A Study on the Disturbing Factors Which Work against Therapeutic Atmosphere and Environment on Hospital Wards as Perceived by Patients and Nurses

As a descriptive survey, this study was attempted to get basic data necessary to recognize the factors that disturb the therapeutic atmosphere of hospital wards as perceived by nurses and hospitalized patients, to identify differences between the perceptions of the nurses and of patients. The subjects, 159 patients in Pusan National Hospital and 68 nurses working there were sampled between March 18 and April 13, 1996. The tool used to measure the disturbing factors was an amended form of the one developed by Kim, Mae Ja(1983). The differences between each subject's score for each factor were analyzed using means and SD, and the highest 3 items above the mean score for each factor were collected and compared. The results are described below : 1. Subject's perception of main disturbing factors : patients reported that the main factors were 'loss of role and economic trouble', 'the prognosis of disease', 'the change of daily life' but nurses replied that the main factors were 'the prognosis of disease', 'the communication trouble with the medical team and interpersonal relationships'. 'The change of daily life' was not a perceived factor by nurses, but ranked third by patients. 2. Subject's perception degree of each disturbing factor : (1) among the items related to interpersonal relationship, the patient group reported that the worst disturbance was due to severely ill patients in the same room' but the nurse group regarded 'greed to monopolize wheelchairs or other supplies' as the worst disturbance. (2) among the items related to physical factors, the patient group regarded 'limitations to wash their body, physical pain and limitations in physical activity' as the worst disturbance, but the nurse group regarded 'physical pain', and 'limitations to activity or change of appearance' as the worst disturbance. (3) among the items related to the change of daily activity, the patient group regarded 'the boredom of hospitalization or in favorable diet' as the worst disturbance, but the nurse group regarded 'too much noise or unclean room' as the worst disturbance. (4) among the items related to the communication trouble with medical team, the patient group regarded 'the ignorance of their disease due to poor information, the inability to understand the language of the medical team or the difficulty in seeing physician in time' as the worst disturbance, but the nurse group regarded 'the inability to trust physicians and physician's poor attention to patients' as the worst disturbance.

The Effect of Infant Massage on Mother-Infant Play Interaction

PURPOSE: This study is attempts to clarify the effect of infant massage for the promotion of primipara's mother-infant interaction METHOD: The term for collecting data for experimental group ranged from April 25, 2001 to June 5, 2001. The infants for this group were sampled among normal mother-infant from one postpartum care center located in J city. The term for collecting data for control group ranged from June 10, 2001 to August 3, 2001. The infants for this group were sampled among normal mothers infant from 1 general hospital, 1 university hospital and 1 postpartum care center located in J city. The experiment was implemented giving primipara education about massage based on protocol for infant massage provided by Johnson and Johnson Korea and they received 10 days of education, 10 minutes a day (from 10 to 11 a.m) In the post test, we videotaped both the control group and the experimental group visiting their homes 4 weeks after delivery to observe mother-infant play interaction. Data analysis was done using SAS and the homogeneity between general properties owned by both control group and experimental group and mother's perception scale for children was verified through x2-test. Mother-infant play interaction with both control group and experimental group was analyzed through t-test in the experiment. And analysis of mother-infant interaction points based on general properties was made using ANOVA and t-test. RESULT: Hypothesis that mother-infant play interaction with primipara who gave her infant a massage will be more active than that of the primipara who didn't was verified (t=-4.27, p=.0001). And the points in each item, points in each item were estimated as follows. Mother behavioral items (t=-4.96, p=.0001), infant behavioral item (t=-0.36, p=.71), mother-infant interaction reciprocity (t=-2.64, p=.01). CONCLUSION: An infant massage program can contribute to promoting the Mother-Infant Play Interaction positively.

