This study was undertaken to investigate the casual perceptions and health seeking behaviors of Rheumatoid arthritis patients, define and understand the typology, and find the relationship between causal perceptions and health seeking behavioral types. There were six types(Physical Fatigue, Dispensation of Nature, Causality to Environment, Conscience of Guilty, Rationally perceiving, Psychological Stress) of subjective opinion about Causal Perceptions of Rheumatoid Arthritis Patients. And there were four types(Oriental medical Treatment, Information Seeking Dietary Control, Western Medical Treatment) of subjective opinion about Health Seeking Behaviors. In the relationship between types of the causal perceptions and health seeking behaviors, oriental medical treatment and information seeking type were common health seeking behaviors of all six causal perception types, Only difference for internal causal perception types was related to hospital instructions and external causal perception types were related to dietary control. The result of this study can help health care provides, especially nurses to understand the types of causal perceptions and health seeking behaviors of Rheumatoid arthritis patients to gain treatment educational nursing intervention to aid health care.
This study was conducted to develop an education program of hospice care for the professional in order to care for nurses for terminally ill patients facing death and their families. The Modified Tyler-Type Ends-Means model was used to guide the curriculum development of the study. The curriculum include a philosophical conception of hospice education, fundamental concepts, purpose, objective and the educational contents. The content was developed based upon a 70% or more demand in educational demand analysis. The education program has a total of 360 hours consisting of 172 hours of theoretical study and 188 hours of practice including fundamental nursing care for hospice.
Koreans rarely complain of depression or other psychological problems because mental illness is viewed as a stigmatizing and threatening experience. This study examined the relationships among stressful life events, close relationships, self-esteem, and depression in 400 college women aged 18 to 30 years. Depression was positively related to stressful life events(r=.21 p=<.01). Depression and stressful life events were negatively associated with self-esteem(r=-.67: r=-.11, p<.01; p<.05 respectively). Close relationships with women and men friends were each negatively associated with depression (r=-.24; r=-.16, p<.05). Close relationships with women and men friends were positively associated with self-esteem (r=.23; r=.20, p<.01). Forty nine percent of variance in depression rates in this sample was explained. After adjustment for degrees of freedom, a total of 49% of the variance in depression was explained by self-esteem and stressful life event. This investigation into the relationships among the variables influencing depression for college women is a critical issue as health professional interventions are those designed for specific populations to meet unique care needs and since young Korean women may be at considerable risk for depression.
This study examined those problems noticed under the Asymptotically Generalized Least Squares estimator in evaluating a structural model of physical health. The problems were highly correlated parameter estimates and high standard errors of some parameter estimates. Separate analyses of the endogenous part of the model and of the metric of a latent factor revealed a highly skewed a kurtotic measurement indicator as the focal point of the manifested problems. Since the sample sizes are far below that needed to produce adequate AGLS estimates in the given modeling conditions, the adequacy of the Maximum Likelihood estimator is further examined with the robust statistics and the bootstrap method. These methods demonstrated that the ML methods were unbiased and statistical decisions based upon the ML standard errors remained almost the same, Suggestions are made for future studies adopting structural equation modeling technique in terms of selecting a reference indicator and adopting those statistics corrected for normality.
The Purpose of this study was to explore the meaning of breastfeeding using a phenomenological research methodology. The sample consisted of 9 primiparous mothers who had breastfed their babies at least for 3 consecutive months. Data were collected from January 10 to February 28, 1998 through interviewing. The research question was "What was the meaning of breastfeeding to breastfeeding mothers?" Responses to non-structured open-ended question was audio-recorded during the interview. Data were analyzed using the phenomenological method of Colazzi. From the protocols, significant statements about meaning of breastfeeding were clustered into 8 themes. Mothers perceived meaning of breastfeeding as feeling of unification through tie, expression of love, gift, nutritional supplement, duty as a mother, nurturing, mystery, and identity. From the results of this study, breastfeeding mothers perceived breastfeeding positively, so nurses need to encourage mothers to breastfeed her baby and develop program which can promote successful breastfeeding.
The central concept of the Salutogenic Model is sense of coherence, which is defined as a global orientation that expresses the extent to which one has a feeling of confidence that one's internal and external environments are comprehensible, manageable, and meaningful. Sense of coherence is proposed as a determinant of positive health consequences and successful coping. The purpose of this article is to review Antonovsky's Salutogenic Model, the concept of sense of coherence, and its central components and sources. For conceptual clarity, sense of coherence is compared and contrasted with the concept of hardiness. The empirical research findings are integrated to better understand sense of coherence and to enhance future implications for nursing researching and practice.
