Purpose
The purpose of this study was to investigate the factors that influenced health related quality of life in women with disabilities. The other purpose was to examine the correlations of activities of these women's daily life, self esteem, and health promoting behavior with health related quality of life.
Method
The subjects consisted of120 adult disabled women. Data was collected from February to June2004 using structured questionnaires. Spearman's correlation, and multiple regression were used to test the quality of life with K-ADL, self esteem, and health promoting behaviors by using SPSS 12.0 for windows.
Result
The health related quality of life of women with disabilities was related to educational level, marital status, residential type, cause of acquired disability and disability of spouse. In addition, it was related to self esteem (r=.385, p=0.00), health promoting behaviors(r=.428, p=0.00), K-ADL(r=-.419, p=0.00) and K-IADL(r=-.439, p=0.00). Activities of daily life, self esteem and health promoting behaviors were significant predictors 37.3% (=0.212, p=0.000) to explain quality of life in disabled women.
Conclusion
The more self esteem, health promoting behaviors, K-ADL and K-IADL are positive, the more the health related quality of life is good. Further studies need to be done to investigate additional effects of health related quality of life and to investigate nursing approaches to improve their health related quality of life with significant predictors.

PURPOSE: It was identified that how many homebound bedridden elderlies and their primary caregivers were depressed, and which factors affected the bedridden elderly's depression.
METHOD: The subjects were 191 homebound bedridden elderlies and their primary caregivers. The affecting factors were classified into two categories: bedridden elderly and their primary caregiver related factors. Then bedridden elderly's factors were classified demographic and disease-related factors again. The stepwise regression was used to identify significant factors.
RESULT: The prevalence of bedridden elderly's and caregiver's depression was 77.8% and 67.0%, respectively. And the model explained 33.3% of variance of bedridden elderly's depression. Cognitively-impaired female elderlies who had depressed caregivers were found to be more depressed. And caregivers who perceived burden were identified to be more depressed.
CONCLUSION: It is recommended that the health professionals need to identify bedridden elderlies and caregivers at risk of depression. Especially elderlies who is in poor cognition, those who are female, and those whose caregivers were depressed might be considered carefully in all counseling or follow-up. Also the primary caregivers must be helped to access already available formal and informal support.

PURPOSE: This study aims to identify the ADL and IADL of bed-ridden elderly. Also it
is used to show fuctional status, and to investigate the content and the level of nursing
services provided. METHOD: The subjects were 191 elderly who received visiting nurse
service through public health centers in the Seoul Metopolitan and Chungnam Province.
Data collection was conducted by public health center nurses during four months in
2000. Result: As for daily living activities, 100% of subjects had at least one difficulty
in ADL and IADL. Among them, only 0.5% had moderate disabilities and 99.5% had
severe disabilities by HFS, 27.9% were in a semi bed-ridden state and 72.1% were
completely bed-ridden by JABC.
The major service provided was a visiting nurse service which was preferable to the
social welfare service. In the visiting nurse service, there was no significant difference
according to the elderlys' functional status.
In addition there was no standadization about the qualification of the visiting nurse, and
single entry point for the nursing service. CONCLUSION: The researchers urgently suggest
that a community based comprehensive service model has to be developed to respond to
the needs of the elderly in Korea.

Depression of Stroke Patient's Family Caregivers and the Relating Factors

In this paper, we examined the depression of stroke patient's caregivers and analyzed influencing factors of the depression. The subjects were 215 caregivers who have takencare of stroke survivors in their home. The conceptual model of this study consisted of
the caregiver's depression, perceived burden, illness intrusiveness, and patient's ADL. Modified Korean CES-D, modified subjective and objective Burden Scale, Illness intrusiveness(II), and Instrumental Activity of Daily Living(IADL) were used to measure concepts. Path analysis was used to test the model of this study.
The results were as follows:
1. The mean depression score was 11.6 which was below the cut-off score of the CES-D. This score indicates that the subjects were higher than normal adults' mean score but not depressive. Eighty-six out of 215 caregivers(40%) were above the cut-off score. This finding was different from previous research results, and the reason might be the patients' capability of ADL. In a group of low capability patient's activities of daily living, caregiver's depression score was 15.5.
2. Caregiver's depression was positively related to caregiver's burden and illness intrusiveness, but negatively related to patient's activities of daily living.
3. The caregiver's perceived burden and illness intrusiveness directly influenced on their depression. Furthermore, the and caregiver's illness intrusiveness led to depression indirectly through their burden. A patient's activities of daily living didn't influence directly on depression but indirectly through caregiver's illness intrusiveness and burden.

