Structural Equation Model for the Health Behaviors of University Students in Korea

Purpose

A structural equation model was analysed to explore the determinants of health behaviors of university students in Korea.

Metnod

Nine hundred sixty nine university students were selected by random cluster sampling from five universities located in the central area of Korea.

Data Collection

The data was collected by questionnaires about demographic characteristics, stressful life events, perceived social support, perceived health status and health behaviors.
RESULTS
1. Gender showed indirect effect on health behaviors. 2. Living together with(out) family had a direct effecton health behaviors: students living with family showed more positive health behaviors. 3. Stressful life events had an indirect effect on health behaviors via perceived health status;a higher score of stressful life events was the predictor for negative health behaviors. 4. A higher score of perceived health status predicted positive health behaviors.

Recommendation

Each university should be encouraged to develop a health behavior control program and health promotion program for their own university students. It would be more effective to develop health programs separately according to the demographic or social characteristics of the students. It is also necessary for the Ministry of Education to reform the School Health Act and school health policy to strengthen a health promotion program for university students. In conclusion, following studies should identify and promote the validity and reliability of perceived health status and health behaviors measurements.

Health as Expanding Consciousness: Based on the Experiences of Victims of Sexual Violence

Purpose

This study was to explore health experiences of victims of sexual violence reflected in Newman's health as expanding consciousness theory.

Method

Nurse as researcher use Newman's praxis methodology because it is good for showing the process of interaction between researcher and sexual abuse victims.

Results

The significant characteristics of early health experiences during negative situations were a lot of suffering. However, after a turning point in health experience, the health experiences of most of the participants evolved as expanding consciousness.

Conclusion

The study has provided support for Newman's theory of health. Most of the participants recognised meanings in their patterns and trustful caring relationships with the nurse as researcher. Pattern recognition as a nursing practice was a meaningful transforming process in the participant-nurse partnership.

Effects of Hand Massage and Hand Holding on the Anxiety in Patients with Local Infiltration Anesthesia

Purpose

This study was to examine the effects of hand massage and hand holding as nursing interventions on the anxiety in patients with local infiltration anesthesia.

Method

The design of this study was a nonequivalent, control group, non- synchronized design. The subjects of this study consisted of 15 patients for the hand group, 15 patients for the hand holding group and 17 patients for the control group awaiting surgery in the operation room of a general hospitalin Daegu. As an experimental treatment, hand massage was carried out by the Hand Massage Protocol developed by Snyder(1995) and interpreted by Cho(1998) and hand holding developed by Cho(1998). The data were analyzed by SPSS/WIN, T-test, ANOVA, Cronbach's α, and the Scheffe test.

Results

The hand massage group and hand holding group were more effective than the control group in reducing anxiety, VAS score, systolic blood pressure and pulse rate.

Conclusion

Hand massage and hand holding are effective nursing interventions that alleviates the psychological and physiological anxiety of patients with local infiltration anesthesia. In particular, the simple contact of hand holding is regarded as an effective and easily accessible nursing intervention in the operating room.

A Meta-Analysis of the Effects of a Self-Efficacy Promoting Program

Purpose

This meta-analysis was conducted to evaluate the effects of a self-efficacy promoting program and analyze its components.

Method

The material used for this study were 18 self-efficacy promoting program studies carried out from Jan. 1980 to Oct. 2003. The studies were analyzed in different categories: 1) types of dependent variables 2) sample characteristics 3) types of experimental treatment conditions 4) types of self-efficacy source and 5) total amount of time.

Result

1) The weighted mean of a self-efficacy promoting program ranged from 1.383 to 0.015 2) for the experimental treatment condition, exercise had a much larger effect in increasing general self-efficacy and self-care than education 3) the studies using 3 sources had a much larger effect in increasing self-care than the studies using 4 sources 4) a time period longer than 900 minutes had a much larger effect in increasing specific self-efficacy, general self-efficacy and self-care than in a time period shorter than 900 minutes. 5) effect size of specific self-efficacy was significantly higher than general self-efficacy.

Conclusion

These results can be used to guide the development of a self-efficacy promoting program for nursing practice.

The Experiences of Perioperative Patients with Cancer

Purpose

This study was to explore the experiences of perioperative patients with cancer. The purpose of this inquiry was to describe the essence of such experiences, and to understand them from the patients'point of view.

Method

Participants in this study were 9 cancer patients with ages ranging from 31 to 70 years old, living in B city, who experienced cancer operations. Data collection consisted of in-depth interviews and an observation method done from October to December in 2003. In analysing data, Colaizzi's(1978) phenomenological research method was adopted.