A Study on the Triage and Statitical Data of Patients in the Emergency Room, PNU

The purpose of this study is to analyze ER patient's Triage and other statistical data. The subjects were 12,618 patients who visited the ER during the year 1998. The study showed the following results; 1. The male vs female ratio was 1.3 : 1.0, the male were in the majority (56.6%), and the age range of 20-29 old was the majority (15.3). The patients who visited ER at 8-10 pm were the majority (11.5%). On Sunday the number of patients who visited the ER were 2,189, and the majority were 17.4%. On Saturday the number of patients was visited the ER were 1,944 patients the second majority (15.4%). Their traffic means : the general passenger cars (75.5%), 119 or hospital ambulance (11.3%). 2. The reasons of visiting ER were : diseases (59.2%), injuries (23.7%). The disease vs injury ratio was 100 : 69. 3. Triage : urgent 40.7%, non-urgent 38.2%, acute 17.8%, and critical 3.2%. 4. The time of waiting and staying in the ER by the Triaget: the average time was 572 minutes (9.53 hrs.). The majority of critical patients (20.5%), acute patients (24.7%) and urgent patients (21.2%) stayed 12-24 hrs., but the majority of non-emergent (27.8%) stayed not longer than one hour. 5. Treatments by the Triage : the 42.9% of critical patients, and 61.3% of acute patients, 57.5% of urgent patients were admitted. But 91.8% of the non-emergents were discharged and 4.7% was admitted. Mortality of total ER visiter were 1.7%. DAA portion was 0.86%. 26.6% of the critical patients were DAA. DAA vs DOA ratio was 1.3 : 1.0. 6. Visiting time, monthly and seasonal distribution by the Triage : the majority of critical patients (12.2%), visited 10-12 am. The majority of acute (12.9%) and urgent (11.7%) visited 4-6 pm, but the majority of non-emergents (15.1%) visited during 8-10 pm. Autumn visiter were the majority (27.6%). The percentage of non-emergent visited in Spring was 41.4% and Autumn was 41.3%. The percentage of urgents who visited in the Summer was 45.3% and the Winter was 40.4%. By clinical departments: the 48.0% of critical patients was NS. The 45.5% of acute and the 33.6% of urgent patients were IM. But the majority of non-emergent patients was PS (21.2%), and the second majority of non-emergent patients was oral Surgery (12.8%).

A Study of the Effects on the Korean Adolescents' Sexual Role Identity and Attitude by the Audio-Visual Program for Sexual Education

The aims of this study were to identify the effects on sexual role identity and sexual attitude of the A-V programed sexual education on Oct. 1. 1997 through Sep. 30, 2000. The program was focused on the formation of the desirable sexual activity, attitude and androgyny sexual role identity of the Korean adolescents, and that was the 25 minute VTR media "Our sexuality is healthful". This study was non equivalent post-test only quasi-experimental design, and the subjects were 530 middle school boys in Busan, Korea. After the manipulation, the change of sexual role identity and sexual attitude was analysed. Outcome measures were middle school boys' sexual role identity toward KSRI on a seven-point Likert scale and sexual attitudes toward SAS on a five-point Likert scale. The data was analysed by SPSS WIN. The results were summarized as follows: 1) The experimental group who was exposed to the A-V media produced by the author showed the higher score of androgyny sexual role identity than the control group who didn't watch the A-V program. 2) The experimental group showed the higher rate of the androgyny than the control group, On the other hand, their rate of the masculinity turned out to be lower than the latter group. 3) The experimental group didn't show the difference of SAS score from the control group. In conclusion, "Our sexuality is Healthful" A-V program for sexual education brought about the significant change of sexual role identity of the middle school boys, but didn't affect their attitude toward sexual activity.

Discriminating Power of Socio-demographic and Psychological Variables on Addictive Use of Cellular Phones Among Middle School Students

PURPOSE: The purpose of this study was to examine the degrees of cellular phone usage among middle school students and to identify discriminating factors of addictive use of cellular phones among sociodemographic and psychological variables. METHODS: From 123 middle schools in Busan, potential participants were identified through stratified random sampling and 747 middle school students participated in the study. The data was collected from December 1, 2004 to December 30, 2004. Descriptive and discriminant analyses were used. RESULTS: Fifty seven percent of the participants were male and 89.7% used cellular phones at school. The participants were grouped into three groups depending on the levels of the cellular phone usage: addicted (n=117), dependent (n=418), non-addicted (n=212). Within the three groups, two functions were produced and only one function was significant, discriminating the addiction group from non-addiction group. Additional discriminant analysis with only two groups produced one function that classified 81.2% of the participants correctly into the two groups. Impulsiveness, anxiety, and stress were significant discriminating factors. CONCLUSION: Based on the findings of this study, developing intervention programs focusing on impulsiveness, anxiety and stress to reduce the possible addictive use of cellular phones is suggested.

The Experiences of Patients in Intensive Care Units(ICU)

PURPOSE: The purpose of this study was to describe the essence of the experiences of patients in an ICU, and to understand them from the patients' point of view. METHODS: Participants in this study were six patients in P hospital. Data collection consisted of in-depth interviews and an observation method done from January to April in 2005. The method was analysis using the phenomenological method proposed by Colaizzi(1978). RESULTS: The themes were classified into eight theme clusters. The eight theme clusters were finally grouped into four categories, 'shock', 'pain', 'gratefulness' and 'pleasure of revival'. CONCLUSION: The ICU patients had negative experiences in physical.mental critical situations, but also positive experiences in consolation and nurses and families' encouragement. Therefore, ICU nurses must support patients and their families to minimize the negative experiences and maximize the positive experiences.

The Effectiveness of the Error Reporting Promoting Program on the Nursing Error Incidence Rate in Korean Operating Rooms

Purpose

The purpose of this study was to develop and evaluate an error reporting promoting program(ERPP) to systematically reduce the incidence rate of nursing errors in operating room.

