Experience of Mothers of Mentally Handicapped Children Having Menarche

Various difficulties and inconvenience arise from having a mentally handicapped child in a family and these place many demands on mothers. There are few studies in Korea on these demands nor on what mothers go through with their mentally handicapped daughters' menarche and puberty. The purpose of the study was to examine the experiences of mothers of mentally handicapped daughters, as it relates to their daughters' menarche and the beginning of puberty. With in depth interviews, both in person and by telephone and participant observation the study used a qualitative research methodology to attempt to understand the experiences of these mothers. The data were gathered from October 1995 to April 1996. The subjects for the research included nine mothers of mentally handicapped daughters whose ages ranged from 12 to 18 and who attended one of three special schools located in either Inchon or Seoul. The data were recorded and analyzed ; meaningful statements were grouped according to subjects raised by the mothers. Content Analysis was also applied to identify similar content and confirm common experiences, and to highlight concepts and categorized them. The results of this study are as follows. Five categories were identified ; mothers' emotional responses to their mentally handicapped daughters' menarche and menstruation were of severe despair accompanied by anxiety, guilt, fear, anguish, shame and pity because the mothers were afraid their daughters would not be able to use appropriate hygienic measures during menstruation and the mothers felt heavily burdened in having to look them. The mothers also had negative feelings about their daughters' physical development. The experience of mothers related to their daughters' possibilities for marriage and pregnancy were of powerlessness, distress, withdrawal, fear, pity and desperation and they were afraid that their daughters might be violated sexually. The mothers rejected the possibility of marriage and pregnancy for their daughters and instead planned very restricting futures for them. The mothers used various coping methods to bring meaning to their lives. Because the negative emotional responses of the mothers, nurses need to work to empower mothers to overcome these negative responses. Sex education can also play an important role especially for the daughters especially through the use of visual aids. Further, nurses should understand the learning difficulties of mentally handicapped daughters, what mothers need and also what they experience with their mentally handicapped daughters. In conclusion, nurses should understand the negative experiences of the mothers in relation to their mentally handicapped daughters' menarche help the mothers cope with the negative. emotions through real life education and counselling. In addition, there is a need for nursing interventions and an administrative system which will minimize the prejudices of society towards handicapped people.

Comparative Analysis of Structural, Process, and Outcome Indicators for Evaluating the Quality of Nursing Care

This study was done to evaluate the quality of clinical nursing care using the variables of structure, process, and outcome and to analyze the relationship between the variables. This study also explored which variables are validating indicators to evaluate the quality of nursing care. The results analyzed by multiple regression showed that, generally structural variables did not contribute to the variance in outcome scores, but process variables of nursing care contributed significantly to the outcome variable of patient satisfaction. A combination of structure and process variables more than structural variables alone. Also, patient satisfaction and hospital preference were significantly related to each other. Therefore, if nursing quality evaluation relies solely upon on structural variables such as number of available nurses and workload, it would be inaccurate because process variables of nursing care are strongly related to outcome variables and the two categories of structure and process variables helped to strengthen the relationships. Thus, it is important to focus on variables of structure, process, and outcome together in evaluating nursing care quality.

A study of the current ethical situation in organ transplantations in Korea

This primary study was done to develop an ethical guideline for organ transplantation, a life-saving treatment which helps improve the quality of life. This study tried to identify the current situation in Korea, in terms of ethical considerations in organ transplantations. This study collected basic data in organ transplantations, in the hope that procedure of organ transplantations could be developed that would be fair to both organ donors and recipients. The immediate goals of this study were : 1)to identify staff in charge of organ transplantations and their jobs in the hospital, 2) to survey whether there exists a Hospital Ethics Committee(HEC), 3) to research what consideration are formally taken in selecting recipients, and 4) to accumulate data on how consent from donors are currently obtained. The study used a survey questionnaire and received responses from 31 hospitals out of 45 hospitals where organ transplantation are being done. Organ transplantation coordinators were found in 16 hospitals, but the job description varied among hospitals. The survey showed that all 16 hospitals with and HEC that health care personnel unnecessarily dominate the committee. The study notes that HECs should be vitalized by recruiting, as members, ethicists, theologians, patients, guardians, as well as the general public outside of the hospital. The study revealed that in selecting recipients the hospital take into account ABO blood type, histocompatibility, age, waiting time, and level of patient compliance. Finally, it was shown that in the cases of living donors the transplanting hospitals seek a formal consent, whereas there are no common consenting practice established for cadaveric donors. The study concludes with three proposals. First, a nationwide institution responsible exclusively for procurement and distribution of cadaveric organs for transplantation should be established. Second, we should rebuild the national health insurance system so that have costly organ transplantation expenses are substantially covered. Last, but certainly not least, there is a need to emphasize the HEC's committment to prepare a proper ethical guideline for organ transplantation in general.