Results

Five categories emerged from ten theme clusters. The five categories were: ‘ shock’, ‘ expectations & wishes’, ‘ despair’, ‘ feelings of burden’, ‘ continual pain’.

Conclusion

The results of this study showthat oncology nurses need to pay more attention to the psychosocial aspect of nursing to empathize and support the cancer patients who suffer and its treatment, and operations.

A Study on the Variables Forecasting Male Adolescents' Sexual Intercourse

Purpose

This study was designed to identify the variables affecting male adolescents' sexual intercourse through a comprehensive analysis of individual and environmental factors.

Method

The subjects of this descriptive survey on causal relations were 462 subjects enrolled in liberal and vocational high schools selected on a convenience sampling basis. The data collected from May-July 2002 was put to logistic regression analysis to build a forecast model. Findings: 1) Individual factors such as school record, experience seeking, non-inhibition and sexual permissiveness, 2) family factors such as parental living arrangement, 3) school factorssuch as career tract and 4) peer factors such as having a boy/girl friend were identified as significant variables forecasting sexual intercourse.

Conclusion and Recommendation

The theoretical model built on the basis of the major findings of this study will hopefully help promote a wholesome youth culture related to sexual intercourse.It is recommended that a program be developed that can help control the variables identified in this study along with a follow-up study to verify the model.

A Study on Influencing Factors in Health Promoting Behaviors of Women Workers at Small-scale Industries

Purpose

This study was aimed to identify the major factors affecting performance in health promoting behaviors in women workers at small-scale industries.

Method

This study was based on the Pender's Health Promotion Model. The subjects for this study were 251 women workers at 23 small-scale industries in Busan city. The data for this study was collected from July 15th to August 15th 2003 by structured questionaries, and were analyzed with ANOVA, t-test, Pearson' correlation coefficient, and multiple Regression in the SPSS/WIN 10.0.

Result

The mean performance of the health promoting behavior was 2.56. The factors related to the performance of the health promoting behaviors were social support, marital status, status of owning a house, perceived barriers to action, working time, and self-efficacy, and they explained 58.4% of the variance of the health promoting behaviors.

Conclusion

The mean performance of the health promoting behavior seemed to be low, and the most important variable related to health promoting behaviorsof women working at a small-scale industry was social support. Therefore, intervention programs to increase the social support for women worker need to be developed.

The Effect of Senior Simulation on Nurses' Attitude Toward the Elderly

Purpose

This study was performed to explore the effect of senior simulation on nurses' attitudes toward the elderly.

Method

Twenty-seven nurses working in various settings such as acute hospitals, community health centers, geriatric hospitals, and clinics were recruited. Among them, 25 subjects completed the whole experimental protocol. Aging Semantic Differential Scaling was utilized to evaluate attitudes toward the elderly and ‘ Suit for Experiencing Being Aged’ from the Sakamoto Model was provided for the experiment. Before and after the experiment subjects filled out questionnaires.

Result

Attitude score before experiment was 4.36, which indicates neutral attitude. Objective attitude scores were not different significantly after experiment. However subjective statements indicated attitude changes in a positive way.

Conclusion

Senior simulation can affect nurses' attitude toward elderly in subjective way. That is, nurses became more empathetic and understanding to elderly's physical limitations and felt more initiative nursing approach were needed in caring elderly.

Estimating the Cost of Visiting Nursing Service by Visiting Nursing Model for Urban Public Health Center in Korea

Purpose

This study focused on analysing costs per visiting nursing care based on nursing activities in a public health center.

Method

The Easley-Storfjell Instrument(1997) was used for a prospective descriptive analysis of self-records for workload data from 10 visiting nurses during 4 weeks on all nursing activities. In addition, analysis of the 478 visiting nursing records and cost data from 5 home visiting departments in public health centers during one year of 2003 was done.

Result

The workload of visiting nurses by the type of model was identified as follows: Type I showed that caseloads made up 32.9 % of all nurse activities, and type II showed that the caseloads made up 45.8 %. Second, The cost per visit in type I was 33,088 won and 31,323 won in type II. Third, the estimated budgets were 1,902,436 won to 12,057,696 won for the type I model. and 4,151,316 won to 17,432,712 won for the type II model for one year.

Conclusion

This study's results will contribute to baseline data used to establish on infrastructure for visiting nursing program and visiting nursing agencies based on the budget of visiting nursing services.

Development of Web-based Multimedia Content for a Physical Examination and Health Assessment Course

Purpose

This study was to develop Web-based multimedia content for Physical Examination and Health Assesment.