Methods

A non-equivalent control group non-synchronized design was used. Twenty-six operating room nurses who were in one university hospital in Busan participated in this study. They were stratified into four groups according to their operating room experience and were allocated to the experimental and control groups using a matching method. Mann-Whitney U Test was used to analyze the differences pre and post incidence rates of nursing errors between the two groups.

Results

The incidence rate of nursing errors decreased significantly in the experimental group compared to the pre-test score from 28.4% to 15.7%. The incidence rate by domains, it decreased significantly in the 3 domains-“compliance of aseptic technique”, “management of document”, “environmental management” in the experimental group while it decreased in the control group which was applied ordinary error-reporting method.

Conclusion

Error-reporting system can make possible to hold the errors in common and to learn from them. ERPP was effective to reduce the errors of recognition-related nursing activities. For the wake of more effective error-prevention, we will be better to apply effort of risk management along the whole health care system with this program.

A Comparative Study on Family Perception between Abused Children and Normal Children by Kinetic Family Drawing

Purpose

The study attempted to find family perception differences between abused children and normal children by Kinetic Family Drawing.

Method

The subjects of the study consisted of two groups, 143 abused who were in the upper 25th percentile, and 150 normal who were in the lower 25th percentile. Collected Kinetic Family Drawings were divided into five dimensions such as actions, human figure characteristics, dynamics, styles and symbols, and they was analyzed with SPSS/WIN 10.0.

Results

In the perception about their family in action dimension, their family in figure characteristics dimension, their family in dynamics dimensions, and their family in symbols dimension, there is a sharp contrast between the two groups.

Conclusion

Putting these results together, abused children feel lower self-esteem and feel more sense of alienation in their family than normal children do. In addition, abused children perceive their parents as negative and aggressive people.

Effects of Walking Exercise on Metabolic Syndrome Risk Factors and Body Composition in Obese Middle School Girls

Purpose

This study was done to determine the effects of walking exercise training(WET) on metabolic syndrome risk factors and body composition in obese middle school girls.

Method

A non-equivalent pretest-posttest experimental design was used. Twenty seven subjects participated in this study from one women's middle school in Busan. The participants were purposely allocated to an experimental group (n=14) and a control group (n=13). The experimental group participated in 30-60 minutes of WET with 55 to 75% of a maximal heart rate six days a week for 12 weeks.

Results

The prevalence of individual risk factors on metabolic syndrome were improved in the experimental group after the intervention. The high waist circumference(WC), high triglyceride(TG), low high density lipoprotein cholesterol(HDL-C), high blood pressure(BP) and high fasting glucose(FG) were 21.4, 21.4, 14.3, 28.6 and 7.2%, respectively. There were significant differences in WC (F=22.24, p<.001), TG (F=5.34, p=0.30), body weight(F=21.99, p<.001), fat mass(F=19.17, p<.001), and % body fat(F=17.93, p<.001) between the experimental and control group after the intervention. However, there were no significant differences in HDL-C, FG and BP between the experimental and control group after the intervention.

Conclusion

These results indicate that WET is effective in decreasing risk factors of the metabolic syndrome and body composition components in obese middle school girls. These findings suggest that WET can be useful as a nursing intervention in the prevention of obesity-related disorders in obese adolescents.

The Experiences of Perioperative Patients with Cancer

Purpose

This study was to explore the experiences of perioperative patients with cancer. The purpose of this inquiry was to describe the essence of such experiences, and to understand them from the patients'point of view.

Method

Participants in this study were 9 cancer patients with ages ranging from 31 to 70 years old, living in B city, who experienced cancer operations. Data collection consisted of in-depth interviews and an observation method done from October to December in 2003. In analysing data, Colaizzi's(1978) phenomenological research method was adopted.

Results

Five categories emerged from ten theme clusters. The five categories were: ‘ shock’, ‘ expectations & wishes’, ‘ despair’, ‘ feelings of burden’, ‘ continual pain’.

Conclusion

The results of this study showthat oncology nurses need to pay more attention to the psychosocial aspect of nursing to empathize and support the cancer patients who suffer and its treatment, and operations.

Conquering Process of the Group-bullies among Teenagers

Purpose

This study was to develop a substantive theory on the conquering experience of group-bullies among teenagers, and thereafter suggest a model for solving the problems.

Method

Data collection and analysis was proceeded by a method of Grounded Theory. Data was collected by interviewing the participants. Five teenagers participated in this study who had already overcome the situation.

Result

In the analysis 31 concepts, 6 categories, and 14 subcategories are extracted from the raw data, and all are constituted on the paradigm model. The causal condition is a painstaking of participants. The context is affirmative reconsidering and the phenomenon is conquering difficulties. The intervening condition is a positive intervention of a supporting system and the strategy for action/interaction is building a relationship. The consequences of this analysis is harmonizing.