An Analysis of the Concept 'Empowerment'

Recently the concept of 'empowerment' not only attracts attention in the area of management but also is considered as an useful concept in the areas of nursing theory, practice, education, and research. The purpose of this paper is to clarify the meaning of the concept 'empowerment'. More concretely, it is to find the concept that can be applied appropriately to the area of nursing management. This study uses Walker and Avant's(1983) process of concept analysis. The attributes of empowerment based on the concept can be defined concisely as follows: 1. a dynamic and interaction process. 2. a partnership which values self and others : power shaving. 3. mutual decision-making using resources, opportunities, and authority 4. accept autonomy and responsibility Antecedents of empowerment consist of 1) mutual trust and respect, 2) education and support, 3) participation and commitment. In regard to the consequences of empowerment, it is expected to 1) positive self-esteem, 2) ability to sex and reach goals, 3) the promotion of organization's effectiveness and productivity, 4) provide effective and high quality health-care, 5) a sense of hope for the future. The concept of empowerment is certainly believed that it will help future nurses since they needs an integrated technique that can treat acute and chronic patients' nursing, individuals, families, and communities. Therefore, it is necessary to develop the instrument including the defining attributes identified in this study. Also, it is need follow up study of this concept.

The purpose of this study was to identify calls, roles and attitudes of the Christian medical staff in a modern medical system for holistic healing through belief in God's healing methods and God's view about medical treatment. The meaning of healing in the Bible is derived from Rapha in the Old Testament, it's meaning is 'heal wound', 'restore to original condition', 'repair', 'console' and 'be heal'. In the New Testament, the meaning of healing is 'to serve' and 'be in one's service' derived from Therapuein and 'preserve', 'rescue', 'save a life from death' derived from Sozo. Therefore the meaning of the healing in the Bible is restoring original completeness to the same as God's characteristics. The meaning of disease is physical, psychological, social and spiritual imbalance or disharmonious. Disease is usually depravity from moral life to immoral life and abnormal life process with accompanying specific symptoms. Medical staff were called to God's work, recognized God's will for them, and absolutely leaned on God's power to intervene and work above spatial-temporal transcendently. They use spiritual power with medical treatment skills, help sick people to possibly have dynamic and individual relation with God and help to maintain their well-being and complete healing. Attitudes of medical staff were compassion and love, virtue of modesty, strong and daring, patience with belief, healing with God's word, using spiritual insight, play, using medical knowledge and techniques, continuing spiritual training, laying on of hands and repentance.

A Comparison of Hospitals Nursing Practice in Korea and the U.S. as Experienced by Korean Nurses

Since the introduction of Western concept of nursing to Korea in late 1880s, Korean nursing practice has evolved into Korean culture. The purpose of this study was to explore and identify similarities and differences in hospital nursing practice between Korea and the United States using the grounded theory method. The data were gathered from 15 Korean nurses working in U.S. hospitals using semi-structured, in-depth interview method. Major similarities in nursing practice between Korea and U.S. existed in the 7 domains the Benner indicated in 1984. The nursing administration system was also similar in both countries. On the other hand, differences existed in how nurses execute these domains of nursing practice. These are the roles of nurses, nurse aides and family members, and the interpersonal relationships, and problem presentation and problem-solving strategies. American family members rarely participated in the patient's bedside nursing care, while Korean family members actively participated. American nurse aide participated in direct patient care, while Korean nurse aides did not. Also Korean nurses were participated more in administering and managing the patient's diagnostic and treatment modalities, while American nurses focused on the needs and demands of the patient. In terms of interpersonal relationships, American patients were more self-centered and demanding than Korean. American nurses focused more on the right of individuals while Korean nurses were more oriented to harmony with the coworkers. With inferiors and superiors, American members were more egalitarian, while Koreans tended to be hierarchical. In ways of suggesting problems, American nurses tended to pick out the problems more frequently than Korean nurses did. As to ways to solve the problems, American nurses relied heavily on explicit verbal explanations, while Korean nurses relied more on bodily action and behaviors. The results of this comparative study contribute to nursing by adding knowledge on the theory of culture care by describing the similarities and differences in nursing practice between Korea and the United States.

