DOI: https://doi.org/10.4040/jkan.2006.36.1.15
AbstractAbstract PDF
Purpose

This study was to examine the effects of foot reflexion massage on sleep disturbance, depression disorder, and the physiological index of the elderly in nursing homes.

Method

The research design was a non-equivalent control group pretest-posttest quasi-experimental study. The subjects in this study were 50 elderly people who resided in two different nursing homes in the same region. An experimental group and a control group were organized with 25 subjects respectively, and foot reflexion massage was provided for 12 sessions, 30 minutes per session. The selected dependent variables were sleep disturbance, depression disorder, and physiological indices(blood plasma serotonin, serum cortisol), which were all measured before and after foot reflexion massage was offered. Data analysis included χ2-test, t-test, paired t-test, and ANCOVA, using the SPSS program package.

Result

1. The experimental group improved sleep quality more than the control group. 2. The experimental group had less depression disorder than the control group. 3. The experimental group had higher serotonin levels than the control group.

Conclusion

It's necessary to give foot reflexion massage as a successful nursing intervention to elderly who undergo a change in sleep, and suffer from a depression disorder due to a deterioration in sleep.

DOI: https://doi.org/10.4040/jkan.2006.36.1.25
AbstractAbstract PDF
Purpose

The purpose of the study was to identify the adaptation process to problem based learning(PBL) among nursing students who have experienced PBL classes for two years.

Method

Data was collected from 11 nursing students with in-depth interviews and direct observation of their PBL experiences by a researcher who has been a facilitator for PBL class for 3years. Immediately after the interviews all of them were transcribed. It was analyzed by the Ground theory of Corbin and Strauss.

Results

A derived core category was ‘Acquiring PBL’. 4 stages of the acquiring process were derived and written in time sequence: chaos, confusion, beginning insight, and achievement stage.

Conclusion

The results will not only expand understanding of the students for the facilitator and school which has adopted PBL but also provide information to develop an orientation program for PBL. Further research on the facilitator's role experiences is recommended.

AbstractAbstract PDF
Purpose

The purpose of this study was to explore Korean mothers' experiences of caregiving for their adult children with schizophrenia.

Method

A iterative descriptive qualitative research design was used to understand and explain the caregiving experience from the perspective of mothers. The data were collected by individual in-depth interviews and one focus group interview from 11 mothers. The ages of the participants ranged from 50 to 60 years old and the ages of their sick children ranged from late 20s to early 40s.

Results

Three major themes were identified from qualitative thematic analysis: (a) “emotional debris from the disease,” (b) “the disease that makes mothers dumb” (c) “space of rational reason.”

Conclusions

This study would provide health care professionals insights in establishing intervention for mothers by deeply understanding the process of recognition and acceptance of their children with schizophrenia.

A Study of the Relationship between Compliance with Therapeutic Regimens and Physiological Parameters of Hemodialysis Patients
Hye Sook Min, Eyn Joo Lee
Journal of Korean Academy of Nursing 2006;36(1):64-73.   Published online February 28, 2006
DOI: https://doi.org/10.4040/jkan.2006.36.1.64
AbstractAbstract PDF
Purpose

This study was done to investigate correlations between compliance and physiological parameters of hemodialysis patients.

Method

The subjects were 102 patients on hemodialysis at 3 hospitals in B city. Data was collected using Shon(1986)'s questionnaire and measuring physiological parameters (serum urea nitrogen, creatinine, hemoglobin, albumin, potassium, phosphorus, interdialytic weight gain).

Result

Mean scores of compliance with the therapeutic regimen was 4.00±0.55 on a 5 point scale. The area of visiting hospitals and taking medicines were shown to have high compliance with therapeutic regimens; on the other hand, the areas concerning diet and symptoms were shown to be low. Interdialytic weight gain and phosphorus were significantly related to the compliance with therapeutic regimens.

Conclusion

Hemodialysis patients' therapeutic compliance was related to the physiological parameters(potassium, phosphorus, interdialytic weight gain). Therefore, these findings give hemodialysis patients useful information for raising their therapeutic compliance.

AbstractAbstract PDF
Purpose

The purpose of this study was to confirm the effects of a moderate and fast walking exercise program on middle-aged women's fatigue, serum lipids and immunoglobulins.

Method

A non-equivalent control group pretest-posttest design was used for this study. The experiment was conducted for 10 weeks from May 17th to July 25th, 2004 with 44 middle-aged women, consisting of 16 for the moderate walking group, 15 for the fast walking group and 13 for the control group.

