Effects of a Drug Misuse and Abuse Prevention Program on Knowledge, Attitude, and Preventive Behaviors Related to Drug Misuse and Abuse, and Depression in Low-income Elderly Women

Purpose

This study was conducted to determine the effects of a drug misuse and abuse prevention programon knowledge, attitude, and preventive behaviorsrelated to drug misuse and abuse, and depression in low-income elderly women in the urban area.

Method

The design of this study was a nonequivalent control group pretest-posttest design. The subjects consisted of 26 in the experimental group and 23 in the control group. The program was performed for about 1hour, once a week for 5 weeks. Data was analyzed by the SAS(ver.8.02) computer program, and it included descriptive statistics, Fisher's exact test, Mann-Whitney U test, Wilcoxon signed ranks test, and ANCOVA.

Result

There were statistically significant differences in knowledge, attitude, and preventive behaviors related to drug misuse and abuse, and depression between the experimental group and the control group.

Conclusion

This study showed that this prevention program of drug misuse and abuse is appropriate for low-income elderly women, Therefore this program is recommended as a nursing intervention strategy for the elderly.

Home Health Resource Utilization Measures Using a Case-Mix Adjustor Model

Purpose

The purpose of this study was to measure home health resource utilization using a Case-Mix Adjustor Model developed in the U.S.

Method

The subjects of this study were 484 patients who had received home health care more than 4 visits during a 60-day episode at 31 home health care institutions. Data on the 484 patients had to be merged onto a 60-day payment segment. Based on the results, the researcher classified home health resource groups (HHRG).

Result

The subjects were classified into 34 HHRGs in Korea. Home health resource utilization according to clinical severity was in order of Minimum (C0) < ‘ Low (C1) < ‘ Moderate (C2) < ‘ High (C3), according to dependency in daily activities was in order of Minimum (F0) < ‘ High (F3) < ”Medium (F2) < ”Low (F1) < ”Maximum (F4). Resource utilization by HHRGs was the highest 564,735 won in group C0F0S2 (clinical severity minimum, dependency in daily activity minimum, service utilization moderate), and the lowest 97,000 won in group C2F3S1, so the former was 5.82 times higher than the latter.

Conclusion

Resource utilization in home health care has become an issue of concern due to rising costs for home health care. The results suggest the need for more analytical attention on the utilization and expenditures for home care using a Case-Mix Adjustor Model.

Body Weight, Cardiovascular Risk Factors, and Self-Efficacy of Diabetic Control among Obese Type II Diabetic Patients

Purpose

The purpose of this study was to examine the effects of problem solving nursing counseling and walking exerciseon weight loss, cardiovascular risk factors, and self-efficacy of diabetic control among obese diabetic patients. The Polar heart rate monitor was used for walking exercise to utilize the Biofeedback mechanism.

Method

Fifty nine diabetic patients were conveniently placed into experimental (n=35) and control groups (n=24). The experimental group participated inweekly nursing counseling for 12 weeks and was encouraged to do walking exercise using a Polar monitor. The control group remained in the same treatment as before. The data wascollected from November 2003 to August 2004 and analyzed using t-tests and ANCOVAs.

Results

After 12 weeks, the participants in the experimental group reported significantly decreased body weight (p=.004) and total scores on theParma scale (p=.001). While the participants in the control group reported significantly increased levels of blood triglyceride (p=.046) and HDL (p=.018).

Conclusion

Based on the findings, we concluded that problem focused nursing counseling with intensified walking exercise could reduce the risk of cardiovascular complications and body weight among obese diabetic patients. Future research to explore the long-term effects of nursing counseling on diabetic complications is warranted.

The Relationship between Early Neo-maternal Exposure, and Maternal Attachment, Maternal Self-esteem and Postpartum Depression in the Mothers of NICU Infants

Purpose

This study was performed to investigate the quantities of three neo-maternal exposures; visiting frequency, auditory contact and physical contact, and to examine the relationship between the quantities of each exposure and maternal attachment, maternal self-esteem and postpartum depression in 40 mothers of NICU babies during the first week in the NICU.

Method

Each neo-maternal exposure was counted at every mother's visit to the newborn and maternal attachment, maternal self-esteem and postpartum depression were measured using the maternal attachment inventory, the maternal self-report inventory and Edinburgh Postpartum Depression Scale (EPDS) on the first and seventh day in the NICU.