A Study on the Adolescent's Experiences in Domestic Violence

This study is designed to contribute to the nursing intervention of adolescent domestic violence victims by describing specifically what they go through and on this basis work out a grounded theory on the adolescent's experience in domestic violence. The data were analyzed in the framework of grounded theory as mapped out by Strauss and Corbin(1990). The major findings of this research are as follows : The core category was found to be the adaptation of "Joyudeum" or "Straight jacketing". The sub-categories identified in the process of grounded data analysis were 'menacing', 'being harassed', 'being pressed', 'being attacked', 'being strapped', 'being filled with anger', 'getting angry', 'being oppressed', 'being filled with anger', 'becoming hard', 'endurance', 'regulation', 'seriousness', 'repetition', 'being helpful', 'dependence', 'thinking oriented', 'blowing off', 'going astray', 'self-deprecating', 'shaking off, covering up', 'governing, 'being devoted', 'performing', 'being transformed', 'bolting', 'being shaken', 'withdrawing', 'disliking', 'bolting', 'being shaken', withdrawing', 'disliking', 'bitterness'. These categories were again grouped into 13 categories including 'threatening', 'straight jacketing', 'tolerance', 'violence', 'supporter', 'supporting type', 'thinking oriented', 'impulsive response', 'self-controlled', 'response', 'self-reliance', 'wandering', 'withdrawal' and 'pain'. Of these the five sub-categories 'being filled with anger', 'getting angry', 'being oppressed', 'being strained', 'becoming hard' were grouped into a core category "straight jacketing". Those adolescents brought up in a violent family go through three stages of the adaptation of "straight jacketing". that is, generation, response and adaptation. The following four theses were confirmed on the basis of repetitive relations : 1) If a subject's tolerance is strong, the episodes of the violence are frequent and serious, with the resulting straight jacketing being stronger but the subject's supporter is highly mature, the supporting type is concrete and the thinking oriented is wishful, the responses to the straight jacketing result in a self-control whose consequence is self-reliance with part of it resulted in withdrawal and pain. 2) If a subject's tolerance is weak, the episodes of the violence are frequent but not serious, with the resulting straight jacketing being weaker but the subject's supporter is immature, the supporting type is superficial and the thinking oriented is extemporary, the responses to the straight jacketing result in an impulsion whose consequence is wandering with prat of it resulted in withdrawal and pain. 3) If a subject's tolerance is weak, the episodes of the violence are frequent and serious, the straight jacketing is strong, the subject's supporter is immature and the supporting type is superficial but the thinking oriented is wishful, the responses to the straight jacketing result in an impulsion whose consequence is withdrawal and pain with part of it resulted in wandering. 4) If a subject's tolerance is strong and the episodes of the violence are infrequent and not serious, the straight jacketing is weak, the subject's supporter is immature and the supporting type is superficial but the thinking oriented is wishful, the responses to the straight jacketing result in a self-control whose consequence is withdrawal and pain with part of it resulted in self-reliance.

