The Effects of Hospitals’ Family Friendly Management on Married Female Nurses’ Retention Intention: Focused on the Mediating Effects of Work-Family Interface

Abstract Purpose

This study examined the effect of hospitals’ family-friendly management on married female nurses’ retention intention. The focus was the mediating effects of the work-family interface (work-family conflict, work-family enrichment and work-family balance).

Methods

This study was a cross-sectional study. The participants were 307 nurses working at five public and five private hospitals with more than 200 beds in Seoul. Data were collected using structured questionnaires from September 10 to September 17, 2018 and analyzed with SPSS 24.0. Data were analyzed using an independent t-test, a one-way ANOVA, Pearson's correlation coefficients, and multiple regression following the Baron and Kenny method and Sobel test for mediation.

Results

There were significant correlations among family-friendly management, the work-family interface, and retention intention. Work-family conflict showed a partial mediating effect on the relationship between family-friendly management and retention intention. Work-family enrichment showed a partial mediating effect on the relationship between family-friendly management and retention intention. Work-family balance showed a partial mediating effect on the relationship between family-friendly management and retention intention.

Conclusion

These findings indicate that both hospitals’ family-friendly management and nurses’ work-family interface are important factors associated with nurses’ retention intention. Therefore, hospitals should actively implement family-friendly management for nurses and establish strategies to enhance nurses’ work-family interface for effective human resource management.

Development and Effects of Social Learning Theory Based Eye-Health Program for Preschoolers

Abstract Purpose

The purpose of this study was to develop an eye-health program based on social learning theory (EPST) of preschoolers and evaluate its effectiveness.

Methods

A nonequivalent control group pre-post test design was utilized and 141 six-year-old preschoolers and their parents participated (experimental group=69, control group=72) in the study. The EPST in this study included eye-health education and eye exercises. Attention, memory, replay, motivation, reinforcement, and self-efficacy were used as interventional strategies. To examine the effectiveness of EPST, proficiency in eye-health activities, refractive power, and visual acuity were measured before and after the intervention. Data were analyzed with SPSS WIN 21.0 using the Shapiro-Wilk test, χ 2-test, Mann-Whitney U test and Wilcoxon signed rank test.

Results

Following the intervention, eye-health activities, refractive power, and visual acuity significantly improved in the experimental group compared to the control group.

Conclusion

The results of this study suggest that EPST is effective in improving eye-health activities, refractive power, and visual acuity in preschoolers, and its wider implementation in educational institutions will promise improved eye-health among preschoolers.

A Comparative Study of the Effect of Two Analgegic Administration Methods on Post Operative Pain

An acute pain is the common experience following surgery. Pain is a most miserable experience in person and most preoperative patients have fear of postoperative pain. In nursing, it is very important to understand and relieve the pain between group of Patient Controlled Analgesia and group of traditional Muscular Injection in surgical patients. This information can be utilized as data of understanding nursing care and treatment planning for pain in surgical patients. The subjects in this study were 45 post-hysterectomy patients in Gynecology ward in C.N.U.H., in Taejon. Data was collected from May 12 to June 27,1996. The instruments used for this study were subjective Visual Analog Scale, Objective nonverbal pain scale composed of Facial Appearance, Vocal Sound Change. and Sweating score, and the Melzack's Mcgill pain Questionnaire, nine Items of Developmental pain Intensity Scale by Lee Eun Ok. Analysis of data was done by using S.P.S.S. percentage, t-test, x2-test, ANOVA, and Repeated measure ANOVA. Results were obtained as follows. 1. Hypothesis 1 : There was very highly statistically significant difference in subjective self-report pain score(Visual Analog Scale) between PCA Group and IM Group(P=0.0001). 2. Hypothesis 2 : There was very highly statistically significant difference in muscle strength score(Visual Analog Scale) between PCA Group and IM group(P=0.0001). 3. Hypothesis 3 : There was very highly statistically significant difference in facial appearance score between PCA Group and IM group(P=0.0001). 4. Hypothesis 4 : There was very highly statistically significance difference in vocal sound change score between PCA Group and IM group(P=0.0001). 5. Hypothesis 5 : There was no statistically significant difference sweating scores between PCA group and IM group(F=2.50, P=.1220). But, post-operation time of 12, 24 was statistically difference between two groups(P=0.0001) So, it was partially supported. 6. Hypothesis 6 : There was very highly statistically significant difference in vocabulary pain score between PCA Group IM group. 7. Hypothesis 7 : There was very highly statistically significant difference in amounts of total analgesic between PCA Group and IM group. There was very highly statistically significant difference in Visual Analog Pain Score, Facial Appearance Score, amounts of total analgesic between PCA group and IM group. So, It is verified to asses of postoperative pain with, VAS, Checklist of facial appearance, vocal sound change, and sweating, and Vocabulary Scale.

