. Three stable factors emerged and these contributed 45.7% of the variance in the total score. All 32 items loaded above .35 on each factor. 2. Factor 1 was named disinhibition(14 items), factor 2 was named cognitive restraint of eating(10 items), factor 3 was named hunger(8 items). Comparison of factor I, factor II, factor III between normal subjects and obese subjects showed that there was a significant difference in factor II and III, not in factor I. 3. Cronbach's alpha coefficient for internal consistency was .9393 for total 32 items and .8820, .8652, .8490 for three dimensions of eating behavior in high school girls. Recommendations are suggested below : 1. Replication study with appropriate age groups be done to test validity and reliability. 2. The present developed scale must be a reliable measure of eating behavior and should have utility in further study on how effective nursing interventions related to different kind of groups such as disinhibition group and cognitive restraint of eating group. 3. To extend validity of the scale, further study is needs using more larger subjects which is consists of dieters and free eaters.

Effects of a Social Support Group on Burden and Well-being of Mothers of Developmentally Delayed Children
Ka Sil Oh
Journal of Nurses Academic Society 1997;27(2):389-400.   Published online March 30, 2017
DOI: https://doi.org/10.4040/jnas.1997.27.2.389
AbstractAbstract PDF

This study examined the effects of group social support on the reduction of burden and increase in well-being of mothers of developmentally delayed children. The research used a one group pre-post experimental design. The independent variable in the experiment was group social support. Two series of 4-weekly meetings for group social support were conducted by the researcher with the intention of developing a self-help group. The dependent variables were burden and well-being. Well-being was operationalized as physical symptoms and quality of life. Thirty mothers of developmentally delayed infants from the rehabilitation center of a medical center participated in the study. Data were collected by interview and a self-administered questionnaire. The mean age of the subjects was 29.9 years. Changes of the dependent variables between pre and post tests were compared using the t-test. Even though there was a slight improvement in the scores for the dependent variables, they were not statistically significant. The items, "I resent my baby", "I feel angry about my interactions with my baby", "I feel guilty in my relationship with my baby" showed a significant decrease in burden score and were statistically significant. Symptoms of loneliness, constipation, anxiety, restlessness were less and feeling of happiness was greater after participation in the group social support, than on the pretest. The mothers showed emotional instability and frustrations during the group sessions but their reactions in general were positive. Emotional support, stress management and information provided were identified as the most valuable content of the sessions. However, participation was not active due to the mother's denial, delayed acceptance and/or avoidance of their infants' problems. It can be seen that group social support for the mothers with developmentally delayed children should be provided after infancy when the mothers have time to accept their children's conditions and are ready to receive support. The use of comprehensive instruments which measure burden in both families and mothers needs to be developed for future research.

Journal of Nurses Academic Society 1997;27(2):411-422.   Published online March 30, 2017
DOI: https://doi.org/10.4040/jnas.1997.27.2.411
AbstractAbstract PDF

The purpose of this study is twofold :(i) to investigate how much effort the married couples are making for the good health of both the pregnant woman and her unborn child from the time of their marriage to and during the period of conception ; and(ii) to comprehensive investigate socio-cultural backgrounds which affect prenatal effort. Result of this study provide a basis for the prenatal care program which will be appropriate to our culture. This study has been done by the ethnographic research method. The subjects of this study are 53 people in all consisting of 33 pregnant women and 20 husbands. In order to investigate socio-cultural factors which influence Taegyo, producers of Taegyo music were interviewed. In addition, the researcher surveyed the markets of Taegyo music, participated in special courses of prenatal education, analyzed the content of the books and periodicals dealing and collected the concept of Taegyo distributed by the mass media. The full-fledged study continued for eight months from February to August, 1996. The data were analyzed as soon as they were collected. Spradly's(1979, 1980) developmental, sequential method of domain analysis, taxonomic analysis, componential analysis, and theme analysis in this order was adopted as the procedure of analyzing the data. To obtain the exactness of study, Sandelowski's(1986) four criteria, that is, Credibility, Fittingness, Auditability, and Confirmability were applied to all stages of data collection, data analysis, the interpretation of the result, and the description of the result. The following are the result : 1. The couples' Taegyo at the stage of preconception was related to their physical, psychological, spiritual conditions under which a healthy baby will be born. Specific methods they prefer are : "the choice of one's spouse," "physical check-up," "physical good health," "praying," and so on. 2. When the married couple have sex in order to conceive, their Taegyo was related to the imposition of their physical, psychological, and environmental conditions. Specific methods they prefer are: "having sex at specific time," "having sex in nice place," "to purify their minds while having sex," and so on. 3. The married couples' Taegyo while they are in pregnancy was related to the imposition of their physical, psychological, emotionmental, environmental, social and spiritual conditions. Specific methods they prefer are : "listening to music," "reading," "looking at beautiful things only," "to avoid looking at or listening to bad things," "to eat food in good shape," "to avoid drugs," "eating Korean herbal medicine," "sexual abstinence," "to avoid dangerous places," " to keep emotional tranquility," "moderate exercises and rest," "leading a pure life," "praying," "being aware of their words and behavior," "for the couple to keep a good relationship," "interaction with their unborn child," "to support Taegyo for pregnant women," and so on. 4. The married couple put Taegyo into practice on the basis of the following principles : the principle of respecting an unborn child, the principle of forming a good disposition, the principle of top-down parental love, the principle of synergy between a pregnant woman and her unborn child, the principle of expecting a good child, the principle of forming a good habit, and the principle of acquiring a parental role. 5. The practice of Taegyo is influenced by such factors as the married couple, the supporting system, and the married couple, the supporting system, and the mass media. As the husband-and-wife factor, their information of Taegyo, the degree of importance is assigned to their characters, their time to spare, their healthiness, the age of pregnant woman, their conception plan, their religion, their belief of the Taegyo effects, and the birth of a baby in this order. The factor of the supporting system consists of her husband's support, he family support, and her neighbor's support. The mass media specialized in Taegyo, periodicals for pregnant women, booklets for advertizing powdered milk, Taegyo music of record manufacturing companies, and the teaching materials for gifted children. Among these the mass media is especially taking advantage of Taegyo as its main source of economic profits and leading the public behavior pattern to a prodigal one. Taegyo is a self-control behavior which requires practice for the following : the physical and psychological good health of the pregnant woman and her unborn child, the development of the unborn child's good character, the envelopment of the unborn child's intelligence and talents, the expectation of the unborn child's good features, shape a good habit, the expectation of the unborn child's bright future, and the learning of a parental role, the expectation of male birth. Above all it is a type of our good cultural tradition which pursues a value higher than the one that the prenatal care does. The principles of pregnancy care inherent in the habit of Taegyo will provide us a guideline for the development of the prenatal care.