Caring Experience of Mothers with IDDM Children

This study is to define the caring experience of mothers with insulin-dependent diabetes mellitus children, by finding core category, contextual factors, intervening factors, and the patterns of caring, and to develop a practice theory on it. Research method followed grounded theory methodology by Strauss and Corbin. Subjects were six mothers, whose children have had insulin-dependent diabetes mellitus for 4 months to 14 years by the interview time. They were selected by theoretical sampling. Data were collected from September, 1995 to January, 1996. Interview were done by long interview took 50 minutes to 2 hours. Content of interview was recorded and transcribed later. Based on the results of previous interview, content of next interview was planned until data reached to the saturation point. Results were as follows : One hundred and forty concepts were found. These concepts were grouped into thirty-three categories, and then to ten categories. Mothers with diabetic child were revealed to face "being overwhelmed by burden". Overwhelming by burden is found to be progressed through the cycle production-coping-decrease or in crease process. Mothers showed four patterns of adaptation in caring the diabetic children. 1) If mothers felt large amount of overwhelming by burden because of difficulty of caring and unsympathizing but they had sufficient support, no condition of the child, and their coping mechanism was positive, most of them overcome with strong will, but some fell into burnout. 2) If mothers felt large amount of overwhelming by burden because of difficulty of caring, unsympathizing, insufficient support, serious condition of the child, and their coping mechanism was negative, they fell into burnout by coping with feeling. 3) In mothers felt small amount of overwhelming by burden because of little difficulty of caring and sympathizing, sufficient support, no serious condition of the child, but their coping was negative, most of them fell into burnout by coping with feeling, but some overcome. 4) If mothers felt small amount of overwhelming by burden because of little difficulty of caring and unsympathizing, sufficient support, no serious condition of the child, and their coping was positive, they overcome with strong will. On the basis of the above result, in order to help mothers take good care of their children, nursing assesment and intervention on life readjustment, and getting support should be required. Especially, through understanding mothers' personalities, individual support consistent with each of them should be required. Therefore education, counseling, mutual support and exchange of information will have to be accomplished.

Work Analysis for the Role of the Emergency Department Nurses

Nursing works in emergency department were analyzed and the importance of nursing works that the emergency department nurses perceived at university hospitals in Seoul. 12 nursing domains including 76 nursing activities were identified. The most frequently performed nursing domain was records and the most frequently performed activity in the emergency department was checking the vital sign of patients. The most important nursing activity that emergency department nurses perceived was physical crisis intervention.

An Analysis of Family Nursing Research in Korea

The purpose of this study was to analyze the trends of family nursing research in Korea. The subjects were 62 articles, 132 master theses and 20 doctoral theses published during the period between 1970-1997. 7. The results are as follows. 1. The types of research that were analyzed were factor isolating, 6.1% and situation producing, 3.7%. 2. Research designs were experimental studies, 6.1% non-experimental studies, 89.7% and qualitative research, 4.2%. Survey research was the method used in 82.7% of the papers. 3. On the research subjects, 94 papers dealt with the family member of a patient or a single person, 59 papers dealt with a patient and 7 papers dealt with whole family. 4. On the places of studies, 64.0% of the studies were done in hospital rooms, 12.9% in the communities and community facilities, 12.0% in schools, 10.7% in the home, and 0.4% an occupational setting. 5. The most frequently used family concept in the title of the articles was "family support"(59 papers) followed by family function and family burden. 6. The most frequently used family assessment tool was Family Environment Scale(FES) developed by Moos and Insel. 7. According to family nursing domain described by Murphy' and Meister' study, the subdomain, relationship of the family and disease was found 83.2%. 8. Four papers built conceptual frameworks based on various theories of researchers. Eleven papers applied family theories and five papers applied nursing theories. Based on the above findings the following suggestions are made. 1. The various domains of family nursing research should be used including in the relationship of family and health, health policy and family, transcultural family research, and theoretical approach to family. 2. Qualitative research should be used for family nursing research. 3. Family assessment tools should be developed for the Korean family.