Method

The multimedia content was developed based on Jung's teaching and learning structure plan model, using the following 5 processes : 1) Analysis Stage, 2) Planning Stage, 3) Storyboard Framing and Production Stage, 4) Program Operation Stage, and 5) Final Evaluation Stage.

Results

The web based multimedia content consisted of an intro movie, main page and sub pages. On the main page, there were 6 menu bars that consisted of Announcement center, Information of professors, Lecture guide, Cyber lecture, Q&A, and Data centers, and a site map which introduced 15 week lectures. In the operation of web based multimedia content, HTML, JavaScript, Flash, and multimedia technology(Audio and Video) were utilized and the content consisted of text content, interactive content, animation, and audio & video. Consultation with the experts in context, computer engineering, and educational technology was utilized in the development of these processes.

Conclusions

Web-based multimedia content is expected to offer individualized and tailored learning opportunities to maximize and facilitate the effectiveness of the teaching and learning process. Therefore, multimedia content should be utilized concurrently with the lecture in the Physical Examination and Health Assesment classes as a vital teaching aid to make up for the weakness of the face-to-face teaching-learning method.

A Meta-analysis of the Effects of Smoking Prevention Programs in Korea

Purpose

The purpose of this paper was to describe the characteristics of smoking prevention programs in Korea, to estimate overall effect size of Korean smoking prevention programs, and to investigate effect size variations by program modality and instruction method.

Method

Meta-analysis was performed on21 programs in 20 studies.

Result

The estimation of overall effect size for knowledge and attitude was not possible because effect sizes were not homogeneous in this meta-analysis. However, effect sizes of studies that were socially influential programs or active/interactive methods were larger than information-oriented programs or passive/non-interactive methods in the pictures. The effects for behavioral outcomes were generally not as positive and not statistically significant. Q statistics showed that variations among effect sizes within program modality and instruction method classifications were heterogeneous.

Conclusion

The results from this meta-analysis support the continued use of socially influential programs and active/interactive methods for smoking prevention programs. Because behavioral effect might be the fundamental objective of smoking prevention programs, the present results indicate that smoking prevention programs should consider adopting more effective programs.

Development of a Nutritional Assessment Tool for Home Care Centers

Purpose

The purpose of this study was to develop a tool for nutritional assessment, so that home care nurses can make early assessment of patients' nutritional status.

Method

The study was done in two partsfirst a tool was developed to assess nutrition, and second the content validity and clinical validity of the categories and indices were verified.

Result

The results of this study are summarized in two ways. First,the nutritional assessment tool was classified into 3 areas, physical measurement, nutritional survey and clinical survey, and into 11 categories with 22 indices. Second, when 5 of the 11 categories were positive, nutritional status was considered to be poor.

Conclusion

By developing a clinically useful nutritional assessment tool for patients receiving home care, which was developed in this study, the quality of life for these patients will improve and contribution to the development of a more effective clinical home nursing practice will occur.

Working with Vulnerable Families: A Nurse Home Visiting Perspective

Purpose

This study defines a vulnerable group in a community that has become the main target of a national health project also, it is descriptive research to suggest an evidence-based direction to meet their deficit health-related needs.

Method

This research examined 833 families and 1,835 family members of the financially vulnerable class that was registered in a home visiting program of a public health center. Among them, 892 persons who had health problems, and their family members were examined in detail to find out their characteristics of vulnerability and health needs by assessment during a nurses home visit. Frequency distribution, stepwise-regression and factor analysis were used to analyze the data.

Result

The vulnerable group that was defined with social indexes set as standards, involved substantial characteristics of vulnerability. The characteristics of demand showed tendencies of being clustered in 5 factors needs of intensive nursing care, chronic nursing care problems and helplessness, maintenance of family functioning with a disability, deficient problem solving ability, and simple financial fragility.

Conclusion

Categorization of needs is an evidence-based estimator of workload in nurse home visiting services, and can be used as a basic resource for direction to meet the deficit needs of a vulnerable group.

Purpose

This one group experimentation was designed to develop a program for relieving ‘ Hwa-Byung’(HB) symptoms and examine its effects on HB symptoms, pain threshold emotions like anger, anxiety and depression.

Method

The program consisted of three components the change of the cognitive thoughts, the formation of a supportive network, and induction of mind-body relaxation. Sixteen middle-aged women with HB were divided into three groups for group dynamics according to the time of the recruitment. Data was collected for nine months at three time points, before, immediately after, and one month later of its application.