Result

The Mean of each neo-maternal exposure was 8.77(2.81) for the visiting frequency, 5.82(3.66) for the auditory contact and 5.60(2.89) for the physical contact during 7 days in the NICU. No significant changes were found in the scores of maternal attachment, maternal self-esteem and postpartum depression between the first and the seventh day in the NICU. The quantities of neo-maternal exposures were positively related to the scores of maternal attachment and maternal self-esteem but not related to postpartum depression.

Conclusion

The results of the study suggest the lack of early neo-maternal exposure in cases of NICU hospitalization negate its beneficial effects on maternal psychological well-being in increasing maternal attachment and self-esteem. More efforts are neededfor the neo-maternal interaction and the reevaluation of NICU visitation hours in order to promote maternal-infant interaction.

The Effect of Web-based Multimedia Contents for a Course of Physical Examination and Health Assessment

Purpose

This study was to test the effectiveness of Web-based multimedia contents for Physical Examination and Health Assessment on learning achievement.

Method

Multimedia contents based on Jung's teaching and learning structure model were used to enhance learning achievement. Learning achievement was measured by the knowledge of Physical Examination and Health Assessment. The participants of this study were students in a BSN and RN-BSN program in a university located in Seoul. 59 students in the experimental group received lectures using web-based multimedia contents and 75 students in the control group received regular lectures.

Results

The mean score of the degree of educational achievement in the experimental group(mean=31.09) was significantly higher than in the control group(mean=25.55)(t=-3.883, p=.000).

Conclusion

These web-based multimedia contents were found to maximizethe effectiveness of the teaching process when used as a teaching aid, and yet kept the strength of a face to face teaching learning method. This program is recommended as part of a main text, vital teaching aid or cyber lecture materials in nursing schools and in health care educational institutions.

The Persuasive Effects according to Types of Exercise Promotion Advertisements for Obesity Prevention in Elementary School Students

Purpose

This study was to identify the persuasive effects of exercise promotion advertisements for obesity prevention according to the source types(ordinary people, experts, celebrity endorsers) and advertising message types(slices of life, testimonials).

Method

Gender, height, body weight, BMI, attitude toward obesity(Aob) and exercise(Aex), and intention to exercise(Iex) were collected from 626 elementary school students in a pretest. After 2 months, six advertisements type attached to a questionnaire were provided for 20 minutes and then Aob, Aex, Iex, source credibility and attitude toward advertisements(Aad) were collected in posttest.

Result

1) In posttest the Iex of 6 the groups increased significantly compared with that of the pretest, 2) Source credibility of the Ordinary+Testimonial group was lower than the Celebrity+Testimonial, Celebrity+Slice of life, Ordinary+Slice of life, and Expert+Testimonial groups. Aad of the Celebrity+Testimonial group was higher than the Ordinary+ Testimonial group. 3) The Main effect and interaction effect of source types and advertising message types were significant in source credibility and Aad.

Conclusion

Persuasive effects of exercise promotion advertisements in elementary school students was found to be the most effective in Celebrity+Testimonial. This study suggests that selection of health education advertisements according to demographic characteristics is important to promote persuasive effects.

Types of Students' Death Attitudes Majoring in Human Service Area: Q-Methodological Approach

Purpose

The purpose of this study was to analyze death attitudes of students majoring in the human service area, such as nursing science, education, and social welfare.

Method

The Q-methodology which provides a method of analyzing the subjectivity of each item was used. The 38 selected Q-statements from each of 42 subjects were classified into a shape of normal distribution using a 9 point scale. The collected data was analyzed using a QUANL PC program.

Result

Four types of death attitudes for research subjects in nursing, education, and social welfare areas were identified. Type I is fatalistic admission, Type II is pursuit of existential life, Type III is uncertainty of life after death, and Type IV is separation-connection between life and death.

Conclusion

The results of the study indicate that different approaches of death educational programs are recommended based on the four types of death attitudes.

A Study of the Effects of Behavior Contact in Early Mother-Infant Attachment

Purpose

The main purpose of this study was to establish a nursing intervention data base to improve maternal attachment.

Method

The first group of mothers(control group), experienced their first physical contact with their infants after being discharged from the hospital. The second group (experimental group) practiced early initial mother-infant postpartum contact known as the most sensitive period for founding maternal-infancy attachment. The subjects of this study gave birth to normal infants at M hospital from Aug.25 to Sept.30, 2004. During the same time, data was collected through direct observation, with instruments designed by Cropley et al., to assess the behaviors of normal attachment. The statistical methods for data analysis were percentage, mean, standard deviation and t-test with an SPSS program.

Result

The group practicing initial mother-infant contact, showed a higher degree of physical and functional bonding than the group experiencing the first contact after discharge from the hospital (control group)(p<.01). The group practicing initial mother-infant contact, showed higher degrees of bonding attachment assessments than the group experiencing their first initial contact after their discharge from the hospital(p<.01).