Development of a Critical Pathway for Patients with Coronary Artery Bypass Graft

The purpose of this study was to develop a critical pathway for case management for patients who have received Coronary Artery Bypass Graft(CABG) because of Ischemic Heart Disease(IHD) which is a factor of rising medical expenses. For this study, a conceptual framework was developed through a review of the literature including six critical pathways which are currently being used in USA. In order to identify the overall service contents required by these patients and to draw up a preliminary critical pathway, 30 cases of medical records of patients who had CABG because of IHD between January, 1995 to June, 1996 at the Cardiovascular Center of Yonsei Medical Center in Seoul were analyzed. An expert validity test was done for the preliminary critical pathway and clinical validity test was also done using seven IHD patients with CABG between November 11 and 23, 1996. After these processes, the final critical pathway was developed. The results of this study are summarized as follows : 1. The vertical axis of the critical pathway includes the following eight items, nutrition medications, consultations, activity, assessments, treatments, education discharge planning and the horizontal axis includes the time from the start of hospitalization to discharge. 2. Analysis of the 30 medical records indicated that the average length of stay was 20.2 days with the average length of stay from hospitalization day to operation day being 6.2 days, and the average length of stay from operation day to discharge day was 13.9 days. Analysis of the service contents showed that the horizontal axis of the preliminary critical pathway was set from hospitalization to the 14th post operation day and the vertical axis was set to include eight items, the contents which ought to have occurred, according to the time frames of the horizontal axis. 3. As a result of the experts validity, it was found the among the total of 571 items, there was over 83% agreement for 482 items, less than 83% for 89 items, which were then deleted and a revision of the critical pathway was done. 4. A clinical validity test was done using seven, IHD patients with CABG. During the process, three patients were deleted because they were out of the criteria the investigator set. Finally, four patients were used. The result of study indicated that only one patient was discharged on the tenth post operation day. which was one day later than the expected day. Three patients were discharged later than the expected day from three days to nine days. All the cases progressed on schedule until the operation day and the first post operation day. but from the second post operation days, there were differences between the critical pathway and the actual practice. The differences came from tests, assessments, and treatments. 5. On the basis of the results of the clinical validity test, the following revisions in the final critical pathway were made : the transfer from ICU to step down ward would be the second post operation day, and the transfer to a general ward, the fifth post operation day, for patients who complained of lack of sleep from the fifth post operation day to discharge, a sleeping pill would be prescribed, skin observations would be performed routinely from immediately after the operation until the third post operation day, and would continue if there was a sign of skin injury on the fourth post operation day, and assessment of chest pain would be done from the third post operation day, and the "stairs climbing" item, expected to be done on the ninth post operation day would be deleted. In conclusion, this critical pathway is partially applicable to the care of patients with CABG but there are some parts needed to be further investigated.

The Effects of Exercise Therapy Applied in an Efficacy Expectation Promoting Program on Self-Efficacy and Metabolism: in NIDDM(Non-Insulin Dependent Diabetes Mellitus) Patients

This study was conducted to investigate whether exercise therapy applied in an efficacy expectation promoting program based on the self-efficacy theory of Bandura(1977) would increase self-efficacy and metabolism in NIDDM patients. The study design was a nonequivalent control group pre-test post-test quasi-experimental design. The exercise therapy applied in the efficacy expectation promoting program was composed of a staged exercise program, a small booklet relating personal experience with diabetes mellitus and a telephone coaching program on performance accomplishment, vicarious experience and verbal persuasion, which are all induction modes of efficacy expectation. The subjects of the study were twenty eight NIDDM patients who received follow-up care regularly through the out-patient department of endocrine medicine in one general hospital which had a diabetic clinic. Fourteen were assigned to the experimental group and fourteen to the control group. The experimental group participated in the exercise therapy applied in the efficacy expectation promoting program from three to five times per week for four weeks and the control group did not have the program. The collected data were analyzed using the x2-test, t-test, paired t-test, and Cronbach's Alpha using SPSS/PC+. The results are summarized as follows : 1. Experimental group had higher efficacy score than control group(t=5.98, p=.00). And, There was a significant different in the efficacy score before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(t=-6.42, p=.00). 2. Experimental group did not have lower level of glucose metabolism than control group(FBS : t=.32, p=.75, HbA1C : t=.60, p=.55, pc 2hrs. glucose : t=-.29, p=.78). But, There was a significant different in the amount of glucose metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(FBS : t=3.63, p=.003, HbA1C : t=4.20, p=.00 2hrs . glucose : t=1.93, p=.001). 3. Levels of lipid metabolism were partly a significant different between Experimental group and control group(triglyceride : t=-1.87, p=.07, HDL cholesterol : t=-.29, p=.77, body weight : t=1.78, p=.09, Total cholesterol : t=-2.17, p=.04). And, There was partly a significant different in the amount of lipid metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(triglyceride : t=2.50, p=.03, HDL cholesterol : t=-.43, p=.67, body weight : t=5.34, p=.00, Total cholesterol : t=2.26, p=.04). In conclusion, it was found that exercise therapy applied in an efficacy expectation promoting program was an effective nursing intervention for increasing self-efficacy and metabolism.