Result

There were statistically significant differences in the severity level of state anger, state anxiety, depression, and HB symptoms according to the time interval. The means of state anger and state anxiety were reduced after the intervention, but it was slightly increased one month later. The means of depression and HB symptoms were continuously reduced after the intervention and one month later.

Conclusion

This was the first management program for HB women in Korea. A future study must be done with the research design formethodological strength revision of the program.

Purpose

The purpose of the study was to research how elder abuse affected the abuse results (self-esteem, depression, somatization) through coping strategies.

Method

A convenient sample consisted of 499 subjects from 65 to 85 years old. Data was collected using structured questionnaires from July to August, 2002. Results

The pathway of elder abuse affecting elder's self-esteem was researched by passive coping, emotional abuse, financial abuse, and physical abuse as the variables that directly affected self-esteem. The pathway of elder abuse affecting elder's depression was researched by passive coping, aggressive coping, verbal abuse, and neglect as the variable that directly affected depression. The pathway of elder abuse affecting somatization was researched by neglect, emotional abuse, and aggressive coping as the variables that directly affected somatization.

Conclusion

We should develop a systematized intervention program so that elders can learn positive coping in their abuse circumstance.

Health Pattern of Elderly According to Age Group Who Living Alone in an Urban Area

Purpose

This study was conducted to identify the differences of health patterns; health-related characteristics, health behaviors, health problems, and self-care levels of elderly, living alone in an urban area according to age group.

Method

The study design was a descriptive survey and the subjects were 1,800 elderly consisting of 937 young old, 704 middle old, and 159 old old. Data was collected from May to July 2003 using the structured questionnaire and analyzed by descriptive statistics, χ2-test, Fisher's exact probability test, ANOVA and Scheffe test using SPSS/PC.

Result

The health related characteristics which involved body mass index, mental status, use of assistant devices, perceived health, concern of health, and social support were different among the groups and more aggravated by aging. Health behaviors and health problems were also significantly aggravated in the old old group. The self-care levels measured by ADL, physical activity, and self-care ability scores were also significantly decreased in the old old group.

Conclusion

For the level of health status, health behaviors and self-care ability were significantly decreased by aging, health care services for the elderly should be planned considering the difference in the health pattern by age group.

Developing and Testing the Effects of a Psychosocial Intervention on Stress Response and Coping in Korean Breast Cancer Survivors: A Pilot Study

Purpose

The purpose of this study was to develop a socioculturally-appropriate psychosocial intervention program for Korean patients with breast cancer and test its effects on stress, anxiety, depression, and coping strategies.

Methods

One group pretest and posttest design was used to test the effects of the intervention. A post-intervention interview was conducted to refine the nature of the intervention. A convenience sample of 10 breast cancer survivors was recruited from the outpatients clinics. Psychosocial intervention was developed to provide the health education, stress management, coping skill training and support weekly(90min) for 6 weeks.

Results

There was a significant decrease in stress scores following the intervention(Z= -2.388, p=0.017). However, no significant changes were noted in the use of problem-focused and emotion-focused coping strategies, nor in the changes of anxiety and depression levels. Content analysis of interview data revealed six clusters; changes in perception, changes in problem solving approaches, changes in anger management, changes in life pattern, social support and reduction of perceived stress.

Conclusion

Based on quantitative and qualitative data, we recommend the refinements of the intervention in the following areas for future studies: 1) duration, activities, and progression of psychosocial intervention; 2) research design and sample size; and 3) measurements.

Purpose

This study was done to identify the risk factors associated with falls among patients with Parkinson's Disease(PD).

Method

A retrospective study design was used through the collection of physiological and physical health, and psychosocial functions.

Results

Of the 100 participants, fifty-nine(59%) reported one or more falls and seventy-one(71%) reported one or more near-falls. Anaverage 34.7 falls and 150.3 near-falls were reported in the previous year per person. Stage of PD, foot problems, balance, fear of falling, and activities of daily living were significantly associated with an increased risk of falls.

Conclusion

The findings confirm the high risk of falling in PD patients. Also these results have implications for developing fall prevention programs for PD patients.

Purpose

A dance exercise program was performed to investigate the effects of exercise on exercise intention and exercise-related affect as these are important determinants of exercise behavior.

Method

The subjects were overweight (23≤or=BMI≤or=25) or obese (BMI≥or=25) postmenopausal women (n=14), who participated in an exercise program from May, 2003 through November, 2003 at one public health center located in Kyong-gi Province. Data was analyzed with an SAS PC program.

Result

Exercise intention was significantly increased after the exercise program (t=-2.24, P=0.04). Exercise-related affect was also increased, but there was no statistically significant change (t=1.81, P=0.09).