Although healthy lifestyle have been proved as an effective way of improving higher well-beings for individuals, researches on health -promoting behaviors of minority elderly with a specific ethnic heritage have been sparsely tried. This study was designed to explore health-promoting lifestyle patterns of Korean immigrant elderly living in Seattle. USA and its relationships with two associated elderly were recruited from two senior centers and interviewed with a structured questionnaire. Data were collected from October 1998 to January 1999, and analyzed using SPSS program through which t-test, ANOVA, and Pearson Correlation Coefficients were tested. As the results, the mean HPLP score of the Korean immigrant elderly was 2.54 (SD =.36), showing significant differences by education (F=3.61, P=.016), economic status (F=3.01, P=.034), and current health status (F=3.69, P=.008). In self-esteem, two socioecomic variables showed statistical association with self-esteem; martial status (t= 2.47, P=.015) and living situation (F= 4.03, P= .021). The HPLP subscales that showed higher mean scores were nutrition (M=3.01, SD= .52) and interpersonal support (M= 2.65, SD= .47) while lower mean scores were detected in the domain of excercise (M= 1.92, SD= .74) and stress management (M= 2.26, SD= .47). Perceived health status revealed significant positive correlation with health-promoting lifestyle patterns (r= .19, P=.043) and self-esteem (r= .19, P= .043) and self-esteem (r= .32, P= .001) in the present study. It is concluded that engagement in health-promoting lifestyle patterns should be actively encouraged to enhance personal health of Korean immigrant elderly. Developing health promotion programs focused on exercise and stress management is also imperatively suggested not only for better health practices of Korean immigrant elderly population but also for enhancing their level of well-beings and life satisfaction.
This study has been attempted using the Q methodology to clarify leader type of nurse managers that head nurses and general nurses recognize, and to clarify its relative relation. Sixty-three statements were extracted through interviews with general and professional people interested in the subject of nurse leaders to extract the Q population. Atotal of 314 Q population was formed added with 251 questions extracted from related documents. Final 32 Q samples were selected by reorganization of 314 Q population after reexamining statements through inquiry of 1 professor of the nurse department, 2 students in course of nurse science masters degree and 2 students in course of doctoral degree. The P sample selection standard of this study were 25 nurses and 30 head nurses. Examination subjects themselves filled out 32 statements classified in a measure of 9 points from agreeable items to disagreeable items, Principal component factors were analyzed using the QUANL pc program after grading the contents of the P sample. Nurses recognizing subjective structure for leader behaviors of nurse manager were analyzed to be 3 factors: vision presentation type, self-capability consideration type, relationship consideration type, and head nurses recognizing subjective structure were analyzed to be 2 types ; task pursuit leader type, and concord pursuit type. Nursing manager's leader behavior, expected by staff nurse are more complex and higher level which may combined with task pursuit leader type in concord pursuit leader of head nurse. Also according to Hersey and Blanchard theory(1977), the effectiveness of leadership becomes to be larger as the accordance rate between the behaviors of nurse leaders and followers reaction increase. Two suggestions have been made based on the conclusion. 1. Studies on creating strategies in relation to development, management, selection of nurse leaders should be made based on this study. 2. There is a need for relative study of production and degree of similarity of leadership types based on this study.