A Study on Self-Esteem, IADL, and Life Satisfaction in the Elderly

This study was designed to identify degrees of self-esteem, instrumental activities of daily living and life satisfaction in the elderly and to investigate the relationship between these factors thus contributing to effective nursing interventions to promote quality of life for both noninstitutionalized and institutionalized elderly. The subjects for this study included 130 noninstitutionalized and institutionalized elderly. The data were collected through personal interviews using a questionnaire, the time being from August 25 to September 7, 1997. The measurement tools were the Self-esteem Scale Developed by Rosenberg and translated by Jon, Byong Je(1974), the IADL scale developed by Lawton and Brody(1968). and the Life Satisfaction developed by Youn, Jin(1982). The data were analyzed by the S.P.S.S computer program and included descriptive statistics, t-test, One was ANOVA and Pearson correlation coefficient. The conclusions of this study are summarized as follows ; 1. There was a significant difference in Self-Esteem(t=7.05, P=.000). IADL(t=4.36, P=.000). and Life Satisfaction(t=6.63, P=.000) between the noninstitutionalized elderly and the institutionalized elderly. 2. There was a significant positive correlation between self-esteem and IADL(gamma=.4028). self-esteem and life satisfaction(gamma=.6415). and IADL and life satisfaction(gamma=.3884) in the noninstitutionalized elderly(P<.001). and between self-esteem and life satisfaction(gamma=.3883, P<.001). IADL and life satisfaction(gamma=.2501, P<.05) in the institutionalized elderly. 3. According to the general characteristics of the noninstitutionalized and institutionalized elderly : Self-Esteem of the noninstitutionalized elderly was significantly different, according to educational level(F=4.491, P=.031). job(F=2.53, P=.014). living expense load(F=2.861, P=.044). and self-esteem, and of the institutionalized elderly, it was significantly different according to educational level only(F=4.480, P=.006). IADL of the noninstitutionalized elderly was significantly different according to age(F=3.018, P=.021). living expense load(F=3.084, P=.033). pocket money(F=3.606, P=.010). and for the institutionalized elderly, it was significantly different according to age(F=2.899, P=.042), and religion(F=2.847, P=.044). Life Satisfaction of the noninstitutionalized elderly(F=2.718, P=.037) and institutionalized elderly(F=3.165, P=.030) was significantly different according to pocket money.

The Effect of Social Support on Compliance with Sick Role Behavior in Hypertensive Clients and Duration of the Effect for up to 6 months

This study was done using a Quasi-experimental research design to determine the effects of social support on compliance with sick role behavior in hypertensive patients and to know if the effect of the social support on compliance lasted for at least 6 months. The subjects consisted of 81 hypertensive patients who were registered in the Cardio-Vascular OPD at Chonnam National University Hospital. They were divided by random sampling into 42 people for the experimental group and 39 for the control group. Data were gathered from June 3, 1996 to June 10, 1997 through individual interviews using a structured questionnaire. The results of the study were summarized as follows : 1. Compliance with sick role behaviors in hypertensive clients was significantly increased in the experimental group who received social support from the nurse as compared to the control group who did not receive social support(t=15.99, p<.001). 2. The effect of social support on compliance with sick role behaviors in hypertensive clients lasted for 6 months(t=7.99, p<.001). 3. Four of six people stopped smoking in experimental group after the intervention of social support, but none of the five in control group were able to stop smoking. Fisher's Exact test showed a significant difference between the experimental and control group(x2=4.385, p<.05). Mantel Haenszel test showed that the effect of social support on stopping smoking in the experimental group lasted for six months because there were no significant differences between one month after the social support and six months after, in the number of subjects who stopped smoking(x2=1.154, p>.05). Finally, social support was effective on compliance with sick role behaviors and stopping smoking in the hypertensive clients, and the effect of social support on compliance lasted for 6 months.