Conclusion

The group practicing early mother-infant contact, showed more maternal-infant interaction than the group experiencing their initial contact after their discharge from the hospital. These results show that maternal attachment behavior increases according with an early initial mother-infant contact.

Knowledge, Health Belief, and Self-efficacy Related to Osteoporosis

Purpose

This research was conducted to examine the relationship of knowledge, health beliefs, and self-efficacy on osteoporosis.

Method

The design of this study was a correlative design. The subjects were 1,615 Korean adults over the age of 20 registered in a new town and participants were asked to fill in a questionnaire developed by Kim, Horan & Gendler (1991), translated into Korean.

Results

1) The subjects recorded an average score of 11.10 on osteoporosis knowledge. The average scores on the osteoporosis health beliefs was 15.68 for perceived susceptibility and the respondents recorded an average score of 40.40 for osteoporosis self-efficacy. 2) There were statistically significant differences in the degree of osteoporosis knowledge, self-efficacy and health belief according to gender, age, scholastic achievement, marital state, and jobs. 3) There were statistically significant positive correlations between osteoporosis knowledge and self-efficacy.

Conclusion

CONCLUSION: According to these results, an osteoporosis education program improving not only knowledge but also self-efficacy and health beliefs should be developed and applied to decrease the perception of barriers to exercise and intake of calcium.

Effects of Walking Exercise on Metabolic Syndrome Risk Factors and Body Composition in Obese Middle School Girls

Purpose

This study was done to determine the effects of walking exercise training(WET) on metabolic syndrome risk factors and body composition in obese middle school girls.

Method

A non-equivalent pretest-posttest experimental design was used. Twenty seven subjects participated in this study from one women's middle school in Busan. The participants were purposely allocated to an experimental group (n=14) and a control group (n=13). The experimental group participated in 30-60 minutes of WET with 55 to 75% of a maximal heart rate six days a week for 12 weeks.

Results

The prevalence of individual risk factors on metabolic syndrome were improved in the experimental group after the intervention. The high waist circumference(WC), high triglyceride(TG), low high density lipoprotein cholesterol(HDL-C), high blood pressure(BP) and high fasting glucose(FG) were 21.4, 21.4, 14.3, 28.6 and 7.2%, respectively. There were significant differences in WC (F=22.24, p<.001), TG (F=5.34, p=0.30), body weight(F=21.99, p<.001), fat mass(F=19.17, p<.001), and % body fat(F=17.93, p<.001) between the experimental and control group after the intervention. However, there were no significant differences in HDL-C, FG and BP between the experimental and control group after the intervention.

Conclusion

These results indicate that WET is effective in decreasing risk factors of the metabolic syndrome and body composition components in obese middle school girls. These findings suggest that WET can be useful as a nursing intervention in the prevention of obesity-related disorders in obese adolescents.

Development and Testing of a Sexually Transmitted Diseases Prevention Program in At-Risk Prostitutes

Purpose

The purpose of the study was to test the effectiveness of an 8 session intervention program to prevent sexually transmitted diseases (STDs) among at-risk prostitutes.

Methods

An experimental research design was employed. Subjects were 59 prostitutes (29 in the control group and 30 in the experimental group) who agreed to participate in this study. An STD Prevention Framework derived from Cox's Interaction Model of Client Health Behavior guided the overall intervention and the components. The intervention had 8 sessions with STD-prevention strategies and was led by Public Health Nurses. Analysis included change scores, χ2-test, and t-test.

Results

The results revealed significant increase in feelings and skills of condom use, peer belief on condom use, condom use practice, and satisfaction with service at public health centers (PHC) on STDs knowledge and skills in the experimental group. However, newly contracted STDs were not significant statistically between groups.

Conclusion

The 8 session STDs prevention program showed a effect on emotions, skills and behaviors of condom use even withthe limitation of methodological rigors because of subject-specific conditions. In the future, a capacity-building model based on collaborating networks among community-based organizations will be needed to develop in effective STDs prevention.

Influencing Factors of Depression in Women with Endometriosis

Purpose

The purpose of this study was to understand depression in women with endometriosis and to identify the factors influencing depression.