Result

Walking exercise at both a moderate and fast speed was effective in middle-aged women in reducing fatigue and serum lipids. It was also revealed that extended periods of exercise was more effective in decreasing fatigue while for reducing serum lipid, high intensity exercise was more effective. In this study, serum immunoglobulins were reduced after moderate and fast walking exercise but its cause was not fully understood so further research is needed.

Conclusion

This study helps us recognize the importance of regular exercise and promotes motivation to exercise for a healthy life among middle-aged women.

AbstractAbstract PDF
Introduction

It is important to understand the nature of the identity through the live experiences of Home Care Nurse Practitioner(HCNP) because the role identity of a professional is constructed by continuous social interactions, This study aims to understand the construction of the role identity of HCNP.

Method

Data was collected from 12 hospital based HCNPs. This study involved two focus group discussion sand four in-depth individual interviews. The main question was “what is the role of HCNP?” The debriefing notes and field notes were analyzed using consistent comparative data analysis method.

Result

First, Home care (HC) is a small clinic. HCNP brings it to home to provide various services. Second, HC is the real nursing and HCNP is the ‘genuine’ nurse who actualizes the essence of nursing in practice. Third, HC is empowering activity to promote self-care ability of the patients and their caregivers. Forth, HC is like the dish-spinning required high-level mastery and HCNP is an expert who provides the most appropriate services to the patients.

Conclusion

HCNPs have the role identity as a highly qualified professional who delivers services from hospital to home, actualizes the essence of nursing in practice, empowers the patients and their caregivers to have self-efficacy to recover, and offers the most appropriate nursing care.

Journal of Korean Academy of Nursing 2006;36(1):114-126.   Published online February 28, 2006
DOI: https://doi.org/10.4040/jkan.2006.36.1.114
AbstractAbstract PDF
Purpose

This study was performed to evaluate the pre-emptive analgesic effects of a small dose of intravenous ketamine on postoperative pain in patients undergoing a hysterectomy.

Method

Sixty patients undergoing a hystrectomy under general anesthesia were randomly allocated to 2 groups. The experimental group(30 patients) received 0.3mg/kg of ketamine after induction of anesthesia, approximately 5 min prior to surgery, but the control group(30 patients)did not receive ketamine. Data was collected in a double-blind manner from April 1st, to October 30th, 2004. Postoperatively, the patients used a patient-controlled analgesia(PCA) pump. Blood pressure, pulse rate, pain, anxiety, count of times pressing the PCA button, administeration of additional analgesics and side effects of ketamine were measured at 1 hour, 3 hours, 6 hours and 24 hours after the operation.

Result

There were no statistical differences in blood pressure, pulse rate, pain and anxiety between the experimental and control groups. There were statistical differences in blood pressure, pulse rate, pain and anxiety during the 24 hours postoperatively. In the experimental group, the number of times pressing the PCA button and administering additional analgesic drugs were significantly lower than those of the control group.

Conclusion

A 0.3 mg/kg dose of ketamine given at approximately 5 min before surgery resulted in decreasing the number of times pressing the PCA and the administration of additional analgesics.

AbstractAbstract PDF
Purpose

The purpose of this study was to identify the factors influencing symptom self management (SSM) in the patient with a chronic mental disorder.

Method

Data was collected by questionnaires from 204 chronic mental disorders in an outpatient clinic in a General Hospital and Public Mental Health Centers in Seoul and Kyunggi Province. The data was analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression.

Results

The score of SSM showed a significantly positive correlation with the score of the level of self efficacy (SE) (r=.33, p=.00), social support (SS) (r=.27, p=.00), self care agency (SCA) (r=.36, p=.00) and daily living ability (DLA) (r=.34, p=.00). The score of DLA showed a significantly positive correlation with the score of level of SE (r=.46, p=.00), SS (r=.51, p=.00), and SCA (r=.52, p=.00). The most powerful predictor of SSM was SCA (14%). A combination of SCA, DLA, and SS account for 20 % of the variance in SSM.

Conclusion

This study suggests that SCA, DLA, and SS are significant influencing factors on SSM in patients with chronic mental disorders.

DOI: https://doi.org/10.4040/jkan.2006.36.1.144
AbstractAbstract PDF
Purpose

This study was conducted to 1) find out the characteristic of smoke related characteristics of nurses. 2) find out the frequency of Smoking cessation intervention delivered by nurses. 3) compare the differences in mean scores of smoking cessation interventions by general characteristics.

Method

The survey questionnaire was mailed out to nurses who were working at the randomly selected hospitals throughout the country from November 28, 2003 to February 15, 2004.