A Study on urinary Incontinence of Adult Women: Preliminary Study

The purpose of this study was to identify the incidence of urinary incontinence in adult women and to identify factors related to life style and sexual intercourse that were related to incontinence. The sample consisted of 1,065 women living in Seoul or one of five provinces. Data for this study were collected from January 16 to June 23 using structured questionnaires. The Urinary Symptom Questionnaire developed by Jackson and a demographic questionnaire were used to collect the data. The data were analyzed using frequency, percentage with and SPSS/PC+ program. The major finding are as follows : 1. The distribution of age of subjects was as follows : 20-29(10.1%), 30-39(17.8%), 40-49(27.3%), 50-59(22.3%), 60-69(12.9%), 70-79(6.1%), 80-89(2.8%), more than 90(0.6%). The frequency of normal delivery, 0(15.5%), 1-2(36.0%), 3-4(29.2%), 5-6(13.0%), more than 7(6.3%). The rate of subjects with menopause was 40.8%. 2. It was reported that 50.7% of the subjects experienced urinary incontinence with stress, mixed, and urgency incontinence being 49.8%, 43.4%, 6.8% respectively. 3. The lower urinary symptoms and incidence with urinary incontinence were as follows : Daily frequency, 22.3%, nocturia, 40.8%, urgency, 71.2%, bladder pain, 47.8%, unexplained incontinence, 32.4%, nocturnal incontinence, 16.1%, and frequency of incontinence, 37.7%. In term of quantity of incontinence, drop/pants damp, 29.5%, dribble/pants wet, 20.5%, flood or soaking through to outer clothing, 1.7%, and flood or running down legs or onto floor, 0.2%. 4. The symptoms related to sexual intercourse and incidence of urinary incontinence were as follows : dry vagina, 39.1%, sex life trouble, 10.8%, pain during sexual intercourse, 27.4%, and urine leakage during sexual intercourse, 8.8%. 5. Life style problems related to urinary incontinence were as follows ; fluid intake restriction, 20.0%, affected daily task, 24.5%, avoidance of places and situations, 35.0%, interference in physical activities, 30.6%, interference in relationships with other people, 19.0%, interference in relationship with husband/companion, 8.1%, and time after attack of urinary symptoms, 76.9%. In term of the feeling about the rest of their lives the women reported : perfectly happy, 11.3%, pleased, 16.9%, mostly satisfied, 20.2%, mixed feelings, 21.0%, mostly dissatisfied, 21.0%, very unhappy 8.5%, and desperate, 1.0%. In conclusion, this study was a preliminary study to provide nursing practices guidelines for incontinence in adult women. Nurses working with adult women should develop and provide adequate care for these women.

Correlations between Weight, Body Mass Index(BMI) and Risk Factors of Coronary Artery Disease in Men and Women in their Forties and Fifties

This study was done to examine the correlations between weight, BMI and risk factors of coronary heart disease in men and women in their forties and fifties. The subjects were 412 adults, who had regular health examinations between January and December of 1996 at S-Hospital in Seoul. The data were analyzed using ANOVA, Scheffe test, and Pearson correlation coefficient. The results are as follows : 1. The men between 50 and 59 years of age had higher levels for BMI, weight, systolic blood pressure, diastolic blood pressure, total cholesterol, LDL-cholesterol, triglyceride, fasting blood sugar, plasminogen activator-1, and hemoglobin A1C than the group of women in their forties. Yet, HDL-cholesterol was lower than in the former group. 2. In the group of men in their forties, weight was significantly correlated to diastolic blood pressure(r=.22), LDL-cholesterol(r=.20), plasminogen activator inhibitor-1(r=.35) HDL-cholesterol(r=-.19). Their BMI was significantly corrected to systolic blood pressure(r=.27), diastolic blood pressure(r=.33), total cholesterol(r=.23), LDL-cholesterol(r=.26), plasminogen activator-1(r=.36) and HDL-cholesterol(r=-.25). 3. As for the group of women in their forties weight was significantly correlated to systolic blood pressure(r=.20), diastolic blood pressure(r=.22), triglyceride(r=.32), plasminogen activator inhibitor-1(r=.30) and HDL-cholesterol(r=-.37). Their BMI was significantly correlated to diastolic blood pressure(r=.25) triglyceride(r=.47), plasminogen activator-1(r=.35), fibrinogen(r=.27) and HDL-cholesterol(r=-.47). 4. In the group of men in their fifties, weight was significantly correlated to total cholesterol(r=.32), LDL-cholesterol(r=.29), plasminogen activator inhibitor-1(r=.26). Their BMI was significantly correlated to systolic blood pressure(r=.24), diastolic blood pressure(r=.22), total cholesterol(r=.34), LDL-cholesterol(r=.32), and plasminogen activator-1(r=.25). 5. In the group of women in their fifties, weight was significantly correlated to diastolic blood pressure(r=.33), total cholesterol(r=.21), LDL-cholesterol(r=.20), plasminogen activator inhibitor-1(r=.43) and HDL-cholesterol(r=-.21). Their BMI was significantly corrected to systolic blood pressure(r=.25), diastolic blood pressure(r=.40), total cholesterol(r=.24), LDL-cholesterol(r=.24), triglyceride(r=.22), and HDL-cholesterol(r=-.30). The above findings indicate that the BMI was more predictive than weight as a risk factor for coronary artery disease for men and women in their forties and fifties.