The purpose of this study was to describe the perceived burden of the stroke patient's caregiver and
related factors to analyze relationships between perceived burden and social supports. A convenient sample
of 225 caregivers who take care for a stroke patient at home participated in this study. Caregiver's
perceived burden was measured by the objective and subjective burden scale developed by Montgomery
(1985).
Related factors of burden were studied in terms of the patient's instrumental activties of daily living, cognitive
function, caregiver's demographic variables and caregiver's illness intrusiveness.
The results were as follows: (1) The mean of objective burden score was 4.5, and subjective burden score
was 3.1. These scores show that caregivers perceive moderate level of burden. (2) Caregivers' objective
burden was significantly related to caregivers's illness intrusiveness (r=.62), patient's IADL (r=-.33), and
patient's cognitive function (r=-.15). The subjective burden was related to the caregiver's illness
intrusiveness (r=.29), the patient's IADL (r=.24), and the caregiver's age(r=.23). (3) The percentage of
stroke caregivers who perceived physical support was 49.1%. The percentage of those who perceived emotional
support was 61.0%, and those who perceived financial support totaled 37.6%. (4) Caregivers who received
any type of social supports perceived lower subjective burden, and caregivers who received physical or
psychological support perceived lower objective burden. These results emphasized the necessity of a
rehabilitation programs for stroke patients and support program for family caregivers.

THE NECESSITY AND PURPOSE OF THE STUDY: Recently the number of patients with chronic diseases and the aged patients is increasing steadily. Furthermore, due to the expansion of health insurance system, the number of patients hospitalized in the general hospital is increasing at a surprising speed. However, hospitals urge the early discharge of the patients for the efficiencies of hospital administration, and therefore, the number of patients who must be taken care of in their home is also increasing. Homecare nursing is one of the health care service for the patients at home who require continual attention and care, and now increasing attentions are given to it as one of the professional nursing fields. However, it was almost impossible to find a study on the actual experiences of the homecare nurses written by their own language in Korea, that it also posed a great difficulty in understanding their diverse experience. Considering these situation, this study will help understanding of them, and provide the fundamental data on their experiences for making policies to develop homecare nursing. METHODS OF RESEARCH: Phenomenological research method was employed to analyze the lived experiences of homecare nurses fundamentally.
DATA COLLECTION: Data were collected from August 1998 to December 1998 from ten homecare nurses who worked for patients under the homecare nursing setting as model cases designated by Seoul Nurses Association and who agreed to the purpose of this study after listening to and understanding the explanation completely. The in-depth interview was carried at the time which was convenient both for the researcher and participants for one or two hours, and recovered with the approval participants. The first interview covered diverse and broad areas like the situation of homecare nursing, and their feelings and thoughts over it, and in the second and third interviews, more specific questions are asked.
DATA ANALYSIS: For the phenomenological analysis, contents analysis was employed. The data collected from the participants were analyzed into the following procedures according to Van Manen 's phenomenological analysis.
1) Reserve the preconception of the researcher by restricting it inside parenthesis.
2) Make a thorough observation of the lived experiences by insight process.
3) Analyze the contents (Find out the repetitive factors)
4) Interpret the essence found.
5) State the meaning of the interpretation. RESULTS AND DISCUSSION:
1. Fear and expectation for the first visit. (unfamiliarity, awkwardness, anxiety, shivering)
2. Mingle with the family (feeling friendly with the family, becoming like a family member)
3. Being proud of her own know-how (learning the know-how, organizing alternatives, building up
confidence)
4. Pity for the poor. (criticizing the current government, feeling ashamed, feeling anger)
5. Difficulty of constructing cooperative system with physicians (strenuousness, frustration)
6. Helplessness due to the lack of support system (difficulty to get supplies,
annoyance, embarrassment by institutional restraints)
7. Anxiousness for heavy traffic and parking (annoyance, hastiness)
8. Ethical conflicts (pity for the patients and family, skepticism about lengthening life maintenance)
9. Burden for the possible accident (pressure, anxiety, conflict, physical exhaustion)
10. Establishment of identity as a professional (fulfillment, worth, joy)
11. Being distressed at other's ignorance

