Search for the Meaning of Social Support in Korean Society

In Korea the concept of social support was first used as a research concept in nursing and has not had much application in the clinical field. Another problem is that research on social support has used a direct translation of the words "social support" into Korean as "sawhejukjiji". Three questions were posed to direct the research. 1) Is there a concept of social support in Korean society? 2) if so, what words or expressions are used to describe it? 3) further, if so, how is social support structured and how does it function? In order to answer the research questions a three-step research methodology was used: The first step consisted of a literature review on research related to social support and on information on the background of, and the way of thinking related to interpersonal relations among Korean people. The second step, which was done to identify whether there is a concept of social support in korean society, involved interviewing a sample of the population. The third step involved a panel discussion that included the members of the research team and three consultants, a sociologist, a philosopher and a scholor in korean literature. A review of the literature on interpersonal relationships in traditional korean society identified a four cirole structure that explains interpersonal relationships. The first circle with "me" at the center is the family but here "me" disappears into the "we" that is essential for a cooperative agricultural society. In the second circle are those close to "me" but outside the family. The third circle includes those with whom "I" have infrequent but regular contact and with whom correct conduct is important. The last circle is all the people with whom "I" have nothing in common. They are excluded in interpersonal relationships. The literature on interpersonal relationships showed that within the traditional Korean society people lived in villages where most people were very familiar with each other. "Yun", the social network established the connection and "Jung", the feeling of affection increased with time as the connection was strengthened. In the traditional village psychological support was provided through "Mallaniki", "Pumashi" and "Kae" with the latter two also providing material support. In modern Korea there are more informal and formal social networks, like social services and community activities on the formal level and cultural and leisure groups along with "kae's on the informal level. But even with this modern variety of groups, most social support comes from informal networks that resemble the traditional "Pumashi", "Kai" and "Mallaniki". The six member research team interviewed 65 people in ordsr to identify whether there is a concept of social support and then analysed their responses. There were 20 different words describing the reception of the social support and these could be grouped into seven major categories: virtuous, fortunate, helped, supported, blessed, attached(re-ceiving affection) and receiving (grace) benevolence, there were 27 words describing the act of social support which could be categorized into seven major categories: love, looking after, affection(at-tachment), kindness (goodness), faith, psychological help and material help, for the meaning of social support translated as "sawhe juk jiji" there were a total of 14 different answers which could be categorized into 3 major categories: help, agreement, and faith. In third step, the results of the literature review and the answers to the questions were discussed in a pannel. The results of the discussion led to the following definition of social support in Korea which is shaped like a the four sided pyramid on a base. Social support is the apex of the pyramid and four sides are made up of: "do-oom"(both emotional and material help), "jung"(connectedness, or relationship bound by affection, regard or shared common experience), "midum"(faith or belief in), "eunhae" (kindness or benevolence). The research team identified "Yun"(the basic network of relationships) as the base of the pyramid and as such the foundation for the components of social support in Korean culture. On "Yun" rest the other four components of social support: "Jung", "Midum", "Do-oom", and "Eunhae", For social support to take place there must be "Yun". This is an important factor in social support. In private social network "Jung" is an essential facotr in social support. But not in the public social network. "Yun" is a condition for "Jung" and "Jung" is the manifestation of support.

Social Support, Stressful Life Events, and Health Behaviors of Korean Undergraduate Students

PURPOSE: This cross-sectional study was designed to explore the relationship among social support, experienced stressful life events and health behaviors of Korean undergraduate students, and validate the mediator effect of social support. METHOD: One thousand four hundred fifty-three undergraduate students were randomly selected from five universities located in the middle area of Korea. RESULT: The health behaviors of Korean undergraduates tend to have unhealthy patterns. In the case of the students living without family, experiencing more stressful life events and perceiving lower social support, health behaviors are poor. The relationship between perceived social supports, the frequency of the experienced stressful life events and the score of health behavior patterns is statistically significant. After controlling the effect of social support, the correlation coefficient between the frequency of experienced stressful life events and the score of health behavior patterns was slightly lower. The score of health behaviors between the group with an extremely high score of social support and the group with an extremely low score were statistically significantly different. CONCLUSION: Future studies need to be pursued to develop various strategies such as a health education programs and counseling programs for health maintenance and health promotion of undergraduates.