Method

The instruments used were the Beck Depression Inventory for depression, Lemaire Scale for endometriosis syndrome, Mishel Uncertainty in Illness Scale-Community form (MUIS-C), the Rosenberg Self-esteem Scale, and Quality of Life (Endometriosis Health Profile Questionnaire; EHP-5) by Jones et al. The Subjects of Study were 118 outpatients diagnosed with endometriosis from 7 hospitals in Busan City and Kyungsangnam-do. Data was collected from Aug 1, 2004 to Jan 31, 2005. For statistical analysis of collected data, frequency analysis, analysis of variance, and stepwise multiple regression analysis were used with the SPSS statistical program.

Result

The general characteristic showing a statistically significant difference in depression in the women with endometriosis was marital status. The score of the depression showed a significantly positive correlation with the score of endometriosis TSD(r= .464, p= .000), uncertainty(r= .393, p= .000), and quality of life(r= .543, p= .000). Depression showed a significantly negative correlation with the score of self esteem(r= - .557, p= .000). Stepwise multiple regression analysis revealed that the most powerful predictor of depression in the women with endometriosis was self-esteem (R2=0.311). A combination of self esteem, quality of life, and total symptom distress accounted for 50.0% of the variance in depression in women with endometriosis.

Conclusion

The influencing factor on depression in women with endometriosis was self- esteem, quality of life, and endometriosis TSD. Further studies need to be done to identify methods of overcoming and the presentation of depression in endometriosis.

Anger and Health Status in Late School-Age Children

Purpose

This cross-sectional study was designed to identify anger-expression types in late school-age children and investigate the relation between the identified anger-expression types and their health status.

Methods

One thousand twenty seven children in elementary school fifth and sixth grades were recruited from November to December, 2004. Data was analyzed using descriptive statistics, cluster analysis, 2-test, ANOVA, Duncan's multiple comparison test, and Wilcoxon rank sum test.

Results

Three anger-expression types in late school-age children were found; Anger-out/in, Anger-control, and Low anger-expression types. Children frequently using the anger-out/in type among the three types and with a higher state anger reported higher psychosomatic symptoms and depression. Children from a divorced or separated family reported higher state anger.

Conclusion

This study suggests that a specific anger management program needs to be developed for late school-age children with high state anger and frequently using the anger out/in expression type. For understanding the anger level and the anger expression types of Korean school-age children, further research needs to be done with large samples using a randomized sampling method.

Effects of Assertive Training on Interpersonal Relations, Social Behavior, and Psychiatric Symptoms in Patients with a Mental Disorder

Purpose

This study was designed to examine the effects of an assertive training program on interpersonal relations, and psychiatric symptoms in patients with a mental disorder.

Method

The study employed a quasi experimental design. The subjects included44 patients with a mental disorder, 20 in the experimental group, and 24 in the control group. Data was collected using structured questionnaires over a 3 month period.

Results

There were greater significant increases in scores of interpersonal relations and content of communication in the experimental group than the control group. Also, there was a greater significant decrease in the score of psychiatric symptoms in the experimental group than the control group.

Conclusion

Assertive training has an effect on increasing content of communication and decreasing psychiatric symptoms.

Differences in Processes of Change, Decisional Balance, and Temptation Across the Stages of Change for Smoking Cessation

PURPOSE: The purpose of this study was to investigate smoking cessation behavior of male taxi drivers in Korea on the basis of the Transtheoretical model(TTM), and to validate the usefulness of TTM. METHOD: Data were collected using a self-reported questionnaire including smoking history and major factors of TTM from 208 subjects who were current smokers or ex-smokers. Data were analyzed by descriptive statistics and ANOVA. RESULT: Most subjects (85.1%) were current smokers. Stages of change were precontemplation (44.7%) and contemplation (27.4%). Subjects in precontemplation stages had the lowest mean score in processes of change and the highest mean scores in decisional balance(pros) and temptation(positive affective, habitual/craving). According to stages of change, there were statistically significant differences in processes of change, decisional balance, and temptation. CONCLUSION: This study supported the generalization of TTM. As this study showed that the subjects didn't have motivation in smoking cessation, applying tailored smoking cessation programs for taxi drivers is needed.

Feminism in Nursing Science in Korea

Purpose

Although feminism has been actively discussed and applied to nursing in Western societies since the 1980s, it is little known among Korean scholars as well as Korean nurses. This article explores the use of feminist perspectives in nursing science in other developed countries and suggests how feminism could be applied to nursing science in Korea.

Method

The literature related to nursing and feminism were reviewed in terms of nursing practice, education, and research.

Result

This article describes what feminism is and how feminism and nursing have evolved historically over time in other countries, especially in Western societies. In addition, it discusses how it can be applied to nursing practice, education, and research in Korea.

Conclusion

Accepting feminist perspective in Korean nursing could benefit in empowering nurses by valuing nursing, by raising self-esteem of nurses, and by raising the consciousness of socio-political realities. Eventually it could benefit in changing and developing nursing science in Korea.