The results were as follows : 1. Their mean ages were 21.8 for female and 23.3 for male students, 2. 78.0% of the males and 46.5% of the females permitted premarital sex, 57.1% of male and 10.3% of female college students had experienced sexual intercourse, 7.1% of males and 2.4% of females had experienced pregnancy, 10.3% of males and 3.4% of females had been infected with STDs, 72.1% of male and 13.8% of female didn't use condoms at the time of infection. 3. Most of the factors related to STDs infection, such as drinking, smoking, frequency of sexual intercourse, pregnancy, knowledge of STDs, the score of STDs prevention were statistically higher in the male student group than in the female group. 4. The student's mean score of knowledge about STDs was similar between the male group(7.80)and the female group(7.84) with a possible score range from 0-18. 5. Only fifteen percent of male and 9.6% of female students expressed that they will do something to prevent STD. 6. The group having the experience of sexual intercourse(t=3.924, P=.048) and the group of having experience of contracting STDs(t=16.638, P=.000) had shown statistically higher STDs prevention score than the group not having that kind of experience, but the group not having experience with pregnancy didn't show any difference from the group not having experience with pregnancy. Considering that 57.1% of males and 10.3% of females had sexual intercourse experience, 78% of male and 46.5% of female permitted premarital sex, 10.3% of male and 3.4% of female had been infected with STDs. It could be concluded that the college students were ignorant about the prevention of STDs and had unrealistic stage of the STDs prevention. Therefore, enforcement of education for the prevention of STDs including the dynamics of the sexual intercourse and STDs infection is needed.

Cardiac catheterization is a diagnostic procedure which is intrusive and anxiety provoking. Patient education and information offer have been suggested as effective nursing interventions to reduce patients' anxiety and stress. Main objectives of this study are : 1) to develop concrete objectives information for patients undergoing cardiac catheterization ; 2) to analyze the types of information included in the newly developed concrete objective information. The subjects of this study are 11 patients who were admitted to the CCU of a general hospital in Seoul. The subjects were provided with preparatory information about cardiac catheterization by using interview and a booklet. After the procedure, the subjects were asked to describe additional information which they suggest to be added into the booklet and the physical sensations felt during the procedure. The results of the study are summarized as follows : Most subjects were satisfied with the preparatory information that was provided by interview and the booklet before the procedure. But patients suggested several problems related to the content of the booklet. First, they reported difficulty to differentiate the terms-coronary angio gram and cardiac catheterization. Also, some patients expressed that they feared after reading the information about the incision of inguinal area. Subjects responded that the information about the direct process of the test did not reduce their anxiety and the information was not detail enough. Next, most subjects would want to know about the monitor and the sound from monitor. They said that they could not hear instructions from doctor of nurse during the procedure due to tension. Considering above response results, the need for more effective way to provide information, like visual and auditory information through video tape for giving information is suggested. Sensations related to the procedure were the smell coming from sterilization of inguinal area, stinging pain in groin when the doctor inserts a needle into artery, and the sensation of pressure and moving of vessels surrounding neck when the catheter was inserted and visualized on fluoroscopy. Besides, subject reported hot sensation and burning feeling in face and chest area, and nausea when dye is injected by hand. In the analysis of information content, there was 79% agreement on the actual units of analysis that were coded. In the analysis of type of information, procedural information was 60.4%. Concrete objective information was 28.1%, and other information was 11.5%. Agreement of the coders in categorizing the units of information was determined by using Cohen's kappa which corrects for chance agreement. Cohen's kappa was .84.