The Trends of Nursing Research in the Journals of Seven Branches of the Korean Academy of Nursing

This study was designed to analyze the research methodology and the key concepts used in articles published in each nursing journal of seven branches of the Korean Academy of Nursing. The purpose of this study was for reflecting the trends of nursing research and suggesting the direction of future nursing research in Korea. One thousand three hundred seventy two articles published in seven nursing journals from the beginning year of 2000 were analyzed. The prevailing research designs for these journals were the non-experimental design ranging from 54.3% to 75.7%, the experimental design ranging from 6.2% to 30.4%, and qualitative research design ranging from 3.7% to 10.6%. Research subjects were 10.0% to 46.0% for clients with health problems, 2.1% to 42.4% for generally healthy persons, 1.4% to 43.9% for primary care-givers, 7.0% to 53.5% for nurses or nursing students, and 3.1% to 7.3% for health organizations or nursing organizations. The data collection method used most often self-report questionnaires using psychosocial measures. Interviewing methods and physiologic measures were used relatively few times. The domains of the key concepts that prevailed was personal domain and health domain. This study has the limitation of focusing on only the superficial structural analysis rather than in-depth content analysis of each article. However, this study is the first study for reflecting the trends of nursing research based on each journal of seven branches of the Korean Academy of Nursing.

Identification of Attributes Constituiting Korean Social Support: Based on Middle Aged Women

This study was conducted to identify the attributes constituting Korean social support and to validate the results of the previous study 'Search for the meaning of social support in Korean Society.' The informants were 41 Korean middle-aged women in three cities : Seoul, Dagjun and Daeku. The data were collected through indepth interviews using the interview guide from Jun. 1994 to Jun. 1995. The interview guide was developed in the simulated situations of 'Stroke attack' which expected to be in need of social support. The women were asked to answer what they felt and the appropriated terms representing the situations. Data analysis were conducted by content analysis. consequently, the Koran social support pyramid was modified as follows ; Support is the apex of the pyramid. The four sides of the pyramid are made up of 'Jung'(Bound by ties of affection, regard or shared common experience, Connectedness), Do-oom(both emotional and material help), Mi-dum(Faith or belief in) and Sa-Rang(Love). The base of the pyramid is 'yun'(the basic network of relationships in Korean culture) that connote the meaning of Eunhae(Benevolence), Euimu(duty, responsibility) and Dori(obligation).

For producing large numbers of professional nurses who could manage 21th century's human health, it is necessary to review the direction of registered nursed' national examination which evaluates the nursing education and is granted a licence. For adapting to social expectation of the nurse, we have to nurture the nurses' problem solving capability in clinical setting. Seven divisions of Korean Academy of Nursing suggested clinical competency according to their categories. This paper was presented in the workshop for setting up direction of registered nurses' national examination. We expect that this paper would be more refine and confirm through reviewing subdivisions' learning objectives and discussing clinical minimum level of competence contents with clinical leaders.

Development and Evaluation of a PBL-based Continuing Education for Clinical Nurses: A Pilot Study

Purpose

The purposes of this study were to develop a PBL program for continuing nurse education and to evaluate the program after its implementation.

Methods

The PBL program was developed in the core cardio-pulmonary nursing concepts through a collaborative approach with a nursing school and a hospital. The PBL packages with simulation on ACLS were implemented to 40 clinical nurses. The entire PBL program consisted of six 3-hour weekly classes and was evaluated by the participants' subjective responses.

Results

Two PBL packages in cardio-pulmonary system including clinical cases and tutorial guidelines were developed. The 57.5% of the participants responded positively about the use of PBL as continuing nurse education in terms of self-motivated and cooperative learning, whereas 20.0% of the participants answered that the PBL method was not suitable for clinical nurses. Some modifications were suggested in grouping participants and program contents for PBL.

Conclusion

The PBL method could be utilized to promote nurses' clinical competencies as well as self-learning abilities. Further research is needed in the implementation strategies of PBL-based continuing education in order to improve its effectiveness.

Structural Equation Model for the Health Behaviors of University Students in Korea

Purpose

A structural equation model was analysed to explore the determinants of health behaviors of university students in Korea.

Metnod

Nine hundred sixty nine university students were selected by random cluster sampling from five universities located in the central area of Korea.

Data Collection

The data was collected by questionnaires about demographic characteristics, stressful life events, perceived social support, perceived health status and health behaviors.
RESULTS
1. Gender showed indirect effect on health behaviors. 2. Living together with(out) family had a direct effecton health behaviors: students living with family showed more positive health behaviors. 3. Stressful life events had an indirect effect on health behaviors via perceived health status;a higher score of stressful life events was the predictor for negative health behaviors. 4. A higher score of perceived health status predicted positive health behaviors.