Nursing Students' Perceptions on Diet and as Environmental Factors Related to Cancer Risk Factors

The purpose of this study was to identify how students majoring in nursing perceive cause of cancers and the effects of diet for preventing cancers. Data for the study were collected by 651 nursing students, who were registered in the second and third year in three technical colleges and third and fourth year in two universities. The research instruments included items on general characteristics of subjects, items about the degree of perception of the frequency of cancer onset and items on the perception of mortality, risk factors, preventive diets, knowledge, and high risk factor for cancer in specific body areas. The findings of this study are as follows : 1. Almost all subjects(92.8%) reported that the frequency of cancer onset increases and that it is 93.9% for people over 40. Degree of perception about cancer mortality was low at 33.0%. 2. As far as the perception of risk factors for cancer onset was concerned, smoking, stress, heredity, family history, and alcohol were rated high, over 80.0%. Risk factor in clouding, virus, hormones, pesticides were rated as low. 3. As to the perception of risk factor for body area as associated with diet salted and scorched food were rated at 44.5% for stomach cancer, alcohol, 50.4% for liver cancer, smoking, 72.8% for lung cancer, pregnancy times, 25.3%, and marriage age, 23.0% for uterine cancer, and no delivery experience, 40% for breast cancer. 4. The knowledge score for cancer was between 12 and 36, with a mean score of 26.75(SD=4.13). There was a statistically significant difference between experience in caring for cancer patients during clinical practice and knowledge score(t=3.09, p=.002).

The Sexual Satisfaction in Married Women

This study was conducted to explore the level of sexual satisfaction in married women. This is a descriptive study. The data was collected from July 19 to Aug 10, 1997 utilizing a questionnaire developed by researchers. Sexual satisfaction(alpha=.926) was measured by the Sexual satisfaction scale(Kim at el, 1997) which is consisted of 17-items. And data was analysed using SPSS/PC+ 7.5. The following is a summary of the study. 1. The study surveyed a total of 400 married women from Seoul, Kyungki do, and Pusan. The mean age of the subjects was 38.61 years with the range from 22 to 69 years. A large proportion of the subjects(69.3%) had received a high school education or further education. In terms of the marital satisfaction, 68.6% of subjects were moderately or highly satisfied; 6.8% were dissatisfied. 2. The mean score and standard deviation of the sexual satisfaction was 42.74+/-10.42 with the range from 17 to 68 score. 3. The variables influencing on sexual satisfaction were age, education level, income, job, stress, and perceived marital satisfaction.

A Curriculum Development on the Disaster Management

The various and serious types of disaster occur everyday and everywhere on the earth. There is no doubt that it is very timely to discuss about the effectiveness and preparedness of disaster. The purpose of this study is to develop a curriculum on the disaster management through reviewing disaster concepts and the disaster management system. For the empirical relevance of the study, researchers participated in a couple or more disaster training program, reviewed references, and consulted to the experts working on action parts in the area. As a result, the 'Integrated Disaster Management System Model(IDMSM)' was designed, in which four dimensions were explained. Then the 'Disaster Curriculum Model(DCM)' was explored with its theoretical framework based on the system model. The developed curriculum is composed of four levels ; the introductory course, the fundamental course, the advanced course, and the expert course. From this DCM, basically the course-outlines of two subjects in the introductory course, 18 subjects in the fundamental course(5 of direct services, 13 of indirect services) were developed. Also each course-outline was explored by its course objective, learning objectives, contents, and its length. Finally to make the most of the results, suggestions are proposed. The governmental considerations on the policy should support the systematic and integrated educational program to practice, appointing [Disaster School] or [Disaster Training Center] of relevance and accountabilities. The further study should explore the higher levels of the DCM through interdisciplinary efforts, and develop the text materials.