Effects of Web-based Diabetic Education in Obese Diabetic Patients

Purpose

The purpose of this study was to evaluate the effect of web-based diabetic education on plasma glucose and serum lipids in obese people with diabetes.

Methods

A random allocation design with control and experimental groups being assessed pre- and post-intervention was used. Eighteen patients were randomly allocated to an intervention group and 16 to a control group. Participants were requested to input their blood glucose levels weekly for 3 months at http://www.biodang.com by cellular phone or wire Internet. The researcher sentoptimal recommendations to each patient weekly for 3 months using a short message service (SMS) of the cellular phone and wire Internet.

Results

Patients in the intervention group had a mean decrease of 1.2% in glycosylated haemoglobin (HbA1c) levels and those in the control group had no difference in HbA1c levels. There was a significant mean change in 2-hour post prandial blood glucose (2HPPG) for the intervention group, with a mean change of -120.1 mg/dl. The mean change in the control group, however, was not significant.

Conclusion

These findings indicate that this web-based intervention using SMS of the cellular phone for 3 months improved HbA1c and 2HPPG, but did not affect total cholesterol, triglyceride, and high density lipoprotein cholesterol in obese type 2 diabetic patient.

Purpose

The purpose of this study was to develop a meaning of life scale with high validity and reliability.

Methods

A conceptual framework composed of 4 phases of meanings of life was identified. And 49 preliminary items on a 4-points scale were developed through content validity. A reliability and validity test of the 49 items was conducted on 564 adults. By means of internal consistency of the 49 items, 1 item was deleted. To verify the 48 items, factor analysis, reliability test, and LISEREL were done.

Results

Through exploratory factor analysis of the 48 items, 8 factors were extracted. These factors were labeled as ‘ self- awareness and self-acceptance’, ‘ hope’, ‘ responsibility awareness’, ‘ love experience’, ‘ self transcendence’, ‘ relation experience’, ‘ self contentedness’, and ‘ Commitment’. Through LISEREL of the 48 items, 2 items were excluded and finally 46 itemsremained. Cronbach's Alpha of the 46 items was .94. The correlation coefficient of the Self-esteem scale was .79.

Conclusion

By the above results, the researchers recommend the following: An exploratory study on the variables related to the meaning of life are needed for criterion validity of this scale. Studies on meaning of life of different groupa, and subjects are needed for reverification.

The Effects of a Health Promotion Program in Rural Elderly on Health Promotion Lifestyle and Health Status

Purpose

The purpose of this study was to identity the effects of a health promotion program for rural elderly on health promotion lifestyle and health status.

Method

The study was a nonequivalent control group pre-post experimental design. Data collection was performed from April 12th, 2003 to August 2nd, 2003. The subjects were selected at Mari Myun Geochang Gun in Korea. 44 elders were in the experimental group and 45 elders were in the control group. The 16-week health promotion program was given to the experimental group. Data was analyzed by descriptive statistics, χ2-test, t-test, and ANCOVA test with SPSS/Win 10.0 program.

Result

The experimental group showed higher scores of a health promotion lifestyle and perceived health status than the control group. In addition, systolic BP, heart rate, body fat and glucose of the experimental group were lower than the control group. Waist flexibility, left hand grip power, back strength and leg strength of the experimental group were higher than the control group. However, there were no significant differences in diastolic BP, total cholesterol and right hand grip power between the two groups.

Conclusion

This health promotion program for rural elderly can be recommended as an effective nursing intervention in rural communities.

The Experience of Life Experiences among Patients with Chronic Low Back and Extremity Pain

Purpose

The purpose of this study was to understand the meaning and nature of pain experienced among patients with chronic pain. The present study adopted a hermeneutic phenomenological method which was developed by van Manen.

Method

The participants for this study were 4 men and 5 women, who were over the age of 20 with chronic pain more than 6 months. Data was collected by using in-depth interviews and observations from September, 2004 to December, 2004.The contents of the interviews were tape-recorded with the consent of the subject.

Result

The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were ‘ untamed and unremitting pain’, ‘ the body as an obstacle’, ‘ continuity of suffering time as if the moment would never end’, ‘ a narrow radial range of action’ and ‘ separating from other people’.

Conclusion

Patients with chronic pain experienced and perceived the world through the filter of their pain. It is necessary for nurses to understand the experiences of chronic pain patients and to provide more empathic, supportive care. Further research is needed on nursing interventions that could help chronic patients cope with and find the meaning in their suffering.