The purpose of this study was to construct a grounded theory as the basis for nursing intervention by describing and analysing the holistic lived experiences of clients receiving long-term hemodialysis. The subjects of this study were fifteen persons receiving regular hemodialysis regimen at artificial kindly treatment centers in two different university hospitals, and who were able to participate in conversation and were available for long and deep interviews. Eight of the subjects were male and seven were female and their ages ranged from 30's to 60's. The length of the hemodialysis experience ranged from two months to six years. The collection and analysis of data were done in accordance with the grounded theory methodology of Strauss and Corbin. The method to collect the data mainly depended on long and deep interviews, participant observation and focused group interviews and the equipment used to collect data were a portable tape recorder and field notes. The study is summarized as follows: 1. The meaning of holistic lived experiences of clients receiving long-term hemodialysis was found to be uncertainty, which was identified as the core category. 2. The main categories following the core care category were found to be shock, ambiguity, social support and quality of life. 3. Through the main category the type of behavior newly formed by clients receiving long-term hemodialysis was found to be as follows. That is to say, in the circumstances of shock caused by the identified fact and the ambiguity of hemodilysis they formed a quality of life based on social support, which was found to be kind of chaotic phenomenon. 4. The lived experiences of clients receiving long-tern hemodialysis was found to include nine categories ; emotional shock, feelings of isolation, burden, unclearness, dependency, help from others, coping strategies, maintenance of self-esteem and transitional life. 5. The intervening factors influencing each category are as follow : 1) The factors influencing 'emotional shock' were found to be sex, age, the level of knowledge received in advance, locus of control, the period of struggle against the disease before hemodialysis and whether any serious illness existed. 2) The factors influencing 'feelings of isolation' were found to be religion and the length of the hemodialysis experience. 3) The factors influencing 'burden' were found to be sex, economic situation, economic situation, employment status and the length of the hemodialysis experience. 4) The factors influencing 'unclearness' were found to be-sex, age, religion, economic situation, the length of the hemodialysis experience, whether they had a transfusion and whether there were any complications. 5) The factors influencing 'help from others' were found to be religion, economic situation, past experiences and whether family members lived together. 6) The factors influencing 'coping strategies' were found to be age, level of education, experiences of illness and locus of control. 7) The factors influencing 'maintenance of self-esteem' were found to be the length of the hemodialysis experience and self-actualization. 8) The factors influencing 'transitional life' were found to be age, religion, economic situation, employment status, locus of control, past experiences and whether there was a plan for a kidney transplant.

AbstractAbstract PDF

The purpose of this study was to identify the correlation between the intention of pregnancy and the child rearing attitudes of mothers with infants. The data was collected from 138 mothers by means of an interview and questionnaire in a period from April 1 to April 30, 1997, when they came to have their infants vaccinated at one hospital and one health care center. The collected data was analyzed by means of the SPSS Program using percentage, means and standard deviation in each area and the ANOVA and Pearson correlation Coefficient. The results of this study were as follows : 1. It showed that the mother's degree of intention of pregnancy was 3.90(+/-1.19) among the possible score of 0~5 and the score of child rearing attitudes was 70.1 among the possible score of 20~100. The mean score of the items was 3.50(+/-.36) among the possible score of 0~5. 2. There was a significant relation between the intention of pregnancy and child rearing attitudes. It was mildly positive but was significant(r=.21, p<0.05). 3. There was a significant relation between the period from marriage to the time of pregnancy and the intention of pregnancy(F=.57, p<0.05). Also, it was found out that the group of mothers who became pregnant later than 12 months after marriage wanted after pregnancy more than the group of women who were pregnant within 12 months after marriage(F=11.30, p<0.01). 4. There was a significant relation between the period from marriage to the time of pregnancy and child rearing attitudes(F=4.14, p<0.05). It showed that the child rearing attitudes of the group of women who became pregnant later than 12 months after marriage was positive(F=11.42, p<0.01). Also, in case tat the both the husband and wife decided on the pregnancy, the child rearing attitudes were more positive(F=7.15, p<0.05). In conclusion, when the intention of pregnancy was stronger, the subjects' child rearing attitudes were more positive. Also when both the husband and wife decided on the pregnancy, the attitudes were more positive. So it seems that both husband and wife should discuss and agree upon pregnancy, having a sufficient period during which both the spouses prepare for the intention of pregnancy. Additionally, it is suggested that when the husband and wife are doing family planning, it would be beneficial to use a program of 'Woman's Intention on Pregnancy' during this process.