Analysis of Experimental Studies on Master's and Doctoral Theses in Nursing

In This study was carried out to describe and analyze experimental studies conducted in graduates nursing degree. Of 170 experimental studies conducted during the past three decades between 1962 and August 1991, 150 studies were available, including 124 master's and 26 doctoral theses. This study examined their general characteristics and detailed research methods using percentiles. The results were as follows: 1. Most of the studies adopted a quasi-experimental design. 2. The subjects of the studies were chosen by convenience sampling except for two studies whose subjects were drawn by randomization. Studies comparing experimental and control groups were in the highest proportion and the most frequent sample sizes of each group were 21 to 30 for both experimental and control groups. 3. As to measurement, physiological measures were most frequent followed by psychsociological measures and active report questionnaires. Each study, on average, adopted two kind of measurement tools. Studies in which the data collection period was of 1~2 months were in the highest proportion. 4. All doctoral theses and 67.0% of master's theses examined specific research hypotheses. Of these studies, the results of 92.5% supported the hypotheses. 5. Parametric statistics were the major analytical methods. In particular, t-test was used most frequently followed by Chi square, F test, and Pearson Correlation Coefficients. 6. Patients were the most frequent study subjects. Frequent nursing interventions were information and education followed by support, distraction, and nursing treatments. 7. With regards to the dependent variables, "feelings" such as anxiety, pain, and depression were most frequent. In addition, "exchanging" such as restoring, metabolism, cardiopulmonary function, infection and vital signs were adopted as the dependent variables in 29.1% of the studies examined, while 12.3% of the studies selected "choosing" such as stress, health behavior, or role performance.

Effectiveness of a Smoking Cessation Program for Adolescents

Purpose

The purpose of this study was to test the effectiveness of a comprehensive smoking cessation program for Korean adolescents.

Method

The study design was quasi-experimental with one pre and three post-tests. The three posttests were done immediately after, three months later, and six months after the completion of the program. A total of 43 high school students who smoked participated in the study with 22 in the experimental group and 21 in the control group. The smoking cessation program consisted of 9 sessions with content on enhancement of self-efficacy, stress management, correction of distorted thoughts, consciousness raising, and assertiveness training. The study variables were urine cotinine levels, self-efficacy, stress, and stages of changed behavior.

Results

Urine cotinine levels significantly decreased in the experimental group after the program (F=3.02, p=.06) but significantly increased in the control group (F=6.32, p=.004). Self-efficacy and the degree of stress did not change in either group. The stages of smoking cessation behavior tended to change when compared with raw data for the experimental group. For most participants, the stages of change had been precontemplation and contemplation, but changed to action and maintenance stage among the experimental group.

Conclusion

The program was effective in smoking cessation and influencing stages of change but did not change psychosocial factors such as self-efficacy and stress. It is suggested a program should be developed to change psychosocial variables on a long-term basis. It is also desirable to involve peers and families of adolescents who smoke when planning programs to enhance social support.

The Development and Effects of a Comprehensive Communication Course for Nursing Students

Purpose

The purposes of this study were to: (a) develop a comprehensive communication course combined with a group program for improving communication skills; and (b) examine the effects of the comprehensive communication course on interpersonal communication, relationship change, self-esteem, and depression in nursing students.

Method

The experimental group consisted of 82 nursing students, and the control group, 108 nursing students. Both groups each took communication courses from March to June, 2002 and 2003. A group program for improving communication skills was conducted for each 8 subgroups of the experimental group for 90 minutes once a week during the 6 weeks, while the existing communication lecture was conducted for the control group. Both groups were post-tested after the intervention for verifying the difference of variables between the two groups, and the experimental group was also pre-tested for verifying the difference between before and after the treatment.

Result & Conclusion

Interpersonal communication score of the post-test in the experimental group was significantly higher than in the control group and the depression score of the post-test in the experimental group was significantly lower than in the control group. Interpersonal communication, relationship change and self-esteem scores were significantly increased and the depression score was significantly decreased in experimental group after the treatment. In conclusion, the comprehensive communication course that was developed in this study had positive effects on communication skills in nursing students.

Characteristics and Mental Health of Battered Women in Shelters

Purpose

The purposes of this descriptive study were to: (a) describe the identifiable characteristics of the population of battered women; (b) examine mental health aspects by means of measuring level of anxiety, self-esteem, and depression.

Method

A convenience sample of 96 battered women was recruited from women who were admitted to a shelter.