This study aimed to analyse the test contents of the national examination for the registered nurses (NERN) over 3 years from 1991 to 1993 in Korea. In recent years in Korea, the MCQ(multiple choice question) has been showing to be a highly recognized method for assessing the qualification of registered nurses. Unfortunately, nursing faculties have found NERN had some bad MCQs through having evaluation workshop for Some MCQs often provide so many unwriting clues which become a bias of the results, and some items fell into the category of the lower level of educational taxonomy such as isolated recall a fact or data. Frequently the stems of the questions are ambigous, unclear, disputable, esoterical or trivial. Considering those fallacies of the national examination, it is very critical to review the test items to see whether it is of high quality, is more fair, reliable and objective in depth. Therefore, this study was done to provide data for the improvement of the test contents as well as the teachers's assessment skill. For this study, the ad hoc committe was composed of 16 members, including 5 education board members of Korean Academic Nurses Association and 11 nursing faculty members. This committe had one day panal discussion and filled the checklist for this study. The porcess of analysing data was held over 10 times during 1992-1994. The analysis focussed on educational taxonomy such as cognitive domain(kowledge), psychmotor domain (skill), affective domain(attitude) and the level of learning such as recall, understanding, problems solving, and learning area of theory and practice, and the learning content categorised by nursing process and disease process. The test analysed using difficulty index and the structure of the test items was analysed. The conclusions and suggestion as follows: 1. In learning area, the average ratio of the theory and practice was 1: 1.1 which was less than 1: 2 suggested by Korean National Health Institute, and the ratio was differnt by the 8 learing sujects of nursing. 2. In category of the educational taxonomy, the knowledge domain was emphasized mostly(79. 7%), the skill domain was 14.9%, and the attitude domain was 5.4% only. 3. In the level of learning, generally, the test items of the level of recall(45.5%) and the understanding (46.3%) were covered almost and the problem solving was 8.1%. 4. In the learning contents, generally, the test items related to nursing process was 67.2% and that of disease process was 32.8%. However, this proportion was different by the 8 leaning subjects. Even though the nursing diagnosis has been emphasized in nursing curricula recently, the test items of this was identified very few. 5. In the structure of the test item, some were not clear, incorrect grammar, unclear description and some have clues to answer. 6. In the item analysis, the non-acceptable level of the difficulty indes(means too easy) was 65.7%, and the acceptable level was 33.9%. Considering the reseults we would like to suggest the followings, 1. Since the test items of knowledge domain was dominant, the test items of the practice domain and attitude domain should be emphasized more. 2. The regular review and analysis of NERN should be arranged in order to improve the quality of the test items which will give influence to the nursing education positively.

The purpose of this study was to exame the effectiveness of the hand massage as an independent nursing intervention for the cancer patients undergoing radiation therapy. The data were collected with quasiexperimental research design from 40 subjects -20 for experimental group and 20 for control group -of the radiation oncology department of one university hospital in Seoul from January 10th to March 29th, 1994. Subjects agreed to participate in this study and ranged in age from 18 to 82 years with mean age of 52.63. They had insight of their disease and had undergone radiotherapy at least twice. The hand massage developed by Mariah Snyder (1993) based on the Swedish Massage was administered to the experimental group prior to having radiation therapy at the waiting place of radiotherapy room. Hand massage was given to each hand of patient for five minutes once a day. Measurement of the patient's blood pressure, pulse rate, state anxiety and mood were done before and after the administration of hand massage for 5 days. The data were analyzed with paired t-test, t-test and the results were summarized as follows; 1. After administration of the hand massage, the diastolic blood pressure of experimental group was decreased significantly, while the systolic blood pressure and the pulse rate were decreased, but not significantaly. 2. In the experimental group, the level of anxiety was significantaly lower than control group. The mean scores of the difference of the anxiety level after having hand massage was 8.60 in the experimental group and 0.75 in the control group. 3. The difference of the mood scores before and after the massage in the experimental group was decreased significantly. The mean mood score was decreased 4.80 in the experimental group as compared with 2.02 in the control group. 4. The patients after having the hand massage expressed subjectively their feeling of relaxation, well-being and being enjoyable, the significant decreased of physical symptoms and anxiety. The findings summarized that the use of the hand massage for the cancer patient undergoing radiation therapy produced significant changes in the level of anxiety and relaxation. Therefore it is recommended to use the hand massage as an intervention for the patients.