Result

0.6% of nurses were current smokers 40.7% of nurses have attended smoking cessation education. Nurses who were older, had masters degree, had oncology experience, higher position, participated in smoking cessation education, and had smoking related disease among family members were variables related to higher frequencies in delivering tobacco interventions.

Conclusion

Although nurses are in an important position in delivering tobacco interventions and provide resources, their participation in consistent delivery of an intervention is less than desirable. To help nurses to participate in the assessment of tobacco use and interventions for cessation, the development of educational program is necessary.

DOI: https://doi.org/10.4040/jkan.2006.36.1.151
AbstractAbstract PDF
Purpose

This study was done to investigate the satisfaction with life in adolescents, and to identify factors affecting satisfaction with life for them.

Method

The participants were 1,057 adolescents. Data was collected through self-report questionnaires which were constructed to include satisfaction with life, self-esteem, hostility and hopelessness. The data was analyzed using the SPSS program.

Result

Satisfaction with life for adolescents was significantly different according to school type, grade, scholastic achievement, religion, monthly income of family, conversation with parents and conversation with friends. Satisfaction with life in adolescents correlated with self-esteem, hostility and hopelessness. Significant predictors influencing satisfaction with life in adolescents were self-esteem, hopelessness, school type, conversation with parents, monthly income of family and religion, and these predictors accounted for 37.8% of the variance in satisfaction with life.

Conclusion

The above findings indicate that satisfaction with life in adolescents is influenced by self-esteem, hostility and hopelessness. Therefore when nursing interventions are developed to improve satisfaction with life in adolescents, these factors need to be considered.

Method

A convenience sample of 32 adult burn patients who were eligible and provided consent were included in the study which was a quasi-experimental study of a nonequivalent control group pretest-posttest design. Fifteen patients in the control group received the routine burn dressing changes but 17 patients in the experimental group listened to self-selected music through headphones connected to a CD player during burn dressing changes for three days. All subjects of the music group chose the type of music that would relax them. Before and after burn dressing changes, subjects completed the State Anxiety Inventory and self-report of pain scores.

Result

There was a significant reduction in state anxiety before and after burn dressing changes in those who received music therapy in contrast to those who did not receive music therapy. The music group reported lower pain scores before and after burn dressing changes than did the non-music group.

Conclusion

These findings indicate that music therapy composed of self-selected music is a valuable intervention for the treatment of pain and anxiety in patients undergoing burn dressing changes.

AbstractAbstract PDF
Purpose

The purpose of this study was to examine stress, coping, and immune response effects of a psychosocial intervention program based on the PNI model and Stress-Appraisal-Coping for Korean patients with breast cancer.

Methods

A nonequivalent control group pretest-posttest design was used. The participants who had survived breast cancer and lived in Wonju city and the surrounding area were assigned to an intervention group (N=21) or a control group(N=18).We conducted a 12-week intervention, 2 hours a day weekly, and measured the variables at baseline, six and twelve weeks later. Dependent variables are: stress, anxiety-depression and anger, and immune response.

Results

Patients in the psychosocial intervention program reported significantly less stress perception (U=31.500, p=.023), more problem solving ability and less problem avoidance in coping (U=20.500, p= .013; U=29.500, p=.040), and less anxiety-depression (U=22.000, p=.023). No difference, however, was found in anger and immune responses between the two groups. Intervention effects were evident at week 6 and 12 for anxiety-depression, and at week 6 for problem avoidance in coping, the same time that NK cell counts and the T8 decreased.

Conclusions

These results suggested positive effects of a psychosocial intervention program. However, the results are inconclusive due to the small sample.

AbstractAbstract PDF
Purpose

This study was aimed to identify the effects of a self-foot reflexology massage on depression, stress responses and functions of the immune system of middle-aged women.

Method

This study was a one group pretest-posttest experimental design and the data was collected from August 1st, 2004 to May 31st, 2005. The subjects consisted of 46 middle-aged women (40 - 64 years) who were recruited from the Community Health Center in Busan city. Subjects were not treated for 4 weeks, subsequently they were trained in self foot reflexology massage for 2 weeks, and then they did their own daily for 6weeks (2 days at the research center, 5 days at home). The outcome variables were measured 4 times, at baseline, pre training, after training, and after the intervention. The collected data was analyzed using repeated measure ANOVA by the SPSS/WIN program.

Result

There was a statistically significant difference in depression, perceived stress, systolic blood pressure, natural-killer cells and Ig G. However, there was not a statistically significant difference in dyastolic blood pressure, pulse or serum cortisol.

Conclusion

These results suggest that a self-foot reflexology massage could be utilized as an effective nursing intervention to reduce depression and stress responses, and to strengthen immune systems in middle-aged women.
