A Preliminary Study on Development and Evaluation of Home Health Care Nurse Clinical Practice Program: Focused on Postoperative Orthopedic Patients

The clinical practice program for home care nurses was implemented in June 1994, to help to set up a hospital-based home care system in the Kwangju City area as a collaborative work between the Department of Orthopedic Surgery at Chunnam University Hospital and Chunnam University School of Nursing. Under the developed clinical practice strategy, the eight week training was given to five licensed home care nurses who had completed Part I and II of the home health care nursing practicum from June 1994. The purpose of this descriptive evaluation study was to identify the effectiveness of the clinical practice program for home care nurses specialized in the area of patient care for people with musculoskeletal function impairment. As a method in data analysis, data triangulation was used in the five home care nurse case evaluations. The variety of data analyzed included confidence score by home care nurse self-evaluation, patient and family member satisfaction scores, and competency score by preceptor evaluation. The study findings revealed that an increase rate in nursing performance didrate necessarily coincide with an increase not in competency score and also, not with the patient/family member satisfaction scores. And an order derived from the clinical performance scores of five home care nurses corresponded to those from three measurements-competency score, patient satisfaction score, and family member satisfaction score. However, it differed from the order associated with the confidence score. Consistency derived from the three objective evaluation methods may lead to the possibility that the level of competency measured by educator can be further explained by the levels of patient/family member satisfaction. The salient finding of this study was that, in case of nurse A who had had little clinical experience in the orthopedic patient care, there was a significant increase in the level of confidence and competency in subscale of professional skill with the home care clinical practice. Therefore, the effect of the clinical practice program would be successful for nurses who have had little experience in the area of specialization. The study results suggest that there might be some time difference in the development of cognitive sense (confidence) in performance and actual clinical performance (competency). In future research, relationships between the confidence and competency score, and between the confidence score and the patient satisfaction score should to be measured in different time frame to achieve a better explanation power of the study outcome.

The Effect of Social Support Intervention on Mood and Maternal Confidence of Premature's Mothers

The purpose of this study is to examine the effect of social support intervention on mood and maternal confidence of premature's mothers. The social support intervention is known to induce improved mood state and provide information on caretaking so as to increase the maternal confidence in the mother of a premature. To systematically investigate its effect, this study employed a nonequivalent randomized post-repeated quasi-experimental design. The intervention was given individually to mothers of prematures five times spanning five weeks. The sample consisted of the 50 mothers (experimental 27, control 23) of a premature. The data were collected using the structured questionaires twice as post tests. Various instruments were used in this study. The POMS developed by Lee(1990) was used to measure the mothers' mood state, Mother and Baby Scales by Wolke et al (1987). The results are as follows: 1. For the hypothesis test to see the effect of the social support intervention, the mean of the experimental group and the control group was compared by means of t-test and the following results are obtained. Hypothesis I. "The mood state of mothers with social support intervention is more positive than that of the mothers without such intervention." was not statistically supported and thus discarded (t=.799, p=.429). However the mean scores were 49.68 and 51.38 for the experimantal and control group, respectively, indicating more positive mood for the experimental group. Hypothesis II. "The maternal confidence of mothers with social support intervention is higher than that of the mothers without the intervention." was statistically supported (t=3.667, p=.001). 2. The mean score of the mood state was highest before discharge (52.29), meaning most negative, declined to 49.68 shortly after the discharge, again increased a bit to 50.07 at four weeks after the discharge, and stabilized to 49.22 around six weeks after the discharge. On the other hand the mean score of the maternal confidence was continuously increased with time. In view of the above results, it is concluded that the social support intervention with a preprogrammed protocol has the definite positive effect on increasing the maternal confidence and positive effect on improving mother's mood state.

Maternal Perception of the Newborn and Confidence and Gratification of Mothering Role