The study was conceived in relation to a concern over the growing gap between the needs of chronic patients and the availability of care from the current health care system in Korea. Patients with agonizing chronic pain, discomfort, despair and disability are left with helplessly unprepared families with little help from the acute care oriented health care system after discharge from hospital. There is a great need for the development of an alternative means of quality care that is economically feasible and culturally adaptable to our society.
Thus, the study was designed to demonstrate the effectiveness of home health care as an alternative to bridge the existing gap between the patients' needs and the current practice of health care. The study specifically purports to test the effects of home care on health expenditure, readmission, job retention, compliance to health care regime, general conditions, complications, and self-care knowledge and practices. The study was guided by the operations research method advocated by the Primary Health Care Operations Research Institute(PRICOR) which constitutes 3 stages of research : namely, problems analysis solution development, and solution validation. The first step in the operations research was field preparation to develop the necessary consensus and cooperation. This was done through the formation of a consulting body at the hospital and a steering committee among the researchers.
For the stage of problem analysis, the Annual Report of Seoul National University Hospital and the patients records for last 5 years were reviewed and selective patient interviews were conducted to find out the magnitude of chronic health problems and areas of unmect health care needs to finally decide on the kinds of health problems to study. On the basis of problem analysis, the solution development stage was devoted to home care program development as a solution alternative. Assessment tools, teaching guidelines and care protocols were developed and tested for their validity. The final stage was the stage of experimentation and evaluation. Patients with liver diseases, hemiplegic and diabetic conditions were selected as study samples. Discharge evaluation, follow up home care, measurement and evaluation were carried out according to the protocols of care and measurement plan for each patient for the period of 6 months after discharge. The study was carried out for the period from Jan. 1987 to Dec. 1989.
The following are the results of the study presented according to the hypothesis set forth for the study :
1. Total expenditures for the period of study were not reduced for the experimental group, however, since the cost per hospital visit is about 4 times as great as the cost per home visit, the effect of cost saving by home care will become a reality as home care replaces part of the hospital visits.
2. The effect on the rate of readmission and job retention was found to be statistically nonsignificant though the number of readmission was less among the experimental group receiving home care.
3. The effect on compliance to the health care regime was found to be statistically significant at the 5% level for hepatopathic and diabetic patients.
4. Education on diet, rest and exercise, and medication through home care had an effect on improved liver function test scores, prevention of complications and self-care knowledge in hepatopathic patients at a statistically significant level.
5. In hemiplegic patient, home care had an effect on increased grasping power at a significant level. However, there was no significant difference between the experimental and control groups in the level of compliance, prevention of complications or in self-care practices.
6. In diabetic patients, there was no difference between the experimental and control groups in scores of laboratory tests, appearance of complications, and self-care practices.
The above findings indicate that a home care program instituted for such short term as 6 months period could not totally demonstrate its effectiveness at a statistically significant level by quantitative analysis however, what was shown in part in this analysis, and in the continuous consultation sought by those who had been in the experimental group, is that home health care has a great potential in retarding or preventing pathological progress, facilitating rehabilitative and productive life, and improving quality of life by adding comfort, confidence and strength to patients and their families.
For the further studies of this kind with chronic patients it is recommended that a sample of newly diagnosed patients be followed up for a longer period of time with more frequent observations to demonstrate a more clear-cut picture of the effectiveness of home care.