Conclusion

This study suggests that exercise participation can increase the level of future exercise behavior by increasing exercise intention and exercise-related affect.

Scale Development of Job Stress for Home Care Nurses

The purpose of this study was to develop a job stress scale for hospital-based home care nurses in Korea. The process was construction of the conceptual framework, development of the preliminary items, verification of the content validity, item analysis and test of the reliability.

The preliminary items were based on literature review and in-depth interviews with home care nurses. As a result, eight categories and sixty items were selected. These were reviewed by seven specialists for content validity and finally fifty one items were chosen. Data was collected from 180 home care nurses who were engaged in 87 hospitals from August to September 2003.

The result of item analysis one was excepted. The final item count was 50. Categories were as follows: overload work(8 items), lack of specialized knowledge and technique(5 items), ethical dilemma(4 items), role conflict(5 items), interpersonal relationships(6 items), visiting home environment(9 items), driving conditions(4 items) and lack of administrative support(9 items), The reliability of the scale by Cronbach's alpha was .948 and the domain's reliability ranged from .649 to .841.

The result of this study could be used to measure the job stress of home care nurses. However, for further validity and reliability, repeated studies will be necessary.

Purpose

This research was conducted to provide basic information about the effects of aerobic exercise on physiological change in middle-aged obese women according to differences of β3-adrenergic receptor polymorphisms.

Method

Twenty-nine middle aged obese women with over 30%BMI were divided into three groups according to β3-adrenergic receptor gene polymorphism[Variable Group(VG):9, Normal Group(NG):10, Control Group(CG):10]. The VG and NG groups performed walking at 50% exercise intensity for 30 minutes a day, 4 days a week, for 12 weeks. The data was analyzed using the SPSS program.

Result

The level of leptin, insulin and % body fat in the VG and NG groups was significantly lower than those of the CG after 12 weeks. In addition, the level of HDL-C in the VG and NG was significantly higher than that of the CG after 12 weeks. However, TC, TG and body weight between groups didn't appear significant at the end of 12 weeks.

Conclusions

Aerobic exercise didn't cause differences in persons with differing β3-adrenergic receptor gene polymorphisms, but aerobic exercise affected the physiological change in middle-aged obese women. The findings suggest that aerobic exercise is a desirable nursing intervention for obesity control in middle-aged obese women.

Purpose

This study was carried out to substantiate the application process of activity-based costing on the current cost of hospital home care (HHC) service. The study materials were documents, 120 client charts, health insurance demand bills, salary of 215 HHC nurses, operating expense, 6 HHC agencies, and 31 HHC nurses.

Method

The research was carried out by analyzing the HHC activities and then collecting labor and operating expenses. For resource drivers, HHC activity performance time and workload were studied. For activity drivers, the number of HHC activity performances and the activity number of visits were studied.

Result

The HHC activities were classified into 70 activities. In resource, the labor cost was 245\per minute, operating cost was 9,570\ per visit and traffic expense was an average of 12,750\. In resource drivers, education and training had the longest time of 67 minutes. Average length of performance for activities was 13.7 minutes. The workload was applied as a relative value. The average cost of HHC was 62,741\ and the cost ranged from 55,560\ to 74,016\.

Conclusion

The fixed base rate for a visit in the current HHC medical fee should be increased. Exclusion from the current fee structure or flexible operation of traveling expenses should be reviewed.

The Effects of the Systemic Follow up Health Care Program on the Health Promotion and the Risk Reduction in Premature Infants and Their Mothers

Purpose

This research was conducted to evaluate the effects of asystemic follow-up care program on health promotion and risk reduction in 64 high-risk infants(HRI) including premature infants and their mothers.

Method

The intervention consisted of systemic NICU education, tele-counseling and 3 home visits in 6 months. The subjects were divided into either the intervention group or the control group receiving the conventional education without the tele-counseling and home visiting. Infant health promotion was measured using physical assessment, types of health problems, reflexes, OPD visiting history, DDST, immunization, feeding assessment, Infant death rate, etc. Maternal self-esteem, postpartum depression and family function were measured using the maternal self-report inventory(MRI), EPDS, and family apgar score(Fapgar), retrospectively.

Result

All premature infants in the intervention group were in the normal range of growth and development, and the regular vaccination schedule. The health problems in the intervention group were addressed early so not to develop into adverse effects. The follow-up program for 6 months showed beneficial effects on MRI, EPDS, and Fapgar.

Conclusion

A systemic follow-up health care program is beneficial on health promotion and risk reduction in 64 HRI including premature infants and their mothers.