Diabetes mellitus, a universal and prevalent chronic disease, is projected to be one of the most formidable worldwide health problems in the 21st century. For those living with diabetes, there is a need for self-care skills to manage a complex medical regimen. Self-efficacy which refers to one's belief in his/her capability to monitor and perform the daily activities required to manage diabletes has found to be related to self-care. The concept of self-efficacy comes from social cognitive theory which maintains that cognitive mechanism mediate the performance of behavior. The literature cities several research studies which show a strong relationship between self-efficacy and self-care behavior. Meta-analysis is a technique that enables systematic review and quantitative integration of the results from multiple primary studies that are relevant to a particular research question. Therefore, this study was done using meta-analysis to quantitatively integrate the results of independent research studies to obtain numerical estimates of the overall effect of a self-efficacy with diabetic patient on self-care behaviors. The research proceeded in three stages : 1) literature search and retrieval of studies in which self-efficacy was related to self-care, 2) coding, and 3) calculation of mean effect size and data analysis. Seventeen studies which met the research criteria included study population of adults with diabetes, measures of self-care and measures of self-efficacy as a predictive variable. Computation of effect size was done on DSTAT which is a statistical computer program specifically designed for meta-analysis. To determine the effect of self-efficacy on self-care practice homogeneity tests were conducted. Pooled effect size estimates, to determine the best subvariable for composite variables, metabolic control variables and component of self-efficacy and self-care, indicated that the effect of self-efficacy composite on self-care composite was moderate to large. The weighted mean effect size of self-efficacy composite and self-care composite were +.76 and the confidence interval was from +.66 to +.86 with the number of subjects being 1,545. The total for this meta-analysis result showed that the weighted mean effect sizes ranged from +.70 to +1.81 which indicates a large effect. But since reliabilities of the instruments in the primary studies were studies were low or not stated, caution must be applied in unconditionally accepting the results from these effect sizes. Meta-analysis is a useful took for clarifying the status of knowledge development and guiding decision making about future research and this study confirmed that there is a relationship between self-efficacy and self-care in patients with diabetes. It, thus, provides support for nurses to promote self-efficacy in their patients. While most of the studies included in this meta-analysis used social cognitive theory as a framework for the study, some studies use Fishbein and Ajzen's attitude model as a model for active self-care. Future research is needed to more fully define the concept of self-care and to determine what it is that makes patients feel competent in their self-care activities. The results of this study showed that self-efficacy can promote self-care. Future research is needed with experimental design to determine nursing interventions that will increase self-efficacy.
In the last few years, psychiatric nurse practitioners have shown a growing an interest in community psychosocial rehabiliation, caring for chronic psychiatric patients as case manager in South Korea. The purpose of this study was to evaluate the effectiveness of a community psychiatric rehabilitation nursing program on self-care actively and quality of life and to suggest this program as an effective nursing intervention in a group of chronic home-based psychiatric patients in a poor town. A nonequivalent control group, pretest-posttest design was used . Of the twenty women that started the program, sixteen finished it. The data were analyzed by the Wilcoxon Rank Sum Test. The program included the process of case management which consisted of four phases: the first was an active case finding and pre-test, the second was home visiting and contracted by phone, the third was group activity therapy of 12 sessions, and the fourth phase was terminal and post-test. The effects of the program were assessed by quality of life and self-care activity. The quality of life and the self-care activity, especially, area of nutrition, elimination, dressing, leisure activity, and follow-up clinic visiting showed greater improvement than those of the control group. The results of this study suggest that this program was effective in improving the quality of life of chronic home-based psychiatric patients.
The author investigated pain experiences of 90 cancer patients and adequacy of pain treatment they have received during their stay at a large medical center in T city between October 1994 and August 1995. Pain was assessed by the Shortened BPQ and results are summarized as follows: As for ratings of "worst pain" during the 24 hour period, 70% of the patients reported they had "severe" pain. As for ratings on "pain now" 43% of the cancer patients reported "moderate to severe" pain. Over 46% of the patients reported a pain relief score of 0(not at all) or 1(somewhat) even after receiving pain medication. Adequacy of analgesic treatment was evaluated by comparing the patient's reported level of pain and the analgesic use, namely, the pain management index(PMI). The PMI indicated that 58% of the patients were undertreated for the pain control. In review of nurse's notes, systematic pain assessment was scarcely recorded, although pain documentation appeared in 70% of the notes; and the contents were mostly simple description. In conclusion, the results of patient's pain ratings, the PMI and poor pain documentation in the nurse's notes implied poor pain assessment and management.
The concept of empathy was examined in the professional caregiving relationship and its application was extended to the context of informal caregiving. Using the Lazarus and Folkman model. the influence of empathy on the caregiver's experience in the caregiving relationship was illustrated. The effects of the caregiver's empathy on his/her own caregiving outcomes were investigated by examining the relationship between empathy and burnout experience and life satisfaction. Empathy increased emotional exhaustion while increased personal accomplishment and personalization. indicating conflicting relationship between empathy and burnout. This conflict relationship between empathy and burnout can be explained by suggesting the distinct roles of two dimensions of empathy: emotional and caregiving context and to examine the definite roles of two dimensions of empathy were suggested.