Result & Conclusion

Data from this study supported the assertion that many battered women were exposed to severe physical abusive situations. Also the major finding was that the levels of anxiety and depression were high and the level of self-esteem was low in battered women living in shelters.

Effects of a Short-term Home-based Pulmonary Rehabilitation Program in Patients with Chronic Lung Disease

Purpose

The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients.

Method

Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study.

Result

Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group.

Conclusion

This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.

Mothers' Parenting Experience of Premature Infants: Q Methodological Approach

Purpose

This study was done to identify the parenting experience of mothers of premature infants in order to provide basic data for educational solutions and desirable directions.

Methods

Q-methodology was used as it provides a method of analyzing the subjectivity of each item. The participants were 33 mothers of premature infants who sorted 34 selected Q-statements which were then classified into the shape of a normal distribution using a 9-point scale. Subjectivity on parenting experience among the mothers was analyzed using the pc-QUANAL program.

Results

Four types of parenting experience were identified. Type I was named 'struggling', type II, 'self blame', type III, 'information collecting', and type IV, 'self-introspection'.

Conclusion

The results of this study indicate that different approaches to educational programs are needed for mothers of premature infants based on the four types of parenting experience.

Purpose

This study was done to examine the threshold value of estimated height loss at which the risk of osteoporosis increases and to verify its discriminative ability in the detection of osteoporosis.

Methods

It was conducted based on epidemiological descriptive methods on 732 Korean women at a public healthcare center in Seoul between July and November 2010. ANOVA, Pearson correlation, logistic regression analysis and receiver operating characteristics (ROC) curve were used for data analysis.

Results

There was an age-related correlation between bone mineral density (lumbar spine: F=37.88, p<.001; femur: F=54.27, p<.001) and estimated height loss (F=27.68, p<.001). Estimated height loss increased significantly with decreasing bone mineral density (lumbar spine: r=-.23, p<.001; femur: r=-.34, p<.001). The odds ratio for the point at which the estimated height loss affects the occurrence of osteoporosis was found to increase at a cut-off value of 2 cm and the area under ROC curve was .71 and .82 in lumbar spine and femur, respectively.

Conclusion

The optimal cut-off value of the estimated height loss for detection of osteoporosis was 2 cm. Height loss is therefore a useful indicator for the self-assessment and prognosis of osteoporosis.

Development of Key Indicators for Nurses Performance Evaluation and Estimation of Their Weights for Management by Objectives

Purpose

This methodological research was designed to develop performance evaluation key indicators (PEKIs) for management by objectives (MBO) and to estimate their weights for hospital nurses.

Methods

The PEKIs were developed by selecting preliminary indicators from a literature review, examining content validity and identifying their level of importance. Data were collected from November 14, 2007 to February 18, 2008. Data set for importance of indicators was obtained from 464 nurses and weights of PEKIs domain was from 453 nurses, who worked for at least 2 yr in one of three hospitals. Data were analyzed using χ2-test, factor analysis, and the Analytical Hierarchy Process.

Results

Based upon Content Validity Index of .8 or above, 61 indicators were selected from the 100 preliminary indicators. Finally, 40 PEKIs were developed from the 61 indicators, and categorized into 10 domains. The highest weight of the 10 domains was customer satisfaction, which was followed by patient education, direct nursing care, profit increase, safety management, improvement of nursing quality, completeness of nursing records, enhancing competence of nurses, indirect nursing care, and cost reduction, in that order.

Conclusion

PEKIs and their weights can be utilized for impartial evaluation and MBO for hospital nurses. Further research to verify PEKIs would lead to successful implementation of MBO.

The Effects of an Education Program for Safe Drug Use in the Rural Elderly

Purpose

This study was conducted to develop an education program for safe drug use in the rural elderly and to measure the effect of the program.

Method

This study utilized a nonequivalent control group pretest-posttest design. The subjects of this study consisted of 40 older persons who were more than 65 years old and lived in G and C meon, Y gun, Gyeongsang-bukdo and visited the public health subcenter. Twenty were assigned to the experimental group and 20 to the control group. The education was provided for one and a half hours, once a week for 3 weeks. Data was collected before, right after, and one month after the program.

Result

The first hypothesis was supported(F=79.24, p=0.000) showing that the knowledge scores of the drug use of the experimental group were significantly higher than those of the control group at post education and one month after education. The second hypothesis was supported(F=23.84, p=0.000) showing that the drug misuse and abuse prevention behavior scores in the experimental group were significantly higher than those of the control group at post education and one month after the education.

Conclusion

This study suggests that the education for safe drug use is effective in promoting knowledge and behavior for safe drug use of the rural elderly.