Recommendation

Each university should be encouraged to develop a health behavior control program and health promotion program for their own university students. It would be more effective to develop health programs separately according to the demographic or social characteristics of the students. It is also necessary for the Ministry of Education to reform the School Health Act and school health policy to strengthen a health promotion program for university students. In conclusion, following studies should identify and promote the validity and reliability of perceived health status and health behaviors measurements.

Health Problems and Health Behaviors of Preschoolers

Purpose

This study was conducted to understand the health status and health behaviors of preschoolers to provide baseline data for developing health promotion programs.

Method

Parents of children attending day-care centers were recruited for the study. Participating day-care centers were selected using a stratified sampling method. Data was collected from June to August 2002 using a questionnaire.

Result

Among 754 preschoolers, 17.3% were overweight, while 18.2% were underweight. The most frequent infectious diseases that children have had previously were hand-foot-mouth disease (20.2%) and chicken pox (18.7%). Current health conditions that children have frequently are respiratory disease (28%) and atopic dermatitis (23.8%). Only 61% brush their teeth everyday at bedtime, 54.3% wash their hands every time after returning home, 8.8% wear bicycle helmets, 9.3% use a child car seat, and 8.1% eat fruits and vegetables five times a day. Children residing in the metropolitan area were more likely to have positive health behaviors, and children of parents with an advanced college level education were more likely to have positive health behaviors than those with only a high school level education.

Conclusion

Based on the study results, health professionals could plan and develop health promotion programs to change unhealthy behaviors of preschoolers targeting high-risk groups.

Evaluation of a Community-Based Program for Breast Self-Examination Offered by the Community Health Nurse Practitioners in Korea

Background

Breast cancer is the most common form of cancer among Korean women. Only 14% of urban women and 10% of rural women in Korea, however, participated in breast cancer screening behavior in 1998 (Korean Ministry of Health & Welfare, 1999).

Purpose

The aim of this study was to evaluate the effect of community-based breast self-examination (BSE) education programs in Korea.

Methods

First, breast cancer risk appraisals were done with 1,977 rural women. Of the 1,977 women, nearly 30% (n= 494) had a higher or equal to borderline risk of developing breast cancer. This quasi-experimental study was conducted to target these women with a high or equal to borderline risk of breast cancer. The risk appraisal feedback and breast self-examination education were used as an intervention for breast cancer prevention and early detection.

Results

After a 3-month follow-up, 30.5% of the women in the intervention group performed regular BSE compared to 10.2% of women in the control group. The mean knowledge score related to breast cancer and BSE was significantly higher for the women in the intervention group than that in the control group.

Effects of Non-nutritive Sucking on the Physiological and Behavioral States of Pre-term Infants during Tube Feeding

Purpose

This study was to investigate the effects of non-nutritive sucking on physiological and behavioral state of pre-term infants during tube feeding.

Method

This nonequivalent, non-synchronized experimental study included 50 pre-term infants. An attempt was made to match gestational age and birth weight of infants in each group. Infants in the experimental group were given a pacifier 2 minutes before, during, and for 2 minutes after tube feeding. Infants in the control group did not get a pacifier. Both groups were tested at three stages for changes in the physiologic state and behavioral state-2 minutes before, during, and 2 minutes after feeding. Date was analyzed with SPSS WIN 10.0 using an χ2-test, t-test, and repeated measures ANOVA.

Result

Heart rates and oxygen saturation levels of the two groups were significantly different(P=.001, P=.000). The behavioral states of the two groups were significantly different during and post feeding(P=.000, P=.000).

Conclusion

This result suggests non-nutritive sucking by using a pacifier is an effective intervention for pre-term infants during tube feeding.

Health Behaviors between a Health Promotion Demonstration Health Center and a General Health Center

Purpose

The purpose of the study was to compare community residents' perceptions, participation, satisfaction, and behavioral changes between a health promotion demonstration health center and general health center.

Method

The design of the study was ex-post facto that compared community residents in demonstration health centers and general health centers. The sample included 2,261 community residents who were conveniently selected from demonstration (792 participants) and general health centers (1,496 participants).

Result

The results of the study were as follows: 1) Perception and participation rates of exercise, nutrition, and hypertension management programs were significantly higher in the participants of demonstration health centers than those of general health centers.; 2) Satisfaction rates of all programs except the smoking cessation program were significantly higher in the participants of demonstration health centers than those of general health centers. However, only the exercise rate among risk behaviors of participants was significantly higher in demonstration health centers than general health centers.

Conclusion

Systematic efforts for health promotion were effective not only in improving the community's awareness, participation, and satisfaction of the program, but also in changing health behaviors. This evidence should be used to foster and disseminate health promotion programs toother health centers to improve community residents' health status and quality of life.