The purpose of this study was to examine the correlation between juvenile alcohol use and their criminal patterns. The data were collected through questionnaire surveys. Subjects serving for this study were 971 delinquent adolescents in Korea, sampled from 6 juvenile corrective institutions and 2 classification judging institutions, using a census method. Their age range was between 12 and 21. Data were analysed by IBM PC using SAS program. Statistical methods employed were Chi-square and frequency analysis. 1. Of 877 respondents, the number of adolescents committed criminal behaviors while the intoxicated were 230(26.2%), and 647(73.8%) were in a non-intoxicated state. 2. Adolescent under intoxication showed a higher rate of aggressive crimes and assault crimes, whereas adolescents under the non-influence of liquor tended to commit property crimes and Violations of criminal special law. Drunken under intoxication showed the higher rate of aggressive crimes and assault crimes, whereas adolescents under the non drunken state. Most crimes have happened without any tools in both group. 3. In comparison of the alcohol user and the non-user, most alcohol-related crimes among adolescents were committed at AM 0:00 to AM 4:00 during the weekend in the dark, cloudy, and stormy rainy day, while non-alcohol related crimes were at a afternoon of weekday in the clear day. The places that the criminal activities occurred were streets, amusement places such as disco-theque, fields and their own house among alcohol users, whereas victim's house, another person's house and restaurant were chosen among non-alcohol users. 4. The victims assaulted by juvenile offenders in both drunken and non-drunken state were mostly passer-by(65.4%), followed by their friends(25.1%). And the conditions of victims showed a significant differences between the drunken adolescents and the non-drunken adolescents. The victim's conditions assaulted by intoxicated delinquent adolescents were in quarreling or drunken state, whereas non-alcohol related crimes were directed against victims in a sleeping or irresistible state. 5. Almost over the half of delinquent adolescents perceived their delinquency as wrong behaviors, and alcohol non-user tended to more significantly perceive their criminal acts as wrong conducts. About the half of respondents answered that they committed their criminal acts in spite of having a very good judgement while doing crimes, the author did not found a significant difference between the two groups. The reasons given for crimes were manifested as follows: it can be seen that to get money for amusements'(30.4% of all motives) were most common. followed by to commit accidentally the offences'(23.8%). curiosity or heroism'(18.9%). alcohol related crimes tended to be accidental and impulsively without any clear planning, while non-alcohol related crimes tended to be purposeful, directed to make money, motivated by curiosity or a desire to live heroically. In Conclusions, the correlation between alcohol use and juvenile criminal behaviors has been examined in this study. Generally, alcohol use had been found to be highly correlated with aggressive assault crimes including robbery, burglary and rape etc.
The Profile of Mood States (POMS) is the most widely used self-report instrument for the measurement of affect or mood in clinical and nonclinical populations. This paper reports on the translation and testing of a Korean version of the POMS. The translation involved three steps: translation, checking agreement, and panel discussions to arrive at consensus. Then, the Korean version of the POMS was tested with a sample of 47 healthy Koreans who lived in the U.S.: they completed the instrument in the morning, at the beginning of work and in the evening, at the end of work. Internal consistencies for the total scale and subscales were high(alphas= .93 and .94). Face and content validity and the cultural relevance of the Korean version of the POMS were tested through review by five bilingual Korean nursing scholars who were familiar with Korean version of the POMS was then compared with Haeok Fatigue Behavior Check-list and demonstrating significant concurrent validity (r= .87, p<.001). Construct validity was established by demonstrating significant differences between the scores on the scores on the Fatigue and Vigor subscales (p< .001) at the beginning work and at the end of work.
The purpose of this study was to investigate the residents' opinions about community mental health in the Taegu-Kyungpuk area for the future development of a community mental health program and model appropriate for Korea, Yhe subjects were 152 residents in the Taegu-Kyungpuk area. In July 1999, the data was collected using a convenience sample technique. Mental health status was significantly different for the level of occupational advantage(x2=15.684, p<.05) and physical health(x2=39.262, p<.000). Factor structure for mental health problems with the percentage of variance was as follows: optimistic view(27.581). dark view(10.758). mastery(6.200), discomfiture(6.101) and life style(5.641). Most of the respondents(92.1%) took the mental health problems seriously. The serious aspects of the mental health problem were found to be epilepsy, mental retardation, neurosis and schizophrenia respectively. Concerning about the view of community mental health, most of the respondents answered that the a C.M.H.C. was 'useful and urgent' concering the need for C.M.H.(77.6%). They answered positively on the utilization of C.M.H.C(75.7%) and preferred the separately new community mental health center. A psychiatrist was preferred as the key person in charge(44.1%). If community mental health centers were established in a community health center, they answered that the expected major problem would be quality control of care(44.7%). They preferred the psychiatrist's office as the recommended agency for the insane(44.7%). Opinions of the asylum system were found very negative in respect to psychiatric therapy and humanitarianism. The results of this study will help establish a relevant model for this community as the primary site for a community-based mental health model.