The purpose of this study was to evaluate maternal perception of the newborn, confidence and gratification of mothering role, and contribute to maternal nursing and neonatal nursing. The questionnaire, maternal perception of the newborn(I) was completed by 47 mothers at three hospitals in the Kyoung-In area during the first to third day after birth. the questionnaire, maternal perception of the newborn(II), confidence and gratification of mothering role were completed at fourth to sixth week after birth. the data were analyzed by a SPSS program. The results were as follows; 1) The means of perception of the newborn(I) and (II) were 2.28+/-4.81 and 3.83+/-4.84. The means of confidence and gratification of mothering role were 40.38+/-5.51 and 39.30+/-6.70. 2) Half of the mothers(48.9%) rated their newborn as better than average newborn at first to third day after birth, and 74.5% of mothers did at fourth to sixth week after birth. 3) The score of maternal perception of the newborn(I) was significantly correlated with the score of confidence and gratification of mothering role. The score of maternal perception of the newborn(II) was correlated with the score of confidence and gratification of mothering role. The score of confidence and gratification of mothering role was related. 4) Maternal gratification of mothering role was significantly different by maternal perception of the newborn(I), and maternal confidence was different by maternal perception of the newborn(II). 5) Mothers who had religions and who wanted the pregnancy had higher perception of the new-born(I) than those who did not. Mothers whose newborn(II) than those whose newborns were girls. Mothers who wanted the pregnancy had higher confidence of mothering role than those who did not. Mothers who planed cow's milk had lower gratification of mothering role than those who did not. The above findings indicate that mothers who have a different level of perception of the newborn, confidence and gratification of mothering role. Therefore, it is important maternal perception of the newborn, confidence and gratification of mothering role were precisely evaluated, and nursing intervention for improving perception of the newborn should be provided of for mothers.

The Effect of Education Program on Primiparas' knowledge, confidence and Accuracy of Behavior in Newborn Care

This study was to find whether the educational program contributed to the increase of knowledge, confidence, and accuracy of behavior in newborn care of the primiparas. The educational program consists of individual lectures, demonstrations, discussion, and practice of newborn care. Also two telephone counseling with the subjects after they are discharged from hospital. This study is a quasi-experimental design using non-equivalent control group pretest-posttest design. Data collection was done from July 21 to Oct 4 in 1997. the subjects were selected from 2 general hospitals and 1 university hospital in C city. Subjects were 44 primiparas(control group 22, experimental group 22). they were tested on knowledge, confidence, and accuracy of behavior in newborn care. A pretest was done 2-3 years after vaginal delivery(5-6 days after c-sec delivery). A posttest was done 21-28 day(vaginal delivary, c-sec delivary) after delivery. The instruments used for this study were knowledge scale about newborn care developed by the researcher, Pharis' confidence scale modified by the researcher and accuracy of behavior scale developed by the researcher. Primiparas' knowledge and confidence was tested by questionnaire and Primiparas' accuracy of behavior was tested by structured observational method. Analysis of data was done by using of x2-test, t-test, paired t-test. The results of this study are summarized as follows; 1) Knowledge of the experimental group was significant higher than the control group(t=-4.94, P=.000). 2) Confidence of the experimental group was significant higher than the control group(t=-.262, P=.012). 3) Accuracy of behavior of the experimental group was significant higher than the control group(t=-.969, P=.000). In conclusion, the newborn care education along with intensive telephone counseling shows a significant promotion of newborn care in primiparas. Thus this program can be recommended as an intervention model for the newborn and primiparas.

Effects of Kangaroo Care on Anxiety, Maternal Role Confidence, and Maternal Infant Attachment of Mothers who Delivered Preterm Infants

PURPOSE: The purpose of this study was to examine the effects of Kangaroo Care(KC) on anxiety, maternal role confidence, and maternal infant attachment of mothers who delivered preterm infants. METHODS: The research design was a nonequivalent control group pretest-posttest. Data was collected from September 1. 2006 to June 20. 2007. The participants were 22 mothers in the experimental group and 21 in the control group. KC was applied three times per day, for a total of ten times in 4 days to the experimental group. RESULTS: The degree of anxiety was statistically significantly different between the two groups but maternal role confidence and maternal infant attachment was statistically insignificant. CONCLUSION: This data suggests that KC was effective for mothers anxiety relief but it was not effective for maternal role confidence and maternal infant attachment of mothers. The implications for nursing practice and directions for future research need to be discussed.

The effect of a Group Therapy for the Offenders of Family Violence

Purpose

The purpose of this study was to evaluate the contribution of group therapy to the improvement of self-esteem, anger, stress coping and communication of those who were violent in to family.

Methods

The research design was a one-group-pretest-posttest design in quasi-experimental research, and the period of this study was from March to October, 2005. Participants were 14 offenders who were ordered to be counseled according to 'the special exemption law for punishing domestic violence.' Group therapy was applied once a week for 8 weeks. Results were collected by using a questionnaire for self-esteem, anger, stress coping and communication. In the data analysis, Wilcoxon signed test with SPSS/WIN 12.0 program was used.

Results

The scores of self-esteem and communication showed statistically significant improvement from pre to post therapy. However, the scores of anger and stress coping were not significantly changed from pre to post therapy.

Conclusion

This group therapy was effective in improving the self-esteem and communication in offenders of family violence.