Conclusion

The results of this study suggest adequate ways of conquering group-bullies. These are positive attitudes towards solving problems and the full use of support systems surrounding them.

The Effect of Handwashing Improving Program and MRSA Carrier Screening Program on the MRSA Infection Rates in an Intensive Care Unit

Purpose

To assess the effect of handwashing improving program and MRSA carrier detection program on MRSA(methicillin resistant Staphylococcus aureus) infection rate in a intensive care unit.

Method

The intervention was Nosocomial Infection(NI) control program consisted of hand washing improving program and identification and treatment of MRSA carrier. Data on the NI and MRSA infections were collected by an infection control nurse based on the definition of CDC. MRSA infection rates were calculated by the number of MRSA infection per 100 admissions or 1,000 patients-days. The difference of MRSA infection rates between pre and post intervention was tested by Chi-square at =.05.

Result

MRSA infection rates 3.0% or 3.2 per 1,000 patient-days at the pre, 4.6% or 3.7 per 1,000 patient-days at the post, and the differences were not statistically significant (p=.411, p=.769 respectively).

Conclusion

The handwashing improving program and MRSA carrier detection program was not effective in reducing the Nosocomial Infection(NI) or MRSA infection rates. It is recommended further studies with a longer intervention and follow-up period.

Effects of Body Position and Time after Feeding on Gastric Residuals in LBW Infants

Purpose

To examine the effects of body position and time after feeding on gastric residuals in low birth weight infants(LBW).

Methods

A repeated measures design was conducted. Twenty LBW infants being fed via indwelling nasogastric tubes were randomly assigned to one of 5 different position orderings. In each position, gastric residuals were measured at 30, 60, 90, 120, 150, and 180 minutes after feeding.

Results

In comparisons of gastric residuals with position and time, the main effects of position and time were statistically significant (F = 5.038, p = 0.001; F = 429.763, p < 0.001, respectively), but the interaction between position and time was found not to be significant. In pairwise comparisons, the gastric residuals were more reduced in the right anterior oblique and prone positions compared with the left lateral position (P < 0.05) and the differences across time periods were significant (p < 0.001).

Conclusion

The body position after feeding has a significant affect on gastric residuals over time in LBW infants. The right anterior oblique or prone position is recommended rather than left lateral position after feeding. Knowledge of the proper position and the pattern of gastric emptying over time after feeding may lead to the development of evidence-based nursing care.

A Study to the Coping Patterns of Cancer Patients

Purpose

This was to describe the emotions that patients face when diagnosed with cancer to know the problems and coping styles that cancer patients experience during the treatment.

Method

The qualitative method was used for this study. The participants were 90 cancer patients at five general hospital in Pusan, Korea. Data were collected by interviews with a semi-structured questionnaire from November 2000 to June 2001, and were analyzed using the modified constant comparative method.

Result

The most common emotions on the diagnosis of cancer were shock(36.7%), followed by despair(25.6%), acceptance(24.4%), denial(18.9%), complaint(16.7%), and fear(8.9%). The problems identified were the unpleasantness and physical discomfort related with the treatment(50.0%), the feelings of burden(41.1%), finance/occupation(38.8%), and fear of the future(26.6%). Coping styles to problems that the subjects have faced on the course of the treatment were compliance(36.6%), health care(31.1%), positive thinking(22.2%), despair/avoidance(15.5%), seeking social support(6.6%), information seeking(3.3%) and self-control(2.2%).

Conclusion

It is very important to develop nursing interventions which can mitigate shock that patients experience, can help cancer patients to have hope for the future and to positively cope with cancer.

The Experiences of Mental Health Hospital Workers

Purpose

The purpose of this study was to understand the experiences of mental health hospital workers.

Methods

Participants in the study were a total of 8 mental health hospital workers who consisted of nurses, social welfare workers and health managers. To prevent them from being omitted, the interviews were all recorded under the participants prior agreement. The method was analysis using the phenomenological method proposed by Colaizzi(1978).

Results

The experiences of the participants of this study were classified into 15 significant areas, from which 10 subjects hard work, conflicts, heavy feeling, irritability, getting familiar, changes of recognition, aptitude determination, feeling of achievement and sense of pride were drawn out. These subjects were then grouped into 5 themes. These five themes were finally grouped into 5 categories, negative emotion, depressive emotion, changes of thinking, delight and value discovery.

Conclusions

The study tried to analyze the experiences of key informants like nurses, social welfare workers and health managers all of whom were serving at mental health hospitals, contribute to social recognition about the special medical establishment, promote qualitative mental health nursing and further provide educational information necessary for understanding mental health hospital workers.