The purposes of this study are to delineate a profile of the state of a stroke patient's adaptation at 3 months after hospitalization and to explore the relationship between the level of adaptaion and the variables which influnence the adaptation of hemiplegic patients. To these ends, theoretical framework was derived basically from the stress adaptaion model. The basic assumption underlying the level of adaptation is influenced by the presenting focal, contextual and residual stimuli. This group of stimuli is further operationalized and represented by a perception of stress, which is the perceived effect of the disability and by the mediating variables such as sociodemographic factors as an external conditioning variables and perceived social support and hardiness pcraonality chara cteristics as an internal intervening variables. The dependent varibales in this study is the level of physical, psychological and social adaptation and is hypothesized to be a function of the interaction between 3 sets of variables namely, the perceived disability effect, external conditioning variables and internal intevening varibles. A total of fourty three subjects from 3 general hospitals in Seoul were observed and interviewed with the aid of 7 structured instruments. The data were collected twice on each subject: first, at the predischarge period and at 3 months post - discharge from hospital for the second time. The study was carried out for the period from Februry to August, 1988. The instruments used for the study include 4 existing scales and 3 scales developed by the researcher for this study. They are : 1) The ADL dependency scale and the scale of the clinical physical functions for the assessment of physical adaptation. 2) the SDS(self report of depression) to measure the level of psychological adaptation. 3) The scale for the amount of social activities for the measurement of the level of social adaptation. 4) The scale for the perceived effect of disability for the measurment of the focal stimuli. 5) The health related hardiness scale and the perceived interpersonal support self evaluation list(ISEL) for the measurement of the hardiness personality character and the perceived social support. The data obtained were analyzed using percentage, oneway ANOVA, Pearson coefficients correlation and stepwise multiple regression. The findings provide valuable information about the present level of physical adaptation at 3 months after discharge. The patient revealed a decreased ADL dependency and lowered limitation of physical function as compared with pre - discharge state. Psychologic ally, the average degree of depression at follow up was within normal range of depression. Socially the amount of social activities was very low. The one way ANOVA and the correlational analysis revealed the relationship between the 3 sets of variables and the adaptation level as follows : 1) The perceived disability effect was related to the degree of the depression and the amount of social activities but was not related to the physical adaptation. 2) Among the sociodemographic variables, sex and education were related to the difference of ADL dependency and the change of physical function. These factors indicate that women more than men and educated more than the less educated were found more independent. The education was also related to the degree of depression suggesting that the higher the educational level, the more well adapted the patients were both physically and psychologically. Age, marital status and job state were not found to be related to the patient's adaptation level. 3) Among the internal intervening variables, the health related hardiness characteristic was related to the differences of ADL dependency, physical functions and the social activities, indicating that the higher the hardiness character the higher the level of physical and social adaptation. 4) The perceived social support, another internal intervening variable, was related to the degree of depression and the social activities. This data suggest that the higher the perception of social support, the better adapted the patients were psychogically and socially. In summarizing the results of the correlational analysis, the level of physical adaptation was influenced by sex, the years of education and the hardiness character. The level of psychological adaptation was influenced by the years of education, the perceived disability effect and the perceived social support. And the level of social adaptation was influenced by the perceived disability effect, the hardiness character and the perceived social support. The stepwise multiple regression analysis shows findings as follows : 1) The most important factor to explain the difference of ADL dependency was sex, indicating females were more independent than males. 2) The most important factor to explain the difference of physical function and the degree of depression was the patient's education level. 3) The strongest explaining factor for the amount of social activities was perceived self esteem (one of the subconcepts of perceived social support). Thus the most important factros influencing the level of adaptation were found to be sex, education, the hardiness character and self esteem. From the above findings, the significance of this study can be delineated as follows : 1) Corroboiation. of the assumed relationship between the various variables and the adaptation level as sug gested in the conceptual model. 2) Support for the feasibility of the cognitive app roach for nursing intervention such as hardiness character training, counselling and teaching for self-care in the chronic patients.

This study was designed to measure the degree of work stress perceived by clinical nurses working in psychiatric ward, and to identify the factors influencing the difference of work stress. The survey method was used and the subjects of this study consisted of 135 psyciatric nurses from 7 university hospitals and 4 provincial general hospitals, The data were collected from August 1 to 20, 198 8. The instrument used for this study was PNOSS (Psychiatric Nurse Occupational Stress Scale) which was developed by Bai(1989). The confidence verific ation of this
instrument was computed and the Cron -bach a was 0.94050. Data were analysed by t-test and ANOVA on 5 % significant level with SPSS program. The results of study were summerized as follow :
1) The degree of perceived stress among psychiatric nurses was considerably high (4.32 out of 6.00). 2) Among the stressors, inadequate staffing(5.04), hospital administrational problems (4.7) and the conflict of nurse-patient relationship were identified as the stress factors with high rank of degree of stress. 3) There was significant relationship between the degree of stress and the demographic variables of nurses such as the marital status, educational level, and the motivation of working in psychiatric ward. 4) There was significant relationship between the job-related variables such as the working duration, the motivation of working and nurse's attitude toward the psychiatric patients. 5) The relationship between several stress factors and some of the demographic and the job-related variables were significantly identified.

This study of 855 clinical nurses was conducted using a questionnaire that include four different scales; the motives of determining their profession, the attitudes toward their profession, the general items, and desire and expectation to their profession and society. The data were analyzed by Chi-Square Test and Percentage. The results of this study included Hypothesis are as follows; The respondents were 855 (78.630 among 1088 clinical nurses who were employed by General of Educational hospitals through the city of Seoul, Pusan, Tacgu, Daejun, Kwangju. and Wonju. 1) a. In the Age Distribution, the majority of respondents were under the age of 30yrs(88.2%) and the minority were above 31yrs(11.2%), and the preponderance of the majority to minority(9:1) was noted. In compared with area, a group above 31yrs old in Seoul (6.9%) was lower than other area (16,3%). b. The types of Educational background were 16.3% in Voc.Tr. School, 66.5% in Diploma and 17.1% in Degree. 146 clinical nurses were from the Degree course, and 142 (97.3%) CN among those of them were occupied around Seoul and 4(2.7%) around other area. c. In the Marital Status, 71,5% were the unmarried and 28.5% were the married. And compared with the area was 20.4% in Seoul and 41.4% in other area. d. Most common Lenght of Clinical Experience after graduation was under the 2yrs(55.4%), 3yrs(14.2%5, and 4yrs (6.2%). In compared with area, Seoul(15.3%) was lower than other area (38.1%) above 5yrs of clinical experience, and the preponderance of the other area to Seoul as 2.5: 1 was noted. 2) a. Hypothesis 1 was significant relation between the types of Educational Background of the CN and their motives for selection of Nursing, P-value was below 0.01. b. There was a significance on hypothesis 2 (P<0.01)i that was relation between their motives for selection of clinical nursing field after their graduation and the area which they were employed. c. Hypothesis 4 was accepted as significant relation between the level of satisfaction of their clinical experience after their graduation and the types of educational back- ground, P-value wa below 0.01. d. There was a significance on hypothesis 5(P<0.01) that was relation between the CN's response about the orientation program and the area which they were employed. e. Hypoth esis 6 was retained as significant relation between the area and inservice cduca- tional programme of their employed hospital was practising or not. P-value was 0.01. f. Hypothesis 7 was retained as significan relation between the area and the CN's response about the inservice educational programmme of their employed. P-value was below 0.01. g. There was a significance on hypothesis 8 (P<0.01) that was relation between the CN's experience en attending the professional meeting and the area. h. Hypothesis 10 was accepted as significant relation between the response about the present licence system and their educational background. P-value was below 0.01. i. There was a significance on hypothesis 11 (P<0.01) that was relation between the carrying out the regular and delivery vacation and the area. j. Hypothesis 12 was accepted as significant relation between the CN's consideration of the lack of leisure and their marital status. P-value was below 0.01' k. There was a significance on hypothesis 13 (P<0.01) that was relation between the CN's response about their salary and their marital status. l. Hypothesis 14 was significant relation between the most difficulties of CN during their working and the hospital which they were employed. P-value was below 0.01.
